Community Liaison. The Community Liaison shall be a full-time position that develops and maintains relationships with community resources, State agencies, and community entities that traditionally provide services for Enrollees or Potential Enrollees. This individual will coordinate the provision of Community-Based Services to Enrollees, assist in Enrollee outreach, and manage community engagement activities.

Community Liaison. Xxxxx has appointed and will maintain a committee of employees who will meet periodically to evaluate Zoosk’s progress with respect to the goals and objectives set forth in this Community Benefit Agreement. Members of the community can contact Zoosk’s community liaison, Xxxx XxXxxx, at xxx@xxxxx.xxx with questions or concerns about Zoosk’s goals and objectives set forth in this Community Benefit Agreement. Zoosk’s community liaison or a company representative will attend Citizen’s Advisory Committee meetings. If a community liaison or a company representative cannot attend a meeting of the Citizen’s Advisory Committee, the community liaison should notify the Committee Staff from the Office of the City Administrator no later than 72 hours in advance of the meeting about the absence. If Zoosk is not represented by a community liaison or a company representative at four meetings within a 12-­‐month period, this Key Feature #6 shall not be deemed successfully completed. Zoosk’s community liaison or a company representative shall participate in a discussion relating to displacement in San Francisco with the Citizen’s Advisory Committee and other companies that have signed community benefit agreements with the City. Evaluation and Reporting Zoosk will report progress on this Community Benefit Agreement to the City Administrator at least once a month, in a manner mutually agreed to between the City Administrator and Zoosk. Progress reports on key features shall include information sufficient to determine progress and completion, such as Zoosk’s records, e-­‐mails, public communications, invoices, grant agreements, or verification provided by participating community organizations. Documents and reports submitted by Zoosk to the City Administrator’s Office shall not be disclosed to third parties without Zoosk’s prior written consent. Summaries of the progress reports may be shared with the Citizen’s Advisory Committee, provided that they do not contain any personal information of Zoosk employees or other confidential information. Further: • By March 1, 2016, Zoosk will provide the City Administrator with a delivery schedule for meeting the goals set forth in this Community Benefit Agreement, with completion dates to the extent practicable. Community Liaison. Prior to issuance of a building permit to construct the Project and implement the approved use, the Project Sponsor shall appoint a community liaison officer to deal with the issues of concern to owners and occupants of nearby properties. The Project Sponsor shall provide the Zoning Administrator with written notice of the name, business address, and telephone number of the community liaison. Should the contact information change, the Zoning Administrator shall be made aware of such change. The community liaison shall report to the Zoning Administrator what issues, if any, are of concern to the community and what issues have not been resolved by the Project Sponsor. The community liaison will convene a community advisory group (CAG) for the purpose of conveying input to the project sponsor on its operations and providing a forum for community comment and concern. The CAG shall consist of approximately ten (10) members representing diverse neighborhood interests such as health care providers, established neighborhood groups, resident homeowners and local merchants, and its membership is expected to change over time. Once the CAG is established, the community liaison and CAG members will agree to a regular meeting schedule, with a frequency of not less than quarterly or more than monthly. The agenda for meetings will be set jointly by the community liaison and the CAG. The community liaison will facilitate and provide logistical support for all meetings, including scheduling and providing meeting space if needed. Further: • Within 60 days of January 1 of each calendar year of this agreement, Twitter will provide the City Administrator with a quarterly delivery schedule for such calendar year, with completion dates to the extent practicable. • Twitter will report on its progress meeting goals and objectives in this CBA by reporting to the City Administrator on an ongoing basis. • The City Administrator will deem each year of this Community Benefit Agreement successful and completed if the items on Twitter’s delivery schedule are reasonably found to be either (i) substantially completed or (ii) if Twitter made a good faith effort to achieve each item.
