Use of E-mail. If you chose to e-mail our staff, including a doctor or office assistant, your e-mail, as well as our response, may not be secure. As with any form of e-mail communication, confidentiality may be breached. Please use e-mail with discretion. By signing this document, you acknowledge that e-mail is not a secure form of communication and the confidentiality of your child’s information may be breached.

Use of E-mail. The Association shall have the privilege to use the District’s email system to communicate with the Bargaining Unit. That usage shall be in accordance with the District’s procedures for email use, which are delineated in Policy and Procedure. The District’s email system shall not be used to promote or support any political activity or to affect the outcome of an election. The District’s email system is a public system and emails are subject to public records’ rules.
Use of E-mail. E-mail, internet or intranet websites may be used only to distribute routine communications, such as financial statements, billing statements and other like information and to distribute Loan Documents for execution by the parties thereto, but may not be used for any other purpose, unless approved by Lender. Provided, an original signed document that has been scanned and attached to an e-mail shall have the same force and effect as a document sent by facsimile.
Use of E-mail. The ability to conduct certain business by computer under this Agreement will allow You to communicate electronically with TGAL by email and other means. TGAL and other third party Information Certain information including last sale transaction data and other financial market data, third- party research and analysts' reports, opinions, price charts, news reports, and other information relating to TGAL products or other products and securities in the market, may be made available as part of the Site or through a link from the Site. Such information is provided "as is" and on an "as available" basis. This information has been obtained through sources believed reliable but TGAL makes no representation with respect to, nor does it guarantee or endorse the accuracy, completeness, reliability or availability of, such information. Information concerning Client Accounts You may also be able to access information concerning client accounts ("Client Account Information") which is also proprietary and confidential. You will not provide Client Account Information to any person other than a duly authorised officer, employee or partner of Your business and You shall ensure that such officer, employee or partner shall not provide Client Account Information to any other person. Transmitting confidential client information by e-mail, however, has a number of risks, both general and specific, that you should consider. General e-mail risks include: E-mail may be received by many intended and unintended recipients, recipients can forward e-mail messages to other recipients without the original sender's permission or knowledge, users can easily misaddress an e-mail, e-mail is easier to falsify than handwritten or signed documents, and backup copies of e-mail may exist even after the sender or recipient has deleted his or her copy. Specific client e-mail risks include: Employees do not have an expectation of privacy in e-mail they send or receive at their place of employment and clients who send or receive e-mail from their place of employment risk having their employer read their email; clients have no way of anticipating how soon their therapist will respond to a particular e-mail; although I try to read and respond to e-mail promptly, I cannot guarantee that any particular message will be read and responded to within any particular period of time. Conditions for the Use of E-mail: It is my policy that I will treat all e-mail messages that concern the client with the same degree of confidentiality as afforded other portions of the medical record. I will use reasonable means to protect the security and confidentiality of e-mail information but are not liable for improper disclosure of confidential information not caused by our gross negligence or wanton misconduct. Additionally, because of the risks outlined above, I cannot guarantee the security and confidentiality of e-mail communication. Thus, clients must consent to the use of e-mail for confidential medical information after having been informed of the above risks. Finally, clients must not use email in a medical or psychiatric emergency. BILLING AND FEES All fees are based on a 45 minute session. Thus, an initial consultation session, which is typically 90 minutes in length, will be billed at twice the rate of a 45 minute therapy session. At the end of each session, I will provide you with a xxxx detailing the service provided and the total balance due. Payment for a consultation session is always expected at the time of service.
