Medical and Prescription Drug Coverage. A. All full-time employees covered by this Agreement shall be provided the opportunity to enroll themselves and qualified dependents in County-sponsored medical and prescription coverage upon initial hire, subject to requirements of the County of Xxxx Health Benefits Eligibility Policy for Extra Help and Temporary Employees or during the annual open enrollment period. B. The County will contribute toward the payment of the employee’s premiums for individual County- sponsored medical and prescription drug coverage only on the County’s high-deductible self- insured health plan. The employer contribution for individual coverage is limited to the amount equal to or greater than the minimum health care affordability standards required under federal law based on a percentage of hourly pay. The County will not contribute any amount toward the payment of premiums for the employee’s dependents. C. All employees covered by this Agreement may elect to purchase additional coverage for dependents, and/or dental and vision coverage at their own expense. D. New employees may decline coverage under the County’s health insurance provided that the employee executes a declaration, in a form acceptable to the County. New employees who decline coverage may not enroll in the County’s health insurance program until the next annual open enrollment period. E. The County will continue to provide an annual open enrollment for employees to change health insurance plans and/or enroll eligible dependents. F. The County will continue to provide medical and prescription drug coverage as described in the Summary Plan Documents, which may be revised from time to time at the discretion of the Plan Administrator.

Medical and Prescription Drug Coverage.  Starting January 1, 2015, in lieu of the current company sponsored group medical coverage offered to any eligible retiree or eligible dependent who is Medicare eligible, the Company will offer a fully-insured group Medicare Advantage and Prescription Drug Plan (MAPDP). Retirees who are on disability pension & collecting Social Security disability will continue to be offered either the Retiree Medical Expense or Medical Supplement plan until age 65. The Company will, with the Union’s input, issue a Request For Proposal (“RFP”) for a MAPDP for a multi-year term. The plan design for such MAPDP will have a substantially similar design and value to the retirees as the current plans. The Union will have input in the RFP process, including but not limited to attendance at the RFP finalist meetings, but the company, as the plan sponsor and plan administrator of the MAPDP, shall be the ultimate decision-maker as to the MAPDP selected, as a result of the RFP. The RFP will request a multi-year rate increase guarantee.  The Company contribution towards medical coverage will be based on the lesser of two amounts: 1. 80% of the cost of the MAPDP described above. Additionally, for retirees who are eligible for retiree health care coverage based on the Rule of 60 retirement 50% of the cost of the MAPDP described above; or
Medical and Prescription Drug Coverage. The District will provide medical and prescription drug coverage to eligible full-time employees and their eligible dependents (where applicable) from a carrier selected by the District at its discretion. In the event that the District selects a variety of plans or carriers, employees shall have the option of selecting from those carriers or plans and may change to either of those carriers or plans once per year during the open enrollment period. During the term of this Agreement if the District selects Preferred Provider Organization (PPO) insurance that satisfies the requirements of a Qualified High Deductible Health Plan (HDHP), as defined by the United States Treasury and the Internal Revenue Service, the District will deposit into a health savings account of each eligible employee electing medical and prescription coverage the amount necessary so that the Employee’s responsibility toward the deductible is as follows: Single $500 Parent & Child $1,000 Two Adults $1,000 Parent & Children $1,500 Family $1,500 B. Dental: The District will provide dental insurance coverage as administered by Delta Dental or another carrier selected at the discretion of the District to each eligible full-time employee and his/her eligible dependents (subject to the provisions noted below) who elect medical and prescription coverage during the term of this agreement.
Medical and Prescription Drug Coverage. The University’s portion of the base plan medical and prescription drug premium is $4,584.00 for the coverage plan year, October 1, 2015 to September 30, 2016. Base plan premium rates may be adjusted each October, and the most current rates can be found in the Office of Human Resources. In the event that healthcare premiums increase in the base plan for subsequent years of this Agreement, the University’s obligation for the increase in premium is capped at 11%. If available, an Employee may select a buy-up plan option, which depending on the plan design, will require the Employee to pay the difference between the base plan and the buy-up plan offered by the University.

  • Medical Plan ‌ Eligible employees and dependants shall be covered by the British Columbia Medical Services Plan or carrier approved by the British Columbia Medical Services Commission. The Employer shall pay one hundred percent (100%) of the premium. An eligible employee who wishes to have coverage for other than dependants may do so provided the Medical Plan is agreeable and the extra premium is paid by the employee through payroll deduction. Membership shall be a condition of employment for eligible employees who shall be enrolled for coverage following the completion of three (3) months’ employment or upon the initial date of employment for those employees with portable service as outlined in Article 14.12.

  • Prescription Safety Glasses Prescription safety glasses will be furnished by the employer. The employer retains the authority to establish reasonable rules and procedures regarding frequency of issue, replacement of damaged glasses, limits on reimbursement costs and coordination with the employer's vision plan.

  • Medical Verification The Town may require medical verification of an employee’s absence if the Town perceives the employee is abusing sick leave or has used an excessive amount of sick leave.   • Prescription Glasses This plan covers prescription glasses as follows: • Frames - one (1) collection frame per plan year; • Lenses - one (1) pair of glass or plastic collection lenses per plan year. This plan covers the following lens treatments: • UV treatment; • tint (fashion, gradient, and glass-grey); • standard plastic scratch coating; • standard polycarbonate; and • photocromatic/transitions plastic. This plan covers one (1) supply of contact lenses as follows: • conventional contact lenses - one (1) pair per plan year from a selection of • extended wear disposable lenses - up to a 6-month supply of monthly or two- week single vision spherical or toric disposable contact lenses per plan year; or • daily wear disposable lenses - up to a 3-month supply of daily single vision spherical disposable contact lenses per plan year. This plan also covers the evaluation, fitting, or follow-up care related to contact lenses. This plan covers additional contact lenses if your prescribing network provider submits a verification form, with the regular claim form, verifying that you have one of the following conditions: • anisometropia of 3D in meridian powers; • high ametropia exceeding -10D or +10D in meridian powers; • keratoconus when the member’s vision is not correctable to 20/25 in either or both eyes using standard spectacle lenses; and • vision improvement for members whose vision can be corrected two lines of improvement on the visual acuity chart when compared to the best corrected standard spectacle lenses.

  • Safety Glasses Section 1. The City shall supply prescription safety glasses with plastic lenses to employees who are required to wear safety glasses and who are members of the classifications contained in Appendix C to this contract. Safety glasses which are authorized must be industrial grade safety glasses which meet or exceed the requirements of ANSI Specification Z87. 1. All employees who are required to wear safety glasses shall also be required to wear side xxxxxxx, either permanent or snap-on, whenever an eye hazard exists. Solid tinted glasses will not be approved unless required by prescription. Photogray, progressive, scratch coating and/or anti-glare lenses may be considered for those employees who primarily work outdoors or as prescribed. In the event that additional classes are identified as needing either prescription safety glasses or protective eyewear, such classes may be added to the classification list in Appendix C upon approval of PAGE and the City. Section 2. The City agrees to pay the full cost of required prescription safety glasses, with frames not to exceed $75.00. This excludes the cost of the eye examination which will be the responsibility of the employee. The effected employees will be allowed one (1) replacement of safety glasses every two (2) years. In the event the safety glasses become lost, unserviceable, or broken on the job, the employee must present a written request for replacement to the Department Head and Human Resources Director. If the employee breaks his safety glasses while on the job, the Department shall replace the glasses at no cost to the employee. The replacement of lost glasses or glasses that are broken off the job will be at the discretion of the Department Head and Human Resources Director. If an employee has been provided safety glasses by the City, the employee shall be permitted to retain possession of the glasses after separation from the City without reimbursing the City for any costs associated with the glasses. Section 3. An employee who is required to wear prescription safety glasses must present a written request to his department head or designated representative. Section 4. The employee must obtain a current prescription and the employee is authorized the use of sick leave not to exceed two (2) hours to accomplish this examination. The employee will obtain a purchase order from the Department Head prior to ordering the safety glasses. The employee will present the purchase order to the appropriate vendor when ordering. The vendor will contact the appropriate Department Head when the glasses are ready for delivery. The Department Head will then notify the employee who will present himself at the vendor for fitting and pickup. Section 5. In the event a probationary employee has been issued safety glasses and terminates his employment with the City for any reason during the probationary period, he shall be required to reimburse the City for any expenses incurred in the purchase of safety glasses.

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.

  • Medical Coverage The Executive shall be entitled to such continuation of health care coverage as is required under, and in accordance with, applicable law or otherwise provided in accordance with the Company’s policies. The Executive shall be notified in writing of the Executive’s rights to continue such coverage after the termination of the Executive’s employment pursuant to this Section 3(d)(iv), provided that the Executive timely complies with the conditions to continue such coverage. The Executive understands and acknowledges that the Executive is responsible to make all payments required for any such continued health care coverage that the Executive may choose to receive.
