Quality Assurance Reviews. (QARs) – These reviews are compliance oriented reviews and examine how quality control procedures are being used per control and guidance documents such as: 1) environmental preparation manuals, 2) INDOT design manual, 3) bid/contract documents such as specifications and drawings, and 4) General Instructions to Field Employees (GIFE) for construction. INDOT and FHWA will work to establish a QAR Program of reviews on a regular cycle for the following program areas: Consultant Selection, Planning, Environment, Design, Right-of-Way and Construction. A minimum of five (5) QARs will be identified each year and the focus of reviews will be based on an assessment of risk and where improvements may be needed.

Quality Assurance Reviews. The ACRC/CRS shall conduct quality assurance reviews of field offices to evaluate understanding of and compliance with NRCS cultural resources policy and procedures in accordance with NRCS’ State Quality Assurance Plan. In addition, Annual cultural resources spot checks shall also be coordinated with regular spot checks conducted on a sample of conservation practices installed in each field office. A report on NRCS cultural resources quality assurance reviews shall be included in the Summary Reports submitted to MDAH.
Quality Assurance Reviews. 1. The Cabinet, or its agent, may conduct periodic quality assurance review for out-of-home placements. Periodically, the Cabinet or its agent shall monitor the Agency for quality assurance and performance. The periodic review shall assess the substantive clinical aspects of care to assure that services are being provided which meet the resident’s needs and correspond to the child’s assigned level of care. The private child-caring and/or private child-placing Agency shall provide full cooperation with the Cabinet and/or its agent as follows:
a. Cooperate with the Cabinet, or its agent, in the periodic quality assurance review for out-of-home placements.
b. Comply with the Cabinet’s Standards of Care. The Standards of Care are attached as Attachment E, Standards of Care and are subject to change. Compliance with these standards shall be a factor in the Cabinet’s selection of the Agency with whom children will be placed.
Quality Assurance Reviews. Regular QA Reviews are conducted on completed PCE/CE and supporting documents by EHES NEPA Program staff, in coordination with Region and FHWA staff as available. Standard questions developed by EHES NEPA Program and FHWA staff are used to assess the accuracy and consistency of PCE/CE documents and processes using the CE/PCE Procedures (Section 420 of this Manual). Written summaries are produced by EHES NEPA Program staff using a standard format agreed to by the EHES NEPA Program and FHWA that provide QA Review Findings and Recommendations. The QA Review Summaries (Findings and Recommendations) are then shared with RECs and their managers via email, and discussed at REC and environmental managers meetings.
Quality Assurance Reviews. Senior FNI staff will meet with project staff and verify that FNI quality control guidelines and the project-specific quality control plans are being implemented. We anticipate two Quality assurance reviews:
a) Interim Quality Assurance Review: FNI shall prepare for and conduct an internal quality assurance review at approximately the mid-point of the project schedule
Quality Assurance Reviews. All deliverables received will be subjected to a thorough quality assurance review. Deliverables will not be considered completed until accepted by the designated Contracting Officer’s Representative (COR). When the COR reviews a deliverable and raises quality or content issues, the BPA Holder / Contractor must fully address all those comments before resubmitting to USAID. Quality and presentation are of significant essence.
