Maintenance of Personnel Files. The Employer shall maintain in the Human Resources Department one (1) personnel file for each employee. The personnel file shall contain information pertinent to an employee’s qualifications, record of employment and other information required for business and legal purposes. Access to and use of information in the employee’s personnel file shall be restricted to a business or legal purpose. Materials derived from any unknown sources will be excluded from the personnel file. Performance, corrective action or disciplinary documents relating to the employee that are not included in the official personnel file may not be used as evidence in any grievance arbitration regarding discipline of the employee.
Maintenance of Personnel Files. The personnel files of each employee shall be maintained at the District Central Administrative Office.

The City agrees to maintain confidential medical information in accordance with the Americans With Disabilities Act (ADA) and Health Insurance Portability and Accountability Act (HIPAA) and agrees to maintain any other confidential information as required by any other statute requiring maintenance of confidential information.

Maintenance of Personnel Files. 1. There is one official personnel file for each employee kept in the Office of the Superintendent of Schools. A cumulative working evaluation file, in which all material is dated, will be kept by the Principal. Material from the cumulative working evaluation file must be forwarded to the Superintendent on an annual basis by the first day of classes of the new school year. Any material for the preceding school year not forwarded by the first day of classes of the new school year cannot become part of the official personnel file.
2. The official personnel file kept in the Office of the Superintendent of Schools includes:
a. Original application, transcripts, and related papers.
b. Correspondence, memoranda, evaluation, and other papers relating to the employee’s employment, promotion, transfer of position, or change in professional qualifications.
c. Awards and commendations relevant to total employee role and other data referred by the employee, through Principal, or by the Principal with employee’s knowledge.
3. Awards and commendations relevant to total employee role and other data referred by the employee or by an administrator shall be routed to the Office of the Superintendent as follows:
a. When referred by employee: data shall be given to the Principal with request that it be forwarded to the Superintendent. Copies of such data should be kept in the Principal’s office file.
b. When referred by administrator: employee shall be informed of any material being routed to the Superintendent for official file in that office. Employee should sign all such material, indicating that he has seen it and is aware that it is being forwarded to the Superintendent of Schools. Copies of such data should be kept in the Principal’s file as well.
4. No material of a derogatory nature will be placed in the official personnel or the cumulative working file unless the employee has had the opportunity to read and discuss same with the administration. These matters shall be promptly called to the attention of the employee involved and he may submit a written answer, which shall be attached to the original material.
5. Official grievances filed by any employee under the grievance procedure as outlined in this Agreement shall not be placed in the personnel file of the employee or the working evaluation file kept by the Principal; nor shall such grievance become a part of any other file or record which is utilized in the promotion process, nor shall it be used in any recommend...

The Employer agrees to maintain confidential medical information in accordance with the Americans With Disabilities Act and HIPPA Privacy Act and agrees to maintain any other confidential information as required by any other statute requiring maintenance of confidential information.
Maintenance of Personnel Files. The University will maintain in the Human Resources Department one (1) personnel file for each employee. The personnel file shall contain information pertinent to an employee’s qualifications, record of employment and other information required for business and legal purposes. Access to and use of information in the employee’s personnel file shall be restricted to a business or legal purpose. Materials derived from any unknown or anonymous sources will be excluded from the personnel file. In addition to the official personnel file, the University may maintain, as needed, additional employee files relating to ESEs including supervisory files, payroll files and medical files.
Maintenance of Personnel Files. The School Corporation shall (from the execution date of this Agreement) maintain a single personnel file on each teacher in the Administration Building and may maintain an additional single personnel file at the building(s) where the teacher teaches. All personnel information concerning each teacher shall be kept in that teacher's referred-to personnel file(s) (hereinafter referred to in singular).
1. Teachers shall be permitted to see their personnel file and may duplicate any information in the file, except information secured by the Board in the course of employing the teacher shall not be made available.
2. A copy of each formal evaluation shall be placed and kept in the Human Resource Office’s personnel file.
3. If any derogatory material is to be placed in a personnel file, the teacher shall first have the opportunity to read such material and attach a statement of clarification or rebuttal if the teacher so chooses. The teacher shall initial or sign and date such material, which shall not be any indication of any fact other than his/her knowledge of the existence of such material; this sentence shall operate prospectively from the date of this Agreement, and each individual teacher is encouraged to examine his/her personnel file and may initial or sign and date any documents which may have been placed herein prior to the date of this Agreement.
4. Upon request of a teacher, any derogatory material may, at the discretion of the School Corporation, be removed from the teacher’s file if such material has been on file for at least three (3) years and shall be removed after seven
