Benefits for Active Employees. (a) Grace, New Grace and Grace-Conn. shall take all steps necessary or appropriate so that, effective no later than the Distribution Date, one or more members of the Packco Group are the sole sponsors of the Packco Health Plan. Such steps shall include, without limitation, the appointment or reappointment by Grace of all named fiduciaries, trustees, custodians, recordkeepers and other fiduciaries and service providers to the Packco Health Plan, to the extent such appointments or reappointments are necessary. (b) Effective as of the Distribution Date, the New Grace Group shall assume or retain (as applicable) all Liabilities relating to or arising out of claims for benefits under U.S. Welfare Plans by New Grace Participants and Terminated Grace Participants, whenever such claims are incurred, and (ii) by Packco Participants to the extent such claims are incurred before the Distribution Date and reported within 365 days thereafter. Effective as of the Distribution Date, the Packco Group shall assume or retain (as applicable) all Liabilities relating to or arising out of all other claims for benefits under U.S. Welfare Plans by Packco Participants, except as specifically provided in Section 5.02.

Benefits for Active Employees. 117 Effective January 1, 2021 the medical coverage available to Regular Employees will change to the bargained medical plan for Northern California Unions in accordance with 2019 National Coalition bargaining. 118 Dental Plan 119 The Employer will provide a dental plan for eligible Employees and eligible dependents with an annual benefit maximum of $1,500, and a lifetime maximum for child orthodontia of $1,500. 120 Life Insurance 121 Effective January 1, 2021, the Employer will, at its expense, provide each eligible Employee with a fifty thousand dollar ($50,000) Group Life Insurance policy and five thousand dollar ($5,000) Accidental Death and Dismemberment coverage. 122 Effective January 1, 2021, the Employer will provide for employees the option to purchase an additional fourteen thousand dollars ($14,000) worth of life insurance. This fourteen thousand dollars ($14,000) includes six thousand five hundred dollars ($6,500) Accidental Death and Dismemberment coverage. 123 Short/Long Term Disability Insurance: 124 Effective January 1, 2021, Short Term Disability Insurance shall be provided to Regular Employees with less than two (2) years of employment. This benefit shall provide 50% income replacement for a duration up to one (1) year. 125 Effective January 1, 2021, Long Term Disability Insurance shall be provided to Regular Employees with two (2) or more years of service. This benefit shall provide 50% income replacement for a duration up to five (5) years.

Effective July 1, 2023, the District shall contribute a maximum of $10, 600 per year towards a single party plan for medical benefits, $16, 450 for two party, and $20,850 for a family plan. If the District desires to change health carriers during the term of this Agreement, it shall so notify the Union and provide it with the opportunity to negotiate such change to the extent such change impact on matters within the scope of representation. b. Effective 2017-18 school year, PVSD will give $500 per unit member to those who are spouses or domestic partners within the District who enroll in 2-Party or Family medical coverage and are both required to take the District insurance under SISC rules. c. The district’s Health Benefits Committee shall include up to three (3) representatives from SEIU, one of which may be a SEIU staff member. The Committee shall review health providers, plans, and coverage issues making recommendations to the Superintendent. Any questions related to the district cap amount or use shall be discussed with SEIU at the bargaining table. d. Employees shall be eligible for benefits under this section commencing the first day of the month following the first day of regular employment or another event qualifying an employee for health benefits and in conformance with the provider's implementation requirements.

  • Benefits for Part-Time Employees ‌ A part-time employee shall receive in lieu of all fringe benefits (being those benefits to an employee, paid in whole or part by the Hospital, as part of direct compensation or otherwise, including holiday pay, save and except salary, vacation pay, standby pay, call back pay, reporting pay, responsibility allowance, jury and witness duty, bereavement pay, and maternity supplemental unemployment benefits) an amount equal to 14% of his/her regular straight time hourly rate for all straight time hours paid.

  • Probation for Newly Hired Employees (a) The Employer may reject a probationary employee for just cause. A rejection during probation shall not be considered a dismissal for the purpose of Article 11.2

  • Incentive, Savings and Retirement Plans During the Employment Period, the Executive shall be entitled to participate in all incentive, savings and retirement plans, practices, policies and programs applicable generally to other peer executives of the Company and its affiliated companies, but in no event shall such plans, practices, policies and programs provide the Executive with incentive opportunities (measured with respect to both regular and special incentive opportunities, to the extent, if any, that such distinction is applicable), savings opportunities and retirement benefit opportunities, in each case, less favorable, in the aggregate, than the most favorable of those provided by the Company and its affiliated companies for the Executive under such plans, practices, policies and programs as in effect at any time during the 120-day period immediately preceding the Effective Date or if more favorable to the Executive, those provided generally at any time after the Effective Date to other peer executives of the Company and its affiliated companies.

  • Excluded Employees Employees excluded from the bargaining unit who work for an Employer signatory to this Agreement may participate in any of the foregoing benefits under rules and regulations established by the Trustees. The trustees shall determine the contributions required for such benefits.

  • Newly Hired Employees All employees hired to an insurance eligible position must make their benefit elections by their initial effective date of coverage as defined in this Article, Section 5C. Insurance eligible employees will automatically be enrolled in basic life coverage. If employees eligible for a full Employer Contribution do not choose a health plan administrator and a primary care clinic by their initial effective date, and do not waive medical coverage, they will be enrolled in a Benefit Level Two clinic (or Level One, if available) that meets established access standards in the health plan with the largest number of Benefit Level One and Two clinics in the county of the employee’s residence at the beginning of the insurance year. If an employee does not choose a health plan administrator and primary care clinic by their initial effective date, but was previously covered as a dependent immediately prior to their initial effective date, they will be defaulted to the plan administrator and primary care clinic in which they were previously enrolled.

  • Shift Employees Employees who work rotating shift patterns or those who work qualifying shifts shall be entitled, on completion of 12 months employment on shift work, to up to an additional 5 days annual leave, based on the number of qualifying shifts worked. The entitlement will be calculated on the annual leave anniversary date. Qualifying shifts are defined as a shift which involves at least 2 hours work performed outside the hours of 8.00am to 5.00pm, excluding overtime.   • Rehired Employees Amounts forfeited upon termination of employment because of the failure to meet the applicable vesting requirements shall not be reinstated or re-credited if an individual is subsequently rehired or re-employed by the School Corporation. However, if the board shall have approved a leave of absence of not more than one (1) fiscal year for an employee, such period of leave shall not result in forfeiture provided the employee shall promptly return to employment following the expiration of the period of leave.
