Background and Scope. The Health and Care Commissioning Board will have overarching responsibility (subject to reserved matters) for all matters relating to the Integrated Health and Care Fund (Pooled Budget and Aligned Budgets) as set out in the Partnership Agreement Relating to Integrated Health and Care Commissioning Arrangements (the Partnership Agreement) between Salford City Council (SCC) and NHS Salford Clinical Commissioning Group (SCCG). This document sets out commissioning arrangements where GPs via SCCG and Councillors of SCC can contribute to, and make decisions with regards to health and social care matters. The ethos of partnership working will underpin the programme of work, recognising that on occasion, difficult decisions may be required to benefit the Salford population. In respect of the Integrated Health and Care Fund (Pooled Budget), the Health and Care Commissioning Board (‘the Board’) will sit as a joint committee established under Regulation 10(2) of the NHS Bodies and Local Authorities Partnership Arrangements Regulations 2000 (“the 2000 Regulations”). In respect of the CCG Aligned Budget element of the Integrated Health and Care Fund (Aligned Budgets), the Board will sit as a Committee of the Governing Body of SCCG on which there is Council representation. For the avoidance of doubt, insofar as the Board sits as a joint committee under the 2000 Regulations, SCC and/or SCCG are delegating the making of commissioning decisions to the Board and not to their individual representatives on the Board. For the avoidance of doubt where the Board sits as a Committee of the Governing Body of SCCG, SCCG is delegating the making of commissioning decisions to the Board collectively and not to their individual representatives on the Board. The Board also acts as a venue in which decisions may be taken by the relevant SCC or SCCG Officer or SCC Member in respect of Retained Decisions. Retained Decisions relate to statutory duties and are decisions only exercisable by a specified Officer or SCC Member as detailed in Schedule 4 of the Partnership Agreement. For the avoidance of doubt, the relevant SCC or SCCG Officer or SCC Member retains an absolute discretion to take Retained Decisions outside of the venue of the Board. Background and Scope. Early Head Start (EHS) is a program for low-income pregnant women and families with infants and toddlers. The program is based on research that demonstrates the impact of early intervention through high quality programs that enhances opportunities for success among families and young children. Reauthorized by the Head Start Act of 1992, EHS has multifaceted purpose. EHS seeks to enhance children’s physical, social emotional and cognitive development. In addition, EHS works to enable parents to be better caregivers of and teachers to their children. XXX also works with parents to set and meet their own goals, including goal of economic independences. In order to achieve the multi-faceted purpose of EHS, the program provides, early, continuous, intensive, and comprehensive child development and family support services to families and children. Services provided include, but are not limited to: physical, mental and oral health, nutrition, early childhood education, financial literacy, family engagement and strengthening, marriage education and job training. The services provided to families and children are designed to reinforce and respond to the unique strengths and needs to each child and family. The Proposed model by DCF seeks to identify strong community-based family support providers and child care to serve EHS eligible children participation in child care subsidy, Kansas Child Care assistance program. In addition to supporting vulnerable children and their families, Kansas’ goal is to increase the quality of child care environments in which these children are enrolled. KEHS grantees applying to participate in the DCF Kansas Early Head Start-Child Care (KEHS- CC) Partnership grogram and their partners must meet the Federal Head Start regulations and applicable state child care regulations. The Bureau of Labor Statistics (BLS) intends to provide access to the Census of Fatal Occupational Injuries (CFOI) research file within a secure virtual data enclave to [XYZ Organization], hereinafter "the recipient," for statistical purposes only, in accordance with the provisions of this agreement. The data will be used only in aggregated multivariate statistical analyses for a research project specified in Section IV of this agreement. The CFOI research file contains coded data items that include the demographic characteristics of the decedent and information on how the fatal incident occurred. The CFOI data do not include personal identifiers. In addition, there are no State codes in the CFOI research file. Each record is classified into one of four Census region codes to facilitate analysis by region of the country.
Background and Scope. This Part 2 of this Agreement sets out the general terms and conditions relating to inter-Party Data Processing Activities which the Parties agree to meet. Part 1 of the Agreement sets out the specific details of what each Party has agreed to in respect of any intended inter-Party Processing of Personal Data. The Parties agree that no Party will access or otherwise Process Personal Data that solely relates to any other Party’s individual Processing purpose, which is outside of the scope of the Processing set out in Part 1 of the Agreement. To the extent that any other agreement between the Parties in relation to these Data Processing activities contains any provisions which govern the Processing of Personal Data by the Parties, the Parties agree and acknowledge that the provisions of this Agreement shall prevail in the event of any conflict or inconsistency. Data Protection Legislation requires that “[w]here two or more controllers jointly determine the purposes and the means of Processing they shall be joint controllers.” It also requires that Joint Controllers determine their respective responsibilities for compliance “…in a transparent manner…by means of an arrangement between them…” This Agreement meets the requirement of having an arrangement. All Parties shall meet the additional transparency requirements under clause 13. The Independent Living Program under Rehabilitation Services (RS) provides funding to community not-for-profit or faith based organizations meeting the eligibility requirements for the provision of independent living services to persons with significant disabilities. Grantee must successfully communicate a comprehensive approach to provide core and allowable independent living services to persons with a disability and the community served by the organization as it relates to the implementation of the program’s standards. The purpose of the Independent Living Program is to promote a philosophy of independent living , including a philosophy of consumer control (defined as vesting the power or authority of the organization in individuals with disabilities, including individuals who are or have been recipients of IL services), peer support, self-help, self-determination, equal access, and individual and system advocacy, in order to maximize the leadership, empowerment, independence, and productivity of individuals with disabilities, and the integration and full inclusion of individuals with disabilities into the mainstream of American society. The purpose of this program is to provide Kansas Independent Living funding comprised of State General Funds, and federal funds (including funds provided under Title VII Part B of the Rehabilitation Act, and Social Security Reimbursement) to organizations meeting or providing assurance they will meet the requirements established under 34 CFR 366 to implement one or more Center(s) for Independent Living in order to ensure statewide coverage. The goal of this program is to maximize the leadership, empowerment, independence, and productivity of individuals with disabilities, and the integration and full inclusion of individuals with disabilities into the mainstream of American society.
