Dental and Vision Plan. 9.4.1 CSEA-Sponsored Plan: The Town shall pay the premiums for the Dental (EBF Duchess Plan) and Optical (EBF Platinum Plan) coverage either on an individual or a family basis, at the option of the employee.

Dental and Vision Plan. Section 1 Coverage‌ (a) The Employer makes available a dental and vision insurance plan for all employees. These benefits shall be available to each member of this bargaining unit and their eligible dependents on the first of the month following the completion of the employees first ninety (90) days of employment. (b) The dental coverage provides for Class A benefits coverage up to 100% with no deductible within participating member dentists. With non-participating dentists, typical coverage costs for all classes become 90% of reasonable and customary fees, instead of 100%. Class B and C coverage, (combined) has a $25 deductible for single coverage and $75 deductible for Family Coverage. After the deductibles are satisfied, the plan pays 100% of the expense for covered services under Class B, 50% after the deductible for Class C, and 50% of Class D (services to age 19). Detailed coverage information for each class is outlined in the Dental and Vision Summary Plan Description including coordination of benefits language, limitations and exclusions. The Class A, B, and C combined maximum benefit per eligible insured person is $1,000 each calendar year while insured under this policy; and the Class D Services maximum (services to age 19) is $1,000 for each eligible dependent child covered under this policy. In addition, at the employee’s expense, an enhanced plan will be made available. In the enhanced plan, the Class A, B, and C combined maximum benefit per eligible insured person is $2,000 each calendar year while insured under this policy; and the Class D Services maximum is $2,000 for each eligible dependent child covered under this policy. This program is available for all employees and eligible dependents. The Employer will pay 80% of the premium. For the life of this Agreement, employee contributions shall equal the difference between the Employer contribution and the premium cost. As described in the plan booklets provided by the Employer, participation in this plan requires properly signed application forms by each employee and updates as required. (c) The vision benefits listed below show maximum benefits for Covered Vision Services and Supplies which are limited to: Vision Examination $60.00 Lenses Pair Single Vision Lens $72.00 Bifocal Lens $96.00 Trifocal Lens $120.00 Lenticular Lens $144.00 Contact Lens* $180.00 ($60 exam & $72 contact lens in lieu of glasses) Frames $60.00 *See limitation on contact lenses. Dental and Vision Plan. The City shall contribute the sum of thirteen ($13.00) dollars per employee per week as contribution to the Cambridge Public Employees Dental and Vision Fund. Should the Trustees of said fund inform the City and the Union that, at some time in the future the fund requires additional payments, the cost of these payments above $13.00 per week shall be shared on a 50%-50% basis between the City and the employee. Employee shares will be paid through payroll deduction on a pre-tax basis, to the extent allowed by law. Any contribution by the City or subsequent use of said funds, shall be in conformance with applicable state and federal laws regarding the use of same.
Dental and Vision Plan. Effective January 1, 2014, Full-time benefit eligible Employees may enroll in combined Dental and Vision coverage as a separate election from the Medical and Prescription plan noted above. Vision coverage The plan will pay include up to $150 for one set of frames per covered person per twenty-four (24) month period. beginning January 1, 2011. The plan will include RX safety glasses in the vision benefit as of January 1, 2011.

Dental and Vision Plan. The City will contribute $24.70 per week to the Local 25 Health and Welfare Fund for the dental and vision coverage only plan. ($11.70 weekly employee cost.) Effective July 1, 2019, members of this bargaining unit became eligible to participate in the Cambridge Public Employees Dental and Vision Fund’s dental and vision care plans on the same terms as other City employees budgeted to work 37.5 hours or more per week.

The Plan will be the Vision Service Plan - Plan A with benefits at 12/12/24 month intervals and with twenty dollar ($20.00) deductible for examinations and twenty dollar ($20.00) deductible for materials. The County will fully pay the monthly premium for the employee and dependents and pick up inflationary costs during the term of the Agreement.

  • Vision Care Services For purposes of coordination of benefits, vision care services covered under other plans are not considered an allowable expense, as defined in the Coordination of Benefits and Subrogation in Section 7.

  • Dental Services The following dental services are not covered, except as described under Dental Services in Section 3: • Dental injuries incurred as a result of biting or chewing. • General dental services including, but not limited to, extractions including full mouth extractions, prostheses, braces, operative restorations, fillings, frenectomies, medical or surgical treatment of dental caries, gingivitis, gingivectomy, impactions, periodontal surgery, non-surgical treatment of temporomandibular joint dysfunctions, including appliances or restorations necessary to increase vertical dimensions or to restore the occlusion. • Panorex x-rays or dental x-rays. • Orthodontic services, even if related to a covered surgery. • Dental appliances or devices. • Preparation of the mouth for dentures and dental or oral surgeries such as, but not limited to, the following: o apicoectomy, per tooth, first root; o alveolectomy including curettage of osteitis or sequestrectomy; o alveoloplasty, each quadrant; o complete surgical removal of inaccessible impacted mandibular tooth mesial surface; o excision of feberous tuberosities; o excision of hyperplastic alveolar mucosa, each quadrant; o operculectomy excision periocoronal tissues; o removal of partially bony impacted tooth; o removal of completely bony impacted tooth, with or without unusual surgical complications; o surgical removal of partial bony impaction; o surgical removal of impacted maxillary tooth; o surgical removal of residual tooth roots; and o vestibuloplasty with skin/mucosal graft and lowering the floor of the mouth. • The following dialysis services received in your home: o installing or modifying of electric power, water and sanitary disposal or charges for these services; o moving expenses for relocating the machine; o installation expenses not necessary to operate the machine; and o training in the operation of the dialysis machine when the training in the operation of the dialysis machine is billed as a separate service. • Dialysis services received in a physician’s office.

  • Vision Care For the duration of this Agreement, the University will continue to provide a vision care plan for members of the bargaining unit and their dependents with benefit levels not less than those in effect as in the predecessor Agreement.

  • Severance Plan The term “Severance Plan” shall mean the Assured Guaranty Ltd. Executive Severance Plan.

  • Transition Plan In the event of termination by the LHIN pursuant to this section, the LHIN and the HSP will develop a Transition Plan. The HSP agrees that it will take all actions, and provide all information, required by the LHIN to facilitate the transition of the HSP’s clients.

  • Dental Care Plan The Welfare Plan will include a Dental Care Plan which will reimburse members for expenses incurred in respect of the coverages summarized in Appendix "1". The Plan will not duplicate benefits provided now or which may be provided in the future by any government program.
