Accident and Incident Reporting. ‌ When an Artist is involved in an accident or incident that results in, or may potentially lead to, time off or modified work, and the Theatre is informed of the accident or incident, the Theatre shall file a report. The Theatre will file a copy with Equity of any provincial public agency reports or any accident or incident report generated by the Theatre or Stage Manager. Accident and Incident Reporting. Project Co shall do everything reasonably practicable to ensure that no Person suffers an injury or loss of life and that no property is damaged or lost due to the performance of the Project Work. In the event of any injury to any Person or loss of life in relation to the performance of the Project Work, any damage to property arising from the Project or the performance of the Project Work, any near-miss incident or any public safety incident, Project Co shall:

The Contractor shall interview the driver involved in any accident to ensure the clarity, accuracy and comprehensiveness of the report. The Contractor shall provide copies of all incident and accident reports to CTA within three (3) business days of the occurrence. Liquidated Damages: Liquidated damages of three hundred ($300) dollars may be assessed per occurrence of failure to notify the CTA of injury accidents within 24 hours or property damage and disturbances within 48 hours of occurrence. Monitoring: CTA staff will perform checks of monthly reports and checked by random vehicle inspection.
Accident and Incident Reporting. The Employee must promptly report all work accidents/injuries to their Manager on the day they occur, unless prevented by exceptional circumstances. Accident/Incident reports must be completed at the earliest possible time after the incident, usually within 24 hours, no matter how minor or serious. An internal follow up or investigation will then proceed.

Prompt action shall be taken to insure immediate and appropriate medical treatment is provided. Initial notification will be verbal with follow-up in writing as directed by the COR. The contractor shall also maintain an OSHA 300 log of on-the job-injuries and first aid treatments. In addition, there are reports that must be completed in a timely manner when there is a vehicle accident and/or lost or damaged government property.
