Proof of Sickness. Sick leave with pay is only payable because of sickness or injury and employees who are absent from duty because of sickness may be required by the Employer to prove sickness. Failure to meet this requirement can be cause for disciplinary action. Repeated failure to meet this requirement can lead to dismissal. A doctor’s certificate may be requested for each leave of more than three (3) consecutive work days.

Proof of Sickness. A) Xxxx leave with pay is only payable because of sickness or injury and employees who are absent from duty because of sickness may be required by the employer to prove sickness. Failure to meet this requirement can be cause for disciplinary action. Repeated failure to meet this requirement can lead to dismissal. A doctor’s certificate may be requested for each leave of more than two (2) consecutive work days. B) The Employer may request an employee to seek a second medical opinion in the situation of a non-physical illness which duration is greater than ten (10) consecutive work days. The second opinion will be obtained by a physician who is appointed jointly by the employee’s physician and a physician representing the Employer. The employee shall specify whether the physician giving the second opinion will be female or male. The Employer shall bear all costs related to this request.
Proof of Sickness. Provided that no employee employed under the provisions of this Agreement shall be entitled to the benefits of this clause unless he/she produces proof of his/her sickness satisfactory to this employer. If requested such proof of sickness may be in the form of a medical certificate.

The University also reserves the right to refer the employee to a physician satisfactory to the University, the Union and the employee. The costs of such a referral shall be borne by the University.
Proof of Sickness. Management may at any time require any employee to provide a physician's certificate proving sickness requiring absence from work. The Employer shall pay for the physician’s certificate.

A doctor's certificate may be requested for each leave of more than three (3) consecutive work days. 35.07 When an employee is on paid sick leave, all benefits of the Agreement will continue to accrue. 35.08 Employees who are absent due to sickness beyond their accumulated sick leave credits shall be placed on an unpaid leave of absence until they are in receipt of long term disability benefits.
