Dental/Vision Insurance. The District shall contribute up to a maximum of $131.36, a month, effective January 1, 2014, for a group dental plan providing a maximum $2,000 (Two thousand dollars) of annual coverage for eligible unit members and their families (which includes coverage for spouse or domestic partner, and dependents). The District shall contribute up to a maximum of $21.72 a month for a group vision plan for eligible unit members and their families (which includes coverage for spouse or domestic partner, and dependents). The above contribution levels for medical, dental, and vision insurance may be increased as a result of the application of the Resource Allocation Formula. (Reference Appendix B)
Dental/Vision Insurance. The College will provide dental and vision insurance to members. The insurance plans are set forth in detail in the policy or policies of insurance which contain the detailed, specific terms and conditions of coverage. Employees are eligible to participate in group dental and vision insurance as follows:
Dental/Vision Insurance. Three (3) dental coverage options, one base plan and one which provides orthodontia coverage and one Dental HMO (based on home zip code), and a vision coverage option will be offered, the terms of which will be governed by the plan documents. Swedish Medical Center will pay the entire premium of the base dental plan for employees with a 0.75-1.0 FTE who elect the Employee Only coverage. Employees with a 0.5-0.74 FTE will share part of the cost for coverage for single and dependent dental coverage. The 2020 dental and vision plan design summary and premiums (payroll contributions) are attached as Addendum M.
Dental/Vision Insurance. Dental and Vision Insurance are provided by SEBB and administered through the HCA as delineated above in Article 12.1.
Dental/Vision Insurance. The District shall provide the dental/vision insurance for all of its certificated employees and eligible dependents.
Dental/Vision Insurance. The City shall pay one hundred percent (100%) of dental and vision insurance premiums for all members of the LPFA (and their family members where applicable). At no time shall the City allow the dental and vision coverage services provided to become less than that which was in place at the adoption of this Agreement.
Dental/Vision Insurance. 5.2.1 Dental Represented employees and their legal dependents, as defined by the State of California and the Internal Revenue Service, are eligible to participate in a group dental plan selected by the District. The District shall pay the actual premium charged by the approved dental insurance provider for providing coverage to the employees and their dependents under the terms of the plan up to a maximum amount of $153 per employee toward monthly premium.
5.2.2 Vision Represented employees are eligible to participate in a group vision plan selected by the District. The District shall pay the actual premium charged by the approved vision insurance provider for providing coverage to the employees and their dependents under the terms of the plan. The District covers the cost of the employee only and shall contribute a maximum of $9.00 toward the monthly premium.
Dental/Vision Insurance. Beginning on the first day of the month following the date of hire, AP will pay 100% of the premiums of dental and vision coverage to all full-time and part-time employees and their spouses/domestic partners and dependents.
Dental/Vision Insurance. The City shall pay one hundred percent (100%) of dental and vision insurance premiums for employees and eligible dependents.
Dental/Vision Insurance. 24.1 Beginning the first of the month following thirty days (30) of continuous employment, and subject to the plan’s eligibility and other terms and conditions, all regular full‐time Registered Nurses and regular part‐time Registered Nurses shall be covered by the Hospital’s dental and vision plan. The Registered Nurse is responsible for completing all necessary paperwork timely. Hospital will pay one hundred percent (100%) of Registered Nurse’s premium. Eligible family members may be included in the coverage, and premium cost for such coverage shall be deducted from the Registered Nurse’s paycheck. The Hospital reserves the right to change or modify the plan or conditions of participation in regards to dental and vision insurance after consultation with the Pension and Health Benefit Committee provided for in Article 21.
24.2 The Employer shall notify the Associations of any proposed revisions to coverage, cost, or provisions of insurance plans covered by this Article.
