Review of Personnel Files. Every member shall be allowed to review any of his/her personnel files except "confidential law enforcement records" and "trial preparation records" as defined in Ohio Revised Code Section 149.43 at any time, upon request and reasonable notice. Such request shall be made to the supervisor directly responsible for maintenance of such files. Review of the files shall be made in the presence of such supervisor or the supervisor's designated representative. For the Division master personnel file, the request shall be made to the member's Subdivision Deputy Chief or his/her designated representative. Any member, or the member's Lodge representative, may copy documents in the member's file. The City may levy a charge for such copying, which charge shall bear a reasonable relationship to actual costs. A member will be notified in writing any time records within his/her personnel, background, IAB, and/or payroll file(s) are requested, as a public records request pursuant to Ohio Revised Code Section 149.43, provided the City determines that the request is proper under applicable law. A member may request copies of any records provided under this paragraph, and these copies shall be provided at no cost to the member.
Review of Personnel Files. Pursuant to the Government Code, each employee is entitled to review his/her personal history file maintained in the employee’s department or in the Human Resources Department by following the procedure set forth by the Human Resources Director or the Department Head. All employees are further entitled to view any material placed in said files.
Review of Personnel Files. Employees and/or their authorized union representatives shall have the reasonable right to review the contents of their personnel files. Employees shall have access to all materials in their files except those prohibited by ORC Section 1347.08 (C). Such review may be made during normal working hours. Employees who are not normally scheduled to work when the Personnel Office is open may request to review their files through their supervisor. The supervisor will make the file available in a reasonable amount of time. Reasonable requests to provide one copy of documents in the files shall be honored at no charge. The employee’s personnel file shall not be made available to any organization or person other than the Employer or its agents, without the employee’s written authorization unless pursuant to court order, subpoena, or request made pursuant to the Ohio Public Records Act.
Review of Personnel Files. Each teacher shall have the right, upon request, to review the total contents of his/her personnel file (excepting confidential university credentials). The review will be made in the presence of the administrator (or his/her designee) responsible for the safekeeping of such files. No material originating after initial employment will be placed in a teacher’s file unless the teacher concurrently receives a copy of the material. The administration shall have the teacher initial or sign the original or file copy of evaluative material for the purpose of verifying the teacher’s knowledge of the item being placed in the file. Routine non-evaluative communications to the teacher shall include notation that a copy is to be placed in the file. The employee may submit a written notation regarding any material and the same shall be attached to the file copy of the material in question. Such attachments must be submitted within ten

The employee shall have the opportunity to submit a written statement responding to any written reprimand issues. The employees’ responding statement will be entered in the personnel file included with the written reprimand.
Review of Personnel Files. Section 1. Every member shall be allowed to review his personnel file at a reasonable time during regular office hours, upon the written request to the Sheriff, and in the presence of the Sheriff or his designated representative.
Review of Personnel Files. A. Every Bargaining Unit member shall be allowed to review their personnel files at any reasonable time upon written request to the University. Bargaining Unit members may also authorize their Union Representative or attorney to review the personnel file. Such request shall be made to the Manager of Human Resources for Public Safety directly responsible for maintenance of such files. No information in a Bargaining Unit member’s personnel file will be shared with anyone outside the University, except as required by law. Once every calendar year, a Bargaining Unit member may make one (1) copy of the documents in their file at the University’s expense.
B. If a request is made to inspect and/or copy records within the bargaining unit member’s personnel file pursuant to Section 149.43 of the Ohio Revised Code, and if the University complies with this request, the University shall provide notification to the Bargaining Unit member of the nature of the request.

Review of Personnel Files. 1. A teacher shall have the right to review the contents of his/her personnel file upon reasonable notification and to receive copies of any documents contained herein. The teacher may be charged a reasonable copying fee for more than one copy of any individual document(s) requested. A teacher shall be entitled to have a representative of the Association accompany him/her during such review.
2. No material derogatory to a teacher's conduct or service to the district shall be placed in his personnel file until the teacher has had the opportunity to review the material. The teacher shall acknowledge that he/she has had the opportunity to review the material by affixing his/her signature to the copy to be filed with the expressed understanding that the signature does not indicate agreement with the contents. The teacher does have the right to submit a signed written response to such material and his/ her response will be reviewed by the appropriate supervisor(s) and attached to all copies. In the event a teacher refuses to sign any document related to his/her performance or evaluation prior to placement in his/her file, the Association will be notified of the same in writing and the signed written response will fulfill the requirement for review by the teacher.
3. All documents, communications, and records dealing with processed grievances of this contract shall be filed separately from the personnel files of the participant and shall not be forwarded to any prospective employer of the grievant.
Review of Personnel Files. 8.1 Each employee shall have the right upon request to review the contents of their personnel file maintained at the administration building. A representative of the Union may, at the employee's written request, accompany the employee in this review. The review shall be made in the presence of the person responsible for the safekeeping of the employee’s file. The Employer shall reproduce for the employee, one (1) copy of any material requested from such files.
8.2 The employee shall have an opportunity to read all material placed in their personnel file, subject to excluding privileged information as hereinafter defined. The employee shall acknowledge that he has read and received a copy of such material by affixing their signature and date on the actual copy to be filed with the understanding that such signature merely signifies that he read the material to be filed and does not necessarily indicate agreement with its contents.
8.3 The employee shall have the right to answer any material filed and their answer shall become a part of the file copy. In case the teacher does not choose to answer such material, notation to this effect shall be added to the file copy.
8.4 All communications including evaluation by Gibraltar administrators, commendations, and validated complaints directed toward the employee which are to be included in the personnel file, a copy of same shall first be given the employee and shall be called to the employee's attention at the time of inclusion. If a supervisor keeps in their possession, private written notes, or written reports, these may not be used as official records until placed in the employee's file.
8.5 When an employee leaves the District, they have a right to review their file and the superintendent or their designee may remove any adverse material (prior to the last two (2) years of employment) if such adverse complaints have been corrected. Adverse material in this case would be reprimands, parent complaints, rebuttals, and anything of like nature.
8.6 No material relating to an employee's personal life which is unrelated to their teaching shall be placed in their personnel file.
8.7 Privileged information such as confidential credentials and related personal references normally sought at the time of employment are specifically exempted from review. The administrators shall, with the teacher looking on, remove these credentials and confidential reports from the file, prior to the review of the file by the employee.
