Scope of Position. The Step UP Practitioner (Adolescent Violence in the Home) is responsible an active team member of the Step Up Program. The position provides direct client case management and group facilitation whilst ensuring client needs and program targets are met. Duties and Responsibilities Direct client work including: Receiving referrals from police and community sources; Engage with referring agencies to achieve the program outcomes; Engage with adolescents and their families to assess and deliver services required to address violence in their homes and other concurrent issues that may be present; Assess and plan interventions with young people and their parent/s; Provide case management and therapeutic case work to young people and family members, with a focus on education, counselling and family work; Provide intensive casework, including case management when appropriate, with a focus on education and therapy; Co-facilitate group work sessions, implementing the adapted American developed Adolescent Violence in the Home program (Step Up; Maintain up to date and relevant client case notes/records and data as per the funding bodies requirements; Ensure liaison and close cooperation between partnership agencies; Any other relevant duties as required. Other tasks include: Ensure program and agency policies and procedures are implemented; Implement the elements of the Step UP Project; therapeutic case work, group work, referral pathways etc.; Participate in program evaluation activities as and when required; Represent the program & the organisation; Participate in meetings, training and professional development activities and forums as deemed appropriate; Attend meetings as required to fulfil the requirements of the role and also agency-wide staff workshops; Any other relevant duties as required.
