Health Leave Without Pay. An employee who is unable to perform the duties because of personal illness or other disability shall, upon request, be granted health leave without pay (HLWOP) upon the exhaustion of accumulated sick leave. Application for health leave without pay shall be made in writing to the Human Resources office and shall be accompanied by verification from a practicing physician, dentist, or other person licensed to perform customary health services, including an estimated time of recovery.

Health Leave Without Pay. Health leave without pay for a period not exceeding twelve (12) months shall be granted to an employee who does not qualify for health leave with pay or who is unable to return to work due to illness at the termination of the period for which health leave pay is granted. Extension in excess of twelve (12) months may only be granted by mutual agreement between the parties hereto.
Health Leave Without Pay. 7.12.1 A health leave without pay may be requested by a unit member and shall be granted for a period not to exceed four semesters . Such leave request shall contain a statement from a licensed physician indicating the nature of the illness and the period of confinement.

  • Maternity Leave Without Pay (a) An employee who becomes pregnant shall, upon request, be granted maternity leave without pay for a period beginning before, on or after the termination date of pregnancy and ending not later than eighteen (18) weeks after the termination date of pregnancy.
