Notification of Sickness. An employee who is absent from work on account of sickness or accident who wishes to use his sick leave credits for such absence must notify his immediate supervisor or designated Official as soon as possible, as specified by the Employer.
Notification of Sickness. Employees shall make every effort to notify the on-duty Senior Fire Officer (SFO) of the employee's sickness at least forty-five (45) minutes prior to the employee's regular starting time. Sick leave with pay will not be allowed unless proper notification has been given except where such notification was not possible for a valid reason. The Agency may require written doctor's certification verifying any use of sick leave when the Employer believes sick leave usage is being abused or for extended usage of more than three (3) shifts.

Notification of Sickness. The CONFIRE Director, or designee, must be notified within at least two (2) hours prior to the start of the employee’s shift of a sickness on each day of absence, unless under a doctor’s off work order for a specified period of time. In such instance where the employee is under a doctor’s off work order, the employee must only notify the CONFIRE Director, or designee, at least two (2) hours prior to the start of the first day of absence and inform him/her of the length of the absence. The employee will then forward the doctor’s off work order to the CONFIRE Director, or designee, within five calendar days from the first day. Failure to make such notification may result in denial of sick leave with pay. It is the responsibility of the employee to keep the appointing authority informed as to continued absence beyond the first day for reasons due to sickness or occupational disability. Failure to make such notification shall result in denial of sick leave with pay. If the employee receives a doctor’s off-work order and provides notice of same to the appointing authority, the employee is not required to contact the department daily. If the employee does not have an off-work order or has not notified the appointing authority that one has been issued, the employee shall be required to contact the department daily in accordance with the timeframe above.
Notification of Sickness. 16.7 The employee must notify his/her Division as early as possible, (being specific as to his/her condition), but not less than one (1) hour before the regularly scheduled starting time, where such an absence can be reasonably predicted. Upon notification of a requirement to report for work, employees in the Resource Pool who are unable to report due to illness or injury shall immediately report that situation to the caller at the time of notification. The employee shall notify his/her Division as early as possible of return to work, where such a return can be reasonably predicted. The designated Fire Department number to be called is the Sick Line (986-7425/986-SICK).

SEVERANCE ALLOWANCE termination of an employee having sick leave credits in his sick leave bank shall an amount equal to one-half the days to this credit, but shall not exceed an amount equal to one-half earnings at the regular rate of pay received by the employee immediately prior to termination of employee. In the event of the death of an employee this cash payment shall be paid to the employee's beneficiary. SHORT TERM INCOME PROTECTION PLAN The following plan designed to provide both long and short service employees with an income security, after they have completed an eligibility period, who are prevented from doing their normal duties due to illness or non-occupational injury.
