Group Medical Benefits. Under the terms of the Flexible Benefit Plan, an employee who enrolls in Freshwater’s group medical plan will automatically pay his/her share of the premiums for that coverage (if any) on a pre-tax basis through the Flexible Benefit Plan. An employee who has enrolled in the group medical plan and wishes not to pay premiums on a pre-tax basis must affirmatively elect to pay the premiums with after-tax salary deductions by completing and submitting a separate form, which is available upon request. I elect to receive the following reimbursements for the plan year.

Group Medical Benefits. Gxxxxxxxx shall be entitled to continue any existing group medical benefits he may have elected under Section 5(c) of the Initial Consulting Agreement, subject to the terms thereof.
Group Medical Benefits. Xxxxx shall be entitled to receive continuation coverage in Sapient's medical and dental plans pursuant to the provisions of the Consolidated Omnibus Budget Reconciliation Act of 1985 ("COBRA"), such continuation coverage shall be at Xxxxx'x expense, and in all respects be subject to the requirements, conditions and limitations of COBRA and such plans, which may be amended from time to time.

Group Medical Benefits. 23.01 The Employer agrees to pay as of January 1, 2014, seventy (70%) percent of the total premium cost for the Encon Group Inc. Insurance plan and the Employee agrees to pay thirty percent (30%) of the total premiums. An Employee Assistance program for regular full time Employees and regular part time Employees who have completed the probationary period is also provided. 23.02 Life Insurance, Dependent Life Insurance and AD&D coverage is mandatory for all regular full time employees with the premium costs at seventy per cent (70%) for the employer and thirty per cent (30%) for the employee. 23.03 The Employer agrees to notify the Union in writing of any changes to the insurance carrier and to discuss such changes one month in advance of any such change coming into effect. Decisions regarding coverage shall be determined by the insurer of such benefits and shall not be subject to the grievance and arbitration process. Participation in the benefit plan is mandatory for those Employees who are eligible unless they provide proof of similar or better coverage elsewhere. 23.04 Any money realized by the Employer through the operation of the Employment Insurance Rebate Program may be used by the Employer to off-set the cost benefit premiums.

Group Medical Benefits. The COMPANY agrees to provide health Insurance through Family Medical Care Plan (FMCP) through December 31, 2018. (1) Effective January 1, 2019 and for the term of this Agreement, active employees will be eligible to participate in the Agreed upon CDHP medical plan, either through the present carrier/administrator or through another reputable carrier/administrator selected by the Company. Unless expressly provided otherwise by this Agreement, the terms and conditions of the CDHP medical plan document are incorporated herein by reference. Certain Plan benefits are summarized below. The COMPANY will provide $4,931 single/$9,862 family, annually, to the Company’s carrier/administrator for these retirees, SUBJECT TO THE TERMS OF THIS AGREEMENT AND LETTERS OF UNDERSTANDING. (a) (4) For such retirees after age 65, the Company will provide $669 single/$1,338 family, annually, in order to purchase individual coverage through the Company’s post 65 carrier/administrator.
