RESIDENT ELIGIBILITY. Any resident of McHenry County, Illinois or a shelter client located in McHenry County, Illinois may make an application to the Provider for acceptance as a client in the Services to be funded in this Agreement. Said application may be made directly by a resident or on his behalf by his family, a physician, another Mental Health Board funded provider, or by other third parties when permitted by law.
RESIDENT ELIGIBILITY. Any resident of Dundee Township, Kane County, Illinois may make an application to the Provider for acceptance as a client in the Services to be funded in this Agreement. Said application may be made directly by a resident or on his behalf by his family, a physician, another Mental Health Board funded provider, or by other third parties when permitted by law. For purposes of this Agreement, a person shall be a resident of Dundee Township if i) his/her home residence address is assessed by the Dundee Township Assessor and subject to or exempt from Dundee Township real estate taxes, or ii) a person experiencing homelessness located within Dundee Township (even if the Services to be provided, such as shelter services, are to be located outside of Dundee Township).
RESIDENT ELIGIBILITY. Unless otherwise approved by Residence Life in writing, eligibility for residency in the Boryla Apartments is limited to (i) Regis undergraduate students who have completed at least fifty-five (55) credits and are enrolled for a minimum of twelve (12) credit hours per semester, not including summer sessions, and (ii) graduate students who are enrolled at Regis in the Doctor of Physical Therapy or Doctor of Pharmacy programs for a minimum of (8) credit hours per semester, not including summer sessions. Evidence of eligibility may be required by Regis at the time of making an application for residency at the Boryla Apartments, and at any time during the term of this Contract. Failure by You to maintain eligibility for residency will constitute grounds for termination of this Contract. However, Regis will have the right, but not the obligation, to permit You to continue to occupy the Assigned Space notwithstanding a lack of continuous eligibility if, in Regis’ sole judgment, compelling circumstances exist.
RESIDENT ELIGIBILITY. Only TAMU students or TAMU students and their legally dependent spouses and children may reside in University Apartments. Student residents must be full-time graduate or undergraduate students at TAMU enrolled in a degree producing program as defined by the most current TAMU graduate or undergraduate catalog. Exception can be made for students who are sponsored by TAMU’s Study Abroad Program. Student residents must also meet at least one of the following criteria: Graduate student, married student, student with a dependent child or children, international student, US military veteran student, student who is at least 21 years old, or an undergraduate student who has completed at least 24 semester hours or 2 semesters in residence at a 2 or 4 year college. Undergraduate students on Campus Housing Probation or higher for housing matters, or on Conduct Review or higher for discipline matters, or Scholastically Deficient (review or probation) for grade matters are not eligible to live in University Apartments. Post Doctorate students are not eligible as new student residents. Current student residents who receive a Post Doctorate appointment may submit an application to request to remain in University Apartments for a maximum of two additional semesters past graduation. Approval of a Post Doctorate application is based on space availability. At least a 60 day notice will be given to student residents when eligibility terms are amended.

RESIDENT ELIGIBILITY. MHRC Services Contractor shall provide a rehabilitation and recovery program aimed at improving the adaptive functioning of persons with mental disabilities to enable client to move to a less restrictive environment while preventing regression to a lower level of functioning. Services shall include, but are not limited to, the following services. These services shall be provided in a manner consistent with the terms and provisions of this Agreement and subject to procedures and standards mutually agreed upon by Contractor and County. Transportation support shall be provided for groups that take place away from Cordilleras.
RESIDENT ELIGIBILITY. The following information is extracted from the Accreditation Council of Graduate Medical Education (ACGME) “Institutional Requirements” of the “Essentials of Accredited Residencies in Graduate Medical Education.” Applicants with one of the following qualifications are eligible for appointment to accredited residency programs:
4.1. Graduates of medical schools in the United States accredited by either the Liaison Committee on Medical Education (LCME) or the American Osteopathic Association (AOA); graduates of Canadian medical schools approved by the Licentiate of the Medical Council of Canada (LMCC)
4.2. Graduates of medical schools outside the United States and Canada who have a current and valid certificate from the Educational Commission for Foreign Medical Graduates (ECFMG) prior to appointment or have a full and unrestricted license to practice medicine in a United States licensing jurisdiction in their current ACGME specialty/subspecialty program
4.3. United States citizen graduates from medical schools outside the United States and Canada who have successfully completed the licensure examination (USMLE Step 3) in a United States jurisdiction in which the law or regulations provide that a full and unrestricted license to practice will be granted without further examination after successful completion of a specified period of Graduate Medical Education
4.4. Graduates of medical schools in the United States and its territories not accredited by the LCME but recognized by the educational and licensure authorities in a medical licensing jurisdiction who have completed the procedures described in the paragraph above
4.5. Those who have completed the fifth pathway, a period of supervised clinical training for students who obtained their premedical education in the United States, received medical undergraduate abroad, and passed Step 1 of the United States Medical Licensing Examination
4.5.1. After these students successfully complete a year of clinical training sponsored by an LCME-accredited United States medical school and pass USMLE Step 2 components, they become eligible for an ACGME-accredited residency as an international medical graduate.
4.5.2. The Fifth Pathway program is not supported by the American Medical Association after December 2009.

These services shall be provided in a manner consistent with the terms and provisions of this Agreement and subject to procedures and standards mutually agreed upon by Contractor and County. Transportation support shall be provided for groups that take place away from Cordilleras. Programs, groups, meetings, social and recreational activities will be available 7 days a week.
