Group Health and Dental Insurance. 26.01 (a) It shall be voluntary to join either the Health Plan, Dental Plan or both. The Employer shall deduct the employee's share of the cost of premiums of this Plan when so authorized by employees who have completed their probationary period.

Group Health and Dental Insurance. For the 2017-2018 contract year, the Hastings Public schools shall make available to the certificated employees subject to this Negotiated Agreement a group health and dental insurance plan through the Educators Health Alliance (EHA), the EHA Choice Plan with employee elected coverage levels of (1) the Blue Preferred $900 Deductible Plan with 100% A, B, & C dental coverage (PPO) or (2) the Blue Preferred $3,500 Deductible Health Savings Account Eligible Plan with 100% A, B, & C dental coverage (PPO) under the terms and conditions set forth in this Article 6, providing for a schedule of benefits as set forth in Appendix D and D-1, and in effect for the term of this Agreement.
Group Health and Dental Insurance. ACTIVE EMPLOYEES A. Active Employees’ Health and Dental Insurance Eligibility. Regular full-time employees and their eligible dependents are eligible for coverage under the District-selected medical and dental insurance plans, which includes a Flexible Benefits Plan, effective the first of the month following date of hire and upon proper application, acceptance, and continuous service. Regular part-time employees and their eligible dependents are eligible for coverage effective the first of the month following date of hire and upon proper application, acceptance, and continuous service, except that the District’s contribution is limited to one-half (1/2) of the District’s contribution as provided below. The medical and dental plan details are set forth in a separate booklet furnished to eligible employees, is referenced only to provide additional information, and is not incorporated into the MOU.

D. The health and dental insurance premium rates shall not increase more than 15% per year from one plan year to the next for PBA bargaining unit members.

Group Health and Dental Insurance. .1 The Board of Education shall make available an insurance plan as established by the provisions of this agreement for all full-time members of the bargaining unit, as defined in section 1.1.4 of this agreement. The Board shall pay the individual premium equal to that negotiated in the teachers’ contract. If the premiums increase the following will occur: a. A committee will be established to attempt to locate a different insurance carrier with comparable coverage at a lower cost. The committee will consist of five (5) certified staff members in the bargaining unit appointed by the Carterville Education Association (CEA), two (2) support staff members in the bargaining unit appointed by the Union, one (1) non-certified staff member and one (1) District administrator appointed by the Board. Once a carrier is identified by the committee the new proposal will be presented to all eligible employees for approval and to the Board for ratification. b. If step 1 fails, the premium increase in excess of five percent (5%) in the second year will be equally divided between the Board and the employee with each paying fifty percent (50%) of the increase, unless the majority of the c. eligible employees vote to reduce coverage or raise the deductible to keep the increase to five percent (5%) or less. .2 Members of the bargaining unit shall have the option of taking dependent coverage under the District's group hospital and medical insurance program if they pay the additional premium for full family coverage. .3 The Board reserves the right to change or provide alternate insurance carriers, health insurance providers, or to self- insure so long as the new coverage and benefits are sub- stantially similar to the insurance coverage being provided as of the date this Agreement becomes effective. In the event the Board decides to change insurance carriers the Union shall be notified at least sixty (60) days in advance of the change so that the Union will have opportunity to review the new benefits prior to implementation. .4 During the term of this Agreement any increased health insurance benefits which may be provided to the District's certified staff members through negotiation of a new contract will automatically be provided to members of the bargaining unit effective the same date these benefits are made available to District certified staff employees.
Group Health and Dental Insurance. Effective as of the Employment Termination Date, the Executive and his eligible dependents shall cease to be eligible to participate in the Company's group health and dental insurance plan or plans; provided, the Executive and his covered dependents shall be eligible for COBRA continuation coverage under any plan of the Company which is a group health plan subject to the provisions of part 6 of Title I of ERISA, with premiums payable by the Executive at the Company's continuation coverage rates.

Joint Insurance Committee Each year during the term of this agreement, the Association and Board shall establish a Joint Insurance Committee consisting of three (3) members of the Board of Education, and three (3) members of the Association. The Board of Education shall choose which of its members shall serve on this committee, and the Association shall choose which of its members shall serve upon this committee. The purpose of this committee is to enable the Association to assume responsibility for decisions concerning the group health and dental insurance benefit plan offered to full-time teachers and implementation of the Association’s chosen benefit plan upon commencement of the then current school year. The members of the Joint Insurance Committee shall mutually agree on the dates and times that the Joint Insurance Committee shall meet.
