ARTICLE EMPLOYEE BENEFITS. Each employee covered by this Agreement shalljoin the Group Life Insurance, Long Term Disability Insurance and Pension Plan currently in effect in accordance with the regulations and conditions of the policies providing such Plans. Subjectto the terms and conditions of the Plans noted below and subject to employee contribution to the billed premiums through payroll deductions, where applicable, the Employer and the Union agree for the duration of this Agreement as follows: For eligible employees on the active payroll of the Employer on March who have completed their probationary period, the Employer agrees to contribute one hundred percent (100%)of the billed premium for the present group life insurance Plan. For eligible employees hired after March the Employer shall pay one hundred percent (100%)of the billed premium for the present group life Insurance Plan after such eligible employees have completed one (1)year worked with the Employer. The Employer agrees to contribute eighty percent (80%) of the billed premium for the present dental Plan and extended health benefits Plan after such eligible employees have completed one (1)year worked with the Employer and provided that the eligible employees contribute twenty percent (20%) of the billed premium for this Plan. Effective on May until August in the event an employee is required to use his automobileon behalf of the Employer, the Employer shall pay an allowance of thirty-six cents per Commencing September for the balance of the term of the Collective Agreement, in the event an employee is required to use his automobile on behalf of the Employer, the Employer shall pay an allowance of: Forty cents per for the first kilometres Thirty-eight cents per kilometre for the next kilometres Thirty-six cents per thereafter. Such allowance shall be paid upon receipt of an official travel expense form approved by the employee's immediate supervisor. An employee will not be authorized to use his vehicle for business purposes until he has submitted proof from his insurance agent that he has the required insurance coverage for said purposes. ARTICLE EMPLOYEE BENEFITS. All full-time employees who qualify under the terms of the underwriters contracts shall become eligible for Company paid benefits on the first day following the fifty-fifth day worked. A twelve (12) hour shift will be considered as one and a half days worked for the purpose of seniority. ARTICLE EMPLOYEE BENEFITS. Qualifying Employees shall be covered by the Company’s short term disability plan, long term disability plan and extended health care plan according to the terms of these plans and as described in the Benefit Book. Premium cost share arrangements effective upon ratification of this agreement shall be maintained. Insurance carriers may be changed at the discretion of the Company provided comparable benefits are maintained. Sick Leave Sick leave is defined as a period of one or more days or parts thereof, during which an Employee was scheduled to work and was unable to report due to illness or injury. All full time Employees will (87.5) hours of sick leave credit at the start of each year. New Employees hired after the beginning of the calendar year will receive sick leave credits prorated at seven (7) hours per month. All part time Employees will receive (65.25) hours of sick leave credit at the start of each year. New Employees hired after the beginning of the calendar year will receive sick leave credits prorated at five (5) hours per month. Casual time employees will not be entitled to sick leave. Where an Employee has sufficient time in her sick bank, one hour of sick leave time shall be deducted from her sick leave bank for each hour an employee is off sick. Each Employee’s sick leave bank will be cleared to zero each December A Doctor’s certificate must be provided for sick leave in excess of three (3) consecutive days or for a shorter absence if requested by the Company.
ARTICLE EMPLOYEE BENEFITS. Car Allowance Employees who are required to supply their own transportation in the performance of their duties shall upon receipt of proper documentation be paid a mileage allowance as stipulated in the Board’s policy for such purpose.
ARTICLE EMPLOYEE BENEFITS. The Ontario Municipal Retirement System Act, as amended time to time, shall apply to full-time and eligible part-time Nurses covered by this Agreement. The Employer and the Nurse shall contribute equally to the Pension Plan. The Employer agreed to contribute towards the premium coverage of eligible Nurses in the active employ of the Employer under the insurance plans set out below subject to their terms and conditions including any enrolment requirement.
