TERMINAL LEAVE BENEFITS. 24.1 Upon the death of an employee, all accrued and unused overtime, vacation leave, and other terminal leave benefits (other than life insurance proceeds for which a beneficiary has been designated) shall be paid within forty-five (45) days. 24.2 Payment of the terminal leave benefits provided for in Section 24.1 shall be made as follows: A. The benefits will be paid as set forth on a form provided by the Employer; B. If the employee has not provided for distribution of the benefits on the form provided, the benefits will be paid to the employee's surviving spouse. A surviving spouse may elect to receive a lump sum payment of the leave balance or may elect other options available under applicable ordinance. C. In the event the employee leaves no surviving spouse, the benefits will be paid to the employee's children in equal shares, payable as follows: (1) To each of the employee's children over the age of 18 who are known to the Employer; (2) To the legal guardian or representative of each of the employee's children under the age of 18 who are known to the Employer. D. If the employee has no children known to the Employer, the benefits will be paid to the surviving parent(s) of the employee, in equal shares. E. If the employee has no surviving parents known to the Employer, the benefits will be paid to the employee's estate.

TERMINAL LEAVE BENEFITS. An employee whose effective date of retirement occurs during the term of this Agreement retiring on City Pension under the Employes' Retirement System of Milwaukee plan (but excluding an employee retiring on deferred or actuarially reduced pensions, as they are defined under the system) shall, upon retirement, be entitled to receive a one-time-only lump sum payment equivalent to 30% of the employee' s accumulated and unused sick leave plus one-half of the sick leave days accumulated during the previous 12 months of service, for up to 6 additional days.

TERMINAL LEAVE BENEFITS. A. Upon retirement teachers who actively retire, as opposed to taking “deferred” benefits, in accordance with the provisions of the Teachers’ Pension and Annuity Fund after fifteen years of continuous teaching service in the Gloucester City School System shall be paid for accumulated sick leave days in accordance with the following schedule: $60.00 per day Effective July 1, 2008, the total payment under this article will be limited to $15,000.00 per person. B. If an employee dies prior to their use of his/her terminal leave benefit, the pension beneficiary shall receive the payment for those days that have accumulated as a result of a minimum of fifteen (15) years of continuous teaching service in the district. C. Teachers planning to retire must notify the Superintendent no later than December 1 of the year preceding the effective date of said retirement in order to receive prompt payment; those who fail to comply with the notification procedure described herein shall be required to wait for said payment until such time as (1) the money is allocated in the next school budget and (2) the instant the budget becomes effective. D. Teachers leaving the Board’s employment by reason of a disability retirement shall be exempt from the fifteen (15) year service requirement set forth herein. E. Effective July 1, 2007, Payments up to $8500.00 shall be paid on or before July 15; payments in excess of $8500.00 shall be paid on or before July 15 of the following year.
TERMINAL LEAVE BENEFITS. A. Any regular employee who retires shall be eligible for terminal pay of one day's pay for each full year of active service or major portion thereof in the School District of the City of Ferndale. Time spent on leave shall not be counted as active service. B. An employee who retires under this section shall, in addition to A. above, be paid for all sick days in the current allowance and/or accumulated sick bank to a maximum of seventy five (75) paid days at the rate of $55.00 for each of these days. These days must come from the employees’ current sick bank. C. A retiree, to be eligible for terminal pay, must qualify for regular or medical retirement under the Michigan School Employees Retirement System. An employee shall be considered employed for retirement pay purposes if he/she is on the payroll or on a leave when he/she retires. D. Should an employee die subsequent to retirement but prior to receipt of the check for this benefit, the Board agrees to pay the benefit to the eligible retiree's bank account on file. E. Payment shall be made no later than sixty (60) days of request for payment. Such request shall be made within ninety (90) days of termination.

In order to receive this allowance, the employee must provide to the Employer, proof that she has applied for and is eligible to receive employment insurance benefits pursuant to the Employment Insurance Act. (b) Pursuant to the Supplemental Employment Benefit (SEB) Plan, the maternity leave allowance will consist of 15 weekly payments equivalent to the difference between the employment insurance gross benefits and any other earnings received by the employee and 85% of the employee's basic pay.

  • General Benefits During the Term of Employment, the Executive shall be entitled to participate in such employee pension and welfare benefit plans and programs of the Company as are made available to the Company's senior-level executives or to its employees generally, as such plans or programs may be in effect from time to time, including, without limitation, health, medical, dental, long-term disability, travel accident and life insurance plans.

  • Parental Leave Allowance ‌ (a) An employee who qualifies for parental leave pursuant to Article 35.03, shall be paid a parental leave allowance in accordance with the Supplemental Employment Benefit (SEB) Plan. In order to receive this allowance, the employee must provide to the Employer proof of application and eligibility to receive employment insurance benefits pursuant to the Employment Insurance Act. An employee disentitled or disqualified from receiving employment insurance benefits is not eligible for parental leave allowance. (b) Pursuant to the Supplemental Employment Benefit (SEB) Plan and subject to leave apportionment pursuant to Article 35.03(b), the parental leave allowance will consist of a maximum of ten (10) weekly payments, equivalent to the difference between the employment insurance gross benefits and any other earnings received by the employee, and seventy-five (75) percent of the employee’s basic pay.

  • Health Benefits For the eighteen (18) month period following the Termination Date, provided that Executive is eligible for, and timely elects COBRA continuation coverage, the Company will pay on Executive’s behalf, the monthly cost of COBRA continuation coverage under the Company’s group health plan for Executive and, where applicable, her spouse and dependents, at the level in effect as of the Termination Date, adjusted for any increase in such level paid by the Company for active employees, less the employee portion of the applicable premiums that Executive would have paid had she remained employed during the such eighteen (18) month period (the COBRA continuation coverage period shall run concurrently with the eighteen (18) month period that COBRA premium payments are made on Executive’s behalf under this subsection 1(a)(ii)). The reimbursements described herein shall be paid in monthly installments, commencing on the sixtieth (60th) day following the Termination Date, provided that the first such installment payment shall include any unpaid reimbursements that would have been made during the first sixty (60) days following the Termination Date. Notwithstanding the foregoing, the Company’s payment of the monthly COBRA premiums in accordance with this subsection 1(a)(ii) shall cease immediately upon the earlier of: (A) the end of the eighteen (18) month period following the Termination Date, or (B) the date that Executive is eligible for comparable coverage with a subsequent employer. Executive agrees to notify the Company in writing immediately if subsequent employment is accepted prior to the end of the eighteen (18) month period following the Termination Date and Executive agrees to repay to the Company any COBRA premium amount paid on Executive’s behalf during such period for any period of employment during which group health coverage is available through a subsequent employer. Notwithstanding the foregoing, the Company reserves the right to restructure the foregoing COBRA premium payment arrangement in any manner necessary or appropriate to avoid fines, penalties or negative tax consequences to the Company or Executive (including, without limitation, to avoid any penalty imposed for violation of the nondiscrimination requirements under the Patient Protection and Affordable Care Act or the guidance issued thereunder), as determined by the Company in its sole and absolute discretion.

  • Annual Leave Loading During a period of annual leave an employee will receive a loading of 17.5 per cent calculated on the employee’s normal hourly rate of pay and the daily fares allowance if applicable. The loading will also apply to proportionate leave on lawful termination.

  • Part-Time Benefits time employee shall receive in lieu of all fringe benefits (being those benefits to an employee, paid in whole or in part by the hospitals, as part of direct compensation or otherwise, including holiday pay, save and except salary, vacation pay, standby pay, call-in pay, responsibility pay, jury and witness duty, bereavement leave, and pregnancy and parental supplemental unemployment benefits) an amount equal to 14% of his regular straight time hourly rate for all straight time hours paid.
