Dental Care Insurance. During the 2016-2017 open enrollment, members eEligible unit members may select one (1) of the two dental plan options, which includes the following: Option 1: (current plan) – Delta Dental (PPO – no changes) Option 2: (Enhanced Plan) – MetLlife (HMO – includes orthodontia & dental implants) Details on the plan benefits are available in the Benefits Enrollment Guide which is available on the District website at xxxxx://xxxxxxxx.xxx/hr/benefits/benefits- enrollment-forms.php. Vision care insurance Life and Accidental Death and Dismemberment (AD&D) insurance group plan ($50,000) or ($25,000) if eligible unit member is age 70 and over group plan. (District paid); Income protection (long term disability) – employees receive 66.67% of their monthly earnings up to a maximum monthly benefit of $3,000; the plan includes an elimination period of 140 calendar days with a maximum benefit period of 12 months. (District paid); A choice of the following two eEmployee assistance programs (EAP): • Anthem EAP, which offers up to six (6) free counseling visits per person, per issue, per year, and is available to all District 52 employees and their eligible dependents; or 53 • Xxxxxxx Employee Connect EAP, which offers up to five (5) 54 free counseling visits per person, per issue, per year, and is 55 available to full-time benefit eligible employees up to three 56 free consultations per year with a qualified District-provided 57 psychologist. (District paid); 58
Dental Care Insurance. 1. The City will continue to offer one (1) City sponsored Dental plan. The maximum full-time employee contribution for employee only coverage will be 30% of premium and for employee plus dependents is 40% of premium.
2. The City may make changes in the plan providing it consults with the FOP prior to implementing any such changes.
Dental Care Insurance. 1. Subject to Paragraph 2 and 3 below and Section E below, the District shall pay the premium for dental insurance coverage for only the individual member:
2. Full time employees hired prior to July 1, 2017 in positions A & B, as defined in Article XV, who have dependent coverage as of June 30, 2017, may retain dependent coverage with the premium paid by the District.
3. Full time employees hired prior to July 1, 2017 in position C, as defined in Article XV, may retain dependent coverage with the premium paid by the District.
Dental Care Insurance. Employees electing to participate in the group dental insurance plan shall advise the Employer in writing of this intent and shall make arrangements satisfactory to the Employer for the payment of the required monthly premium, if any.
Dental Care Insurance. The Employer will make available a group insurance program covering certain dental expenses for full time employees and their eligible dependents. The dental care insurance program currently provides coverage as set forth in Appendix “B.” The specific terms and conditions governing the group dental care insurance program are set forth in detail in the Master Policy or Policies governing this program as issued by the carrier or carriers. Employees participating in the group dental care insurance plan shall complete the applicable forms and make arrangements satisfactory to the Employer for the payment of the required monthly premium, if any.
Dental Care Insurance. For the duration of this agreement, the Employer shall provide all employees, who are regularly scheduled to perform twenty (20) hours of work per week and their eligible dependents with dental care insurance. Such dental coverage shall include identical benefits to the coverage in effect on June 30, 2006. The Board shall pay the full cost of such dental care insurance.
Dental Care Insurance. 1. The District shall pay the full premium for the following coverage for the individual members and their dependents:
a. Coverage shall be in accordance with that provided by Benefit Administrators, Inc., as outlined in the Master Agreement between the North East School District and Benefit Administrators, Inc. effective February 1, 2008 with edited date of June 1, 2014. The maximum benefit will be $1,500 per year based on a calendar year.
b. Periodontics – a 50% benefit to a yearly maximum of $1,000.
c. Orthodontics benefits will be paid up to 50% or a lifetime maximum of $1,500 for dependent children up to age 19.
Dental Care Insurance. The District shall make available a dental care insurance plan that will include the fol- lowing benefits:

3. The District shall be responsible, as follows, for the payment of premiums for Em- ployees in the following categories: Single; Two Person; or Full Family
4. The Employee and the District will share the cost of the dental care insurance cover- age as follows:
Dental Care Insurance. The payment of contributions for the plan as set out in 9.1 above shall be as follows:
