EMPLOYEE REPORTS. 14.01 The Employer shall notify an employee in writing of any expression of dissatisfaction concerning her/his work within ten (10) working days of the event of the complaint. This notice shall include particulars of the work performance which led to such dissatisfaction. This is not intended to be disciplinary but is intended to be used to enhance the relationship and communication between the Employer and employee.
EMPLOYEE REPORTS. The Employer and/or the Department will, upon request of the Union, provide any regularly produced computer runs containing non-confidential statistics of the Union's bargaining unit members. This will include one (1) printout annually showing names of all bargaining unit employees and monthly information currently furnished. Any cost incurred in compiling and photocopying these materials under this Agreement shall be billed to the Union.
EMPLOYEE REPORTS. A. The Administrator of the Trust Funds shall make available to the District Council No. 36, the LMCC and the FCA affiliate organizations, a monthly roster of all persons for whom contributions have been made in the past and who go for a period of sixty (60) days without contributions being made in their behalf and/or who continually have only minimum hours reported to qualify for trust fund coverage.
B. Every journeyman employed hereunder shall be required to fill out and mail any reporting card to the Administrator of the Trust Funds each week, upon request, or at such other times as the Administrator may require. Upon the failure of a journeyman to comply with these provisions the District Council shall take such disciplinary action against the journeyman as may be necessary to insure compliance; and the Trustees of the Trust Funds may hold in abeyance the benefits of the journeyman failing to make said reports as may be required to obtain compliance herewith.
EMPLOYEE REPORTS. The district encourages any employee who believes that they are a sexual harassment victim to directly inform the harasser that the conduct is unwelcome and must stop. A co-worker or other employee who witnesses sexual harassment should either intervene on the victim's behalf or immediately report the harassing conduct. The district does not, however, require the employees or witnesses to confront the harasser. An employee who believes they are a sexual harassment victim, or who knows of conduct that may constitute sexual harassment should report the alleged conduct to his or her immediate supervisor, or to any supervisor or manager, or to the District Compliance Officer. The report may be verbal or written. Using a formal reporting form is not required, although one is available from the school office, the district personnel office, or the District Compliance Officer if the employee wants to use a written form. A supervisory or management employee receiving a written sexual harassment complaint shall immediately forward it to the designated District Compliance Officer. If a supervisory or management employee receives a verbal complaint, they shall notify the District Compliance Officer immediately and, as soon as possible, forward a written report to the District Compliance Officer. The report shall not be screened or investigated before it is forwarded to the District Compliance Officer. Failure to forward any sexual harassment report or complaint shall result in disciplinary action.

EMPLOYEE REPORTS. The Borrower shall submit a report annually to the Lender and RD as of December 31, indicating the total number permanent, part-time and seasonal employees.
EMPLOYEE REPORTS. Reports on employees will be requested only by the SUBSCRIBER’S designated representatives. SUBSCRIBER and SUBSCRIBER Employees will be forbidden to attempt to obtain reports on themselves, associates,
(1) the consumer has been given a clear and conspicuous written notice, in advance (in a document that consists solely of the disclosure), that a consumer report may be requested for employment purposes; (2) the consumer has authorized the SUBSCRIBER, in writing, to procure the report; (3) the information in the consumer report will not be used in violation of any applicable federal or state equal employment opportunity law or regulation; (4) before taking adverse action, in whole or in part on the report, SUBSCRIBER will provide the consumer a copy of the credit report and a description of the consumers rights under the FCRA.

The City will furnish the Union with a monthly transaction report outlining all new hires, their classification and status that is, temporary, seasonal, regular, etc. This report will also show any promotions and dates of hire, transfer, layoff, or movement into or out of the bargaining unit.
EMPLOYEE REPORTS. Bargaining unit employees working with the public may request time away from the public area if the employee has been threatened either verbally or physically by an applicant. The supervisor will evaluate the situation and allow the employee time away from the public area when it appears helpful and when the work requirements permit. The employee will be given time to prepare a report of the incident and present it to the immediate supervisor.
