DURATION AND APPLICATION. 2.1 Membership will only be valid for the period specified on the Membership Notification and Membership Card unless the membership is cancelled by either the Member or Specialty Emergency Services during the specified period, whichever occurs first.
2.2 Application for membership, including the medical questionnaire of Specialty Emergency Services, on behalf of the Member and/or his family, must be completed in full and signed by the Member or his legal representative.
2.2.1 All particulars so declared are deemed to be certified to be correct by the Member’s signature or that of his legal representative.
2.2.2 In case of false declaration and/or omissions, the Agreement is null and void and all Subscriptions will be forfeited and any claim shall be repudiated.
2.3 Minors will not be accepted for individual membership.
2.3.1 A minor’s eligibility is dependent on a parent’s acceptance for membership. Application for a new dependent will entail completion of a new application form.
2.4 Specialty Emergency Services will not commence cover for a new dependent, whether spouse, partner or child, until the Member’s application for that new dependent has been accepted, and until payment of all Subscriptions has been received.
2.5 Specialty Emergency Services reserves the right to decline any application, including that of a new dependent, at Specialty Emergency Services’ sole discretion and without justification.
2.6 The Member agrees and undertakes to subscribe for the services provided herein at the subscription fees stipulated on the application form, invoice or quotation and agrees to be bound by the terms, conditions and exclusions hereof.
2.7 Upon acceptance by Specialty Emergency Services of the Member’s application and receipt of full payment of Subscriptions and issue of a Membership Card & Membership Notification, Specialty Emergency Services undertakes to perform its obligations, including the provision of Assistance as herein defined, to the Member in the event of Bodily Injury or Acute Illness, subject to the terms, conditions and exclusions contained herein.
2.8 Upon receipt of the application from the Member, Specialty Emergency Services shall be entitled to decline such application and its decision shall be final and binding and Specialty Emergency Services shall not be required to furnish or substantiate its reasons for declining any such application.

DURATION AND APPLICATION. 2.1 This Agreement will apply to the Services offered by WithSecure to the Client and will be effective from the date that both Parties sign this Agreement and will continue in full force and effect until terminated in accordance with Clause 12.
2.2 SOW will commence from the Service Date and continue for the period set out therein.
2.3 Any specific terms applicable to the Services not covered in this Agreement will be integrated as a Service Specific Schedule.
DURATION AND APPLICATION. 2.1 This Agreement will operate from date of lodgement with the Employment Advocate till June 21, 2009.
2.2 This Agreement is binding on the parties to the Agreement and employees of the Company based at the Kurnell Refinery in the classifications described in this Agreement (the employees).
2.3 During the operation of this Agreement, and subject to the requirements of the Workplace Relations Act 1996 (Cth) (the Act), no other existing or future award, industrial agreement or employment agreement will apply to employees of the Company engaged in the classifications contained within this Agreement. As it comprehensively deals with their employment conditions, it also displaces any protected award conditions, protected notional conditions and protected preserved conditions (as those terms are defined by the Act) in any award that would otherwise apply to the employees (including those set out in Appendix 6).
DURATION AND APPLICATION. Market supplements approved under this policy for a particular job(s) will be paid for an initial period which will not exceed three years. The duration of the market supplement will be clearly identified in recruitment and appointment discussions and documentation and contractual variations (for existing staff). Any continuation of a market supplement payment beyond the initial period will be subject to the review clause below. Market supplements for a particular job will be paid to existing staff as well as new recruits. For employees working less than full time, the supplement will be pro–rated on the basis of hours worked. A market supplement is a payment awarded on a temporary basis. It does not affect the grading of the job and is not included for the purposes of calculating leave entitlement in accordance with Appendix B. Market supplements are pensionable. Criteria There will need to be strong and clear evidence to justify a market supplement and this will need to include: • Significant information which demonstrates there is a serious recruitment and/or retention problem. This would include evidence of high vacancy levels, high turnover or a low response to advertisements in terms of quality and/or quantity of candidates AND • Independent evidence that the same or very similar job is being paid at a significantly higher rate in the relevant labour market(s). The Council’s recognised Trade Unions will be consulted about any proposals to introduce, vary or cease market supplements. Market supplements should be introduced in accordance with this policy. A decision to introduce, vary or cease a market supplement will be made by the County Solicitor unless he/she decides that the decision needs to be referred to the Management Board because there would be significant implications for the Council.
DURATION AND APPLICATION. The undertakings in this Clause 12:
(a) remain in force from the date of this Master Agreement until, following the end of the Initial Term, the Equipment is sold or otherwise disposed of or returned to the Lessor under the terms of this Master Agreement; and
(b) apply save to the extent, in any particular case, the Lessor gives to the Lessee its prior written consent to the contrary.
DURATION AND APPLICATION. This Agreement is for the period of one (1) year following execution and shall be automatically renewed for one (1) year on each anniversary date, unless canceled in writing. Either party may cancel this Agreement on a fifteen (15) days written notice. The terms of this Agreement and mutually agreed upon signed written addendum shall govern all shipments tendered by Broker to Carrier.
