HEALTH, DENTAL, AND DISABILITY INSURANCE. 1. Benefits Eligibility Employees are eligible to participate in the Star Tribune Comprehensive Welfare Benefit and Cafeteria Plan (“Comprehensive Welfare Plan,” or “Plan”), and each of its component programs, on the same basis as nonunion employees, provided they have completed the 28-day initial waiting period following their hire date, as required by the plan and its component programs. The Comprehensive Welfare Plan includes the Health Care Reimbursement Account, the Dependent Care Reimbursement Account, the HSA Contribution Feature and the ability to elect pre-tax premiums for eligible component programs. Component programs under the Comprehensive Welfare Plan include medical, dental, vision, basic life, supplemental life, long-term disability and accidental death and dismemberment insurance programs. The Publisher is the “Plan Administrator” of the Comprehensive Welfare Plan. It is intended that the Plan contain all necessary provisions to comply with Section 125 and any other applicable section of the Internal Revenue Code. The Plan shall be terminated in the event that the Internal Revenue Service or a court determines that Plan contributions are not proper business deductions for the Publisher. The parties understand that Congress or the IRS may change the rules that apply to the Plan at any time. It is expressly agreed that the Plan shall be amended from time to time to address such changes. [The Publisher may change the terms, conditions and providers or these plans without bargaining with the Guild so long as such changes are equally applicable to nonunion employees.]
HEALTH, DENTAL, AND DISABILITY INSURANCE. The Superintendent shall be eligible for group health, dental and disability insurance benefits to the same extent as other employees of the Town of Hopkinton in accordance with and subject to the provisions of Massachusetts General Laws Chapter 32B and/or any other limitations applicable to such coverage. The Superintendent shall be expected to make the same contribution towards such insurance coverage as is expected from all other employees of the Town.

The Executive Director shall be eligible for group health, dental, and disability insurance benefits to the same extent as other employees of the Cape Cod Lighthouse Charter School in accordance with and subject to the provisions of Massachusetts General Laws Chapter 32B and/or any other limitations applicable to such coverage. The Executive Director shall be expected to make the same contribution towards such insurance coverage as is expected from all other employees of the School.
HEALTH, DENTAL, AND DISABILITY INSURANCE. Executive shall be entitled to participate in all health, dental and disability plans or programs made available by the Company to its employees generally. In addition, executive will be reimbursed for an annual comprehensive physical conducted by the Xxxxxx Clinic.
HEALTH, DENTAL, AND DISABILITY INSURANCE. At such time as health insurance is available to other executives employed by the Company, Executive and his family, to the extent that Executive and each family member is eligible under the applicable plan or policy, shall be provided with health and/or dental insurance, as the case may be, comparable to that maintained by the Company for its executives generally. Until such time, it shall be Executive's responsibility to provide health coverage for him and his family.
HEALTH, DENTAL, AND DISABILITY INSURANCE. (A) HEALTH – The Employer agrees to pay, based upon the conditions below, premium contributions as specified below, on behalf of each eligible employee covered by this Agreement for the following health care coverages: Through January 1, 2014, the Employer shall continue to calculate the composite premium rate for Plan A (MESSA Choices) by aggregating the monthly premium charges for the medical premiums only and dividing that sum by the number of bargaining unit members enrolled in MESSA Choices at that time. Enrolled employees agree to pay twenty percent (20%) of the composite medical premium. From January 1, 2014 through June 30, 2014 the 2013 state caps rates will be used to determine the employee monthly premium contribution responsibility. From July 1, 2014 through June 30, 2015 the 2014 state caps rates will be used.

HEALTH, DENTAL, AND DISABILITY INSURANCE. (A) HEALTH – The Employer agrees to pay, based upon the conditions below, premium contributions as specified below, on behalf of each eligible employee covered by this Agreement for the following health care coverages: The employee’s monthly premium contribution responsibility (effective 7/1/12 to 9/30/12) will be based on the census as of July 1, 2012 on the MESSA Choices II medical rates effective July 1, 2012. That composite medical rate for this period is $256.33. Effective 7/1/13, a new monthly premium contribution responsibility will be calculated based on the new insurance rates effective 7/1/13 using the same method as in Section A of this Article. This would remain in effect until June 30, 2014. This section is contingent on the continued validity of Public Act 152 of 2011.
HEALTH, DENTAL, AND DISABILITY INSURANCE. The Authority shall offer Executive health and dental insurance selected by the Authority. The Authority will pay one hundred percent (100%) of Executive’s premium for health and dental insurance. Executive shall also have the option of adding a spouse and up to two dependent children to the health and dental insurance with the Authority paying one hundred percent (100%) of the premiums for such individuals. The Authority shall have the unilateral ability to select, change, modify, or alter the health and insurance plans, including, but not limited to, plan benefits, co-payments, co-insurance, etc. In addition, the Authority shall provide Executive with long term disability insurance, the terms of which will be negotiated and selected by the Authority from time to time.
