Sick Leave Bank Committee. The committee shall consist of two members of the administration, appointed by the Superintendent, and two members of the bargaining unit, appointed by the President of the Association. The President of the Association will act as chairperson of this committee and will have no voting power except in the event of a tie vote by the committee.

Sick Leave Bank Committee. The Sick Leave Bank Committee shall consist of two (2) members appointed by the Chairperson of the School Committee, and two (2) members appointed by the bargaining unit chairperson. The Sick Leave Bank Committee shall govern all phases of the Sick Leave Bank, including the option to accept or reject applications for sick leave.
Sick Leave Bank Committee. The Committee shall be appointed by the BTU-ESP for the purpose of administering the Sick Leave Bank. The Committee shall: (a) Maintain adequate records relative to all functions of the Bank. (b) Operate the Bank in accordance with rules and procedures mutually agreed to by the parties. However, the Superintendent shall establish and the BTU-ESP will comply with procedures for identifying and recording contributions to and withdrawals from the Bank. The parties authorize the Committee to establish additional administrative procedures necessary for the operation of the Bank as long as said procedures are consistent with and do not change any of the criteria contained in Section P of this Article. (c) Approve/Disapprove all requests for withdrawal from the Sick Leave Bank. (d) Not grant more sick leave days than are available in the Bank.
Sick Leave Bank Committee. The Bank will be operated on a voluntary basis. A committee shall be formed to administer the Bank and to provide the information whereby the payroll office of the Springfield City Schools will keep the records. This committee will be empowered to adopt rules and regulations and make decisions required to administer the Sick Leave Bank, so long as those rules and regulations and decisions do not modify the agreement contained herein. The Sick Leave Bank Committee (SBC) will be composed of the following persons:
Sick Leave Bank Committee a. The Sick Leave Bank Committee will consist of two (2) Administrators, two (2) members of NESPA and two (2) members of NTA, one of which shall be selected as Chairman by vote or lot. The chairmanship shall rotate yearly. Each participating group may change its respective participants at any time for cause, or by request to withdraw. All other changes in participants shall take place annually on the anniversary date of the Sick Leave Bank. The function of the committee shall be to operate the Sick Leave Bank effectively, economically and fairly. b. The Sick Leave Bank Committee shall appoint from among its own members a designated sub-committee of two (2) members to investigate all claims for benefits. The sub-committee shall be subordinate to the Sick Leave Bank Committee in the performance of its function. Sick Leave Bank Investigative Committee members are appointed and may be removed at any time by the Sick Leave Bank chairman for cause or by the majority of the Sick Leave Bank Committee members by vote with or without cause. c. The Investigative Committee’s function will be to investigate, process and report to the Sick Leave Bank Committee as a whole any claim for benefits or the continuation thereof by an eligible member. The Investigative Committee shall provide supportive documentation and a recommendation as to action relative to any claim made by a member. d. The Investigative Committee’s function will be to investigate and report to the Sick Leave Bank Committee as a whole any alleged abuse as well as provide supportive documentation and a recommendation as to action relative to any such abuse or allegation thereof assigned to the committee. e. The final decision as to each recommendation submitted by the Investigative Committee shall be made by the Sick Leave Bank Committee as a whole or a majority thereof. f. Once all committee functions have been completed and a ruling made as to acceptance, rejection, or abuse, the affected member shall be notified of the committee's decision in writing within five (5) working days. Working days shall be defined as those days on which the employee is working or scheduled to work. The decision of the Sick Leave Bank Committee shall be final and binding on the affected member. In cases of abuse, the committee shall also provide a copy of its decision to the Superintendent.

Maintain adequate records relative to all functions of the bank. b. Meet periodically, as requested by the Superintendent/designee, with a designated administrator of the county to review Federation sick leave bank records. c. Operate the bank in accordance with rules and procedures mutually agreed to by the parties. However, the Superintendent/designee shall establish and the Federation will comply with procedures for identifying and recording contributions to and withdrawals from the bank. The parties authorize the Committee to establish additional administrative procedures necessary for the operation of the Bank as long as said procedures are consistent with and do not change any of the criteria contained in this section of this Article. d. Not grant more sick leave days than are available in the bank.

A committee shall be formed to administer the Bank and to provide the information whereby the business office of the Board will keep the records. This committee shall be empowered to adopt rules and regulations and to make decisions required to administer the Sick Leave Bank, so long as those rules, regulations and decisions do not modify the agreement contained herein. This committee will be titled the "Sick Leave Bank Committee" (hereafter referred to as the SBC). The SBC shall be composed of the following five (5) persons: (1) Superintendent of the Board or his/her designee. (2) The Association President or his/her designee. (3) Treasurer of the Board or his/her designee. (4) Two bargaining unit members. These members are to be appointed by the Association President. b. Should a vacancy occur on the SBC, a replacement for the vacant position shall be appointed by the authority making the original appointment. c. One of the three bargaining unit SBC representatives will be selected to act as chairperson of the SBC. The Association President will designate the chairperson prior to the first meeting of the SBC. d. The SBC will be responsible for developing the forms needed to operate the Bank. e. Guidelines will be reviewed annually by the Sick Leave Bank Committee.

Sick Leave Bank Committee. The sick leave bank shall be administered by a Sick Leave Bank Committee consisting of four (4) persons designated by the Board to serve at its discretion and four (4) persons designated by CSU-AAUP to serve at its discretion. The Sick Leave Bank Committee shall determine the eligibility for the use of the bank and the amount of leave to be granted. The following criteria shall be used by the committee in administering the bank and determining the eligibility and the amount of leave: (1) Adequate medical evidence of catastrophic and extended illness; (2) Prior utilization of all available sick leave.
Sick Leave Bank Committee. The Board of School Trustees of the Nettle Creek School Corporation shall be the administering agent of the sick leave bank. A Sick Leave Bank Committee shall be formed annually to assist the board in administering the sick leave bank. This committee shall consist of two (2) persons appointed by the board and one (1) person appointed by the association. The board shall appoint one (1) board member and one (1) administrator (other than the superintendent) as its representatives on the sick leave bank committee. Appointments to the Sick Leave Bank Committee shall be made in writing to the superintendent not later than the end of the first school day which occurs during the initial week of the school year.
Sick Leave Bank Committee. The Sick Leave Bank Committee shall consist of four (4) union members (one member from each classification) designated by the President of the Local and two (2) representatives appointed by the Superintendent. The Bank Committee shall consider individual requests for the donation of sick leave from the Sick Leave Bank and submit a written recommendation to the Superintendent. The recommendation shall include determination of whether or not the criteria for the donation of sick leave set forth in the section have been satisfied. A separate Bank Committee may design for each individual request as terminated by the Superintendent and the Local President.
