DATA TO BE SUPPLIED TO THE UNION. FTG will supply the Union Committee with the following information semi-annually upon request, and send a copy to the Local Union Office:
1. Employees who acquire seniority.
2. Employees by rate and classification.
3. Employees transferred into or out of the Bargaining Unit.
4. Employees on leave of absence.
5. Employees on Sickness and Accident and Compensation on the date of occurrence.
6. Layoffs and recalls.
7. Employees who have lost seniority.
8. Employees who are discharged.
9. Names, addresses and postal codes of all retired and active employees.
DATA TO BE SUPPLIED TO THE UNION. The Company will supply the Union Chairperson and Local Union Financial Secretary with the following information at the end of every month, (unless otherwise requested by the Chairperson or his/her designate),
1. Employees who acquire seniority.
2. Employees by rate with work codes.
3. Employees transferred into or out of the bargaining unit. (The Company will notify the Union Chairperson within one week of a transfer).
4. Employees on leave of absence.
5. Employees on Sickness and Accident and Compensation on the date of occurrence.
DATA TO BE SUPPLIED TO THE UNION. Amended -The Company will supply the Union committee with the following information every 6 months and send a copy to the local Union office:

DATA TO BE SUPPLIED TO THE UNION. 7.01 The Employer will supply to the Union Chairperson and the Local 3000 office, at the end of each month, the following information:
1) Employees who are in the bargaining unit regardless of whether or not they paid dues in the month.
2) Employee’s number and their hourly rate and classification
3) Employees transferred into or out of the Bargaining Unit
4) The number of hours worked in a month.
5) Employee’s status (i.e. at work, on vacation, WCB, any other leave of absence) and the date of occurrence.
6) Layoffs and recalls.
7) Employees who have lost seniority.
8) Names, addresses, phone numbers and postal codes of all active employees. Monies paid to UNIFOR Local 3000 will be transferred via Electronic Funds Transfer.
DATA TO BE SUPPLIED TO THE UNION. 18.01 By signing the Company's Privacy Policy consent form Bargaining Unit employees authorize the Company to provide the following personal information to the Union Committee on a semi-annual basis:
(a) Employees who are in the bargaining unit regardless of whether or not they paid dues.
(b) Employee's number and their hourly rate and classification.
(c) Employees transferred into or out of the Bargaining Unit.
(d) The number of hours worked per month.
(e) Employee's status and the date of occurrence.
(f) Layoff and recalls.
(g) Employees who have lost seniority.
(h) Names, addresses, and postal codes of all retired and active employees.
DATA TO BE SUPPLIED TO THE UNION. 5.01 The Company agrees to supply the Union with each month's remittance an alphabetical list of all employees' names, seniority, and employee numbers if applicable, hourly rate and employees transferred into or out of the bargaining unit.
5.02 From time to time a list of Supervisors will be given to the Plant Committee as changes occur.
5.03 The Company agrees to supply the CAW Area and National Offices with a semi-annual alphabetical list of all employees' names, seniority, employee number and hourly rate. In addition, upon receipt of written authorization from the employee, the Company will also supply telephone numbers, addresses and postal codes including noting any change in addresses, individuals who retire or terminate, and new employees hired. New employees will be provided with the foregoing authorization cards when hired.
5.04 The Company agrees to supply the Union with employees' status (i.e., at work, on vacation, LTD, WCB, retired in the quarter, any other leave of absence) and the date of occurrence, upon request but not oftener than quarterly.
DATA TO BE SUPPLIED TO THE UNION. 5.1 The Company will supply, monthly, to the Union the following information concerning employees, probationary employees, part-time employees, and employees working during school vacation:
(i) who was hired;
(ii) who completed their probationary period;
(iii) each employee’s hourly rate;
(iv) each employee’s address;
(v) who went on leave of absence;
(vi) who was off on disability or compensation and the date each went off;
(vii) those laid-off and recalled;
(viii) those terminated or resigned; and
(ix) those who retire.
5.2 In January of each year, the Company will supply the Union with a list of names and addresses of all active employees.
5.3 The Company will inform the Union when supervisors change.
5.4 The Union will inform the Company, in writing, of changes to any xxxxxxx or committee members.
DATA TO BE SUPPLIED TO THE UNION. 6.1 The Xxxxxxx Medical Centre will supply the labour management committee with the following information at the end of every month and send a copy to the local union office:
1. Employees who are in the bargaining unit regardless of whether or not they paid dues in the month.
2. Employees number, hourly rate and classification.
3. Employees transferred into or out of the bargaining unit.
4. The number of hours worked in the month.
5. Employee status (i.e. at work, on vacation, weekly sick benefits, LTD, WCB, retired in the month, any other leave of absence) and the date of occurrence.
6. Layoffs and recalls.
7. Employees who have lost seniority.
DATA TO BE SUPPLIED TO THE UNION. 7:01 The Company will supply to the Union committee with the following information at the end of each year unless a lesser time period is stipulated herein every month and send a copy to the local union office:
1. Employees who acquire seniority - monthly.
2. Employees by rate and classification – yearly.
3. Employees transferred into or out of the Bargaining Unit – on the day of the occurrence.
4. Employees on leave of absence – as they occur.
5. Employees on Sickness and Accident and Compensation – on the date of occurrence.
6. Layoffs and recall – as they occur.
7. Employees who have lost seniority – on the date of occurrence.
8. Employees who are discharged – at the time of occurrence.
