Resident Care Coordinator. Contractor will provide 1 FTE Resident Care Coordinators. Persons in this position shall be credentialed as a Licensed Practical Nurse and have the responsibility to ensure the quality care of all Individuals under the Contract, through daily implementation of the established processes, documentation and resident care coordination. This includes oversight of direct care and medication tech tasks, auditing of order processing, incident reports, alert charting and physician and responsible party communication. This staff serves as both an administrative and clinical support role, reporting to the Director of Health Services. Resident Care Coordinators will: a. Participating in service plan meetings; b. Providing nursing care as directed by the RN; c. Assist with the screening of prospective Individuals to determine if their needs can be met under this Contract d. Ensure implementation of record keeping with outside providers, including Home Health and Hospice, and ensure they are communicating around their visits; e. Order entry and medication reviews; f. Training of med techs and caregivers around treatments and medication administration; g. Assist Program Director with the tracking and investigations of Incident Reports; h. Coordinates with the Milieu Counselor in behavior management; i. Gather pertinent information on the care needs of Individuals prior to the SPT meeting and then present any recommended changes to the SPT; j. Implement interim Service Plans as care needs change and assuring all health service staff are aware of the changes as directed by the assigned registered nurse; and k. Be responsible for ordering any durable medical equipment and obtaining all necessary orders.

Resident Care Coordinator. Contractor shall provide 2 FTE designated Resident Care Coordinators. Persons in these positions must have experience with supervising direct care staff and understand quality assurance procedures. Job description must designate responsibility for the following: a. Screening of referrals and other activities related to admission; b. Providing or ensuring availability of 24/7 continuous supervision, as well as ensuring access to emergency backup is available for direct care staff; c. Development and provision of any Individual-specific training needed to implement the Individual’s Service Plan; d. Schedule and provide facilitation of the Service Planning Team meetings; e. Provide monthly review of Individual Service Plans with Contractor’s direct care staff; f. Monitors changes to medical orders daily; g. Conduct record reviews and quality assurance checks of staff documentation h. Manage staffing decisions such as hires and training, performing staff screening, staff scheduling, conducting initial staff on-site training, and scheduling on-call coverage for all Contractor’s staff; i. Audit of each Individual’s Service Plan and medications at least quarterly; j. Ensures documentation in each Individuals’ chart for smooth transition between shifts; k. Facilitate, assist and provide support with client services and activities as needed, including but not limited to Individual admissions/discharges, orientations to Individual services, group facilitation and safety checks; l. Communicate with SPT and other team meetings regarding interactions with Individuals, observations, and changes in status of Individuals; m. Oversee the implementation of all training required by this Contract; n. Conduct record reviews and quality assurance checks of staff documentation; o. Ensure that Contractor’s staff has all supplies necessary for daily life as well as emergency events; p. Liaison with DHS Contract Administrator and local ODHS/AAA office of new referrals and discharges; and q. Develop and implement policies and procedures necessary to implement services in this Contract.
Resident Care Coordinator. Contractor shall provide 1 FTE Resident Care Coordinator position, whose primary duties include management of proper documentation and supplies that supports Individual care under the direction of the Registered Nurse. The Resident Care Coordinator shall: a. Assist in development of admission and transition plans to ensure Individuals have needed medical supports; b. Scheduling medical appointments and coordination of subsequent physician ordered medication, treatments and other medical supplies; c. Assistance with legal and financial issues, including but not limited to, court issues, representative payee services, assistance with paying bills, money management and, if needed, providing application for guardianship; d. Access to needed community services such as transportation, religious, vocational or education opportunities, volunteer groups, support or substance abuse recovery groups; e. Assistance with family interactions, support and outreach; f. Assistance in developing transition plans to support discharge goals and planning;

When census reaches 25 Individuals, Contractor will increase Resident Care Coordinator to 2 FTE. Assist with receiving and processing Individual admissions, readmits, and discharges; d. Compile new Individual medical charts, facilitate chart restructure and dismantling of discharged Individual medical records; e. Coordinate appointments and outside resources for medical need/services; f. Order/Manage Individual care supplies; g. Participate in review of and filing all completed orders; h. Notify Program Director of any Individual illness, injury, and condition change; i. Communicate/coordinate with physicians, outside providers, families, and other appropriate parties, concerning Individual status or change in status; j. Participate in staff hiring, training, education, disciplinary action, and evaluations; k. Review each Individual’s Service Plan with direct care staff at least quarterly; l. Assist Program Director with scheduling and facilitating Service Plan meetings; m. Ensure all Service Plan changes are communicated to Individuals, direct care staff, families, Program Director, RN, and ODHS Designee; n. Conduct, record, review, and perform quality assurance checks of staff documentation; o. Assist with staff scheduling as needed.
