Health and Other Insurance Benefits. Bargaining unit employees may participate in health and other insurance benefit plans offered by the State pursuant to the terms of those plans.
Health and Other Insurance Benefits. ● Employee may elect COBRA continuation coverage under the Company group health or other insurance benefit plans, to the extent permitted under the applicable plan terms and COBRA. Any conversion or other rights shall be governed by the terms of the applicable plans.

Employee shall have the option to participate in the Bank’s health insurance and other insurance benefit programs during his employment on the same terms and conditions as other Bank employees. Employee acknowledges that the Bank’s health insurance and other insurance benefit plans are subject to change at any time at the discretion of the Bank.

Executive (and his dependents) shall be entitled to participate in such retirement plans and such health, hospitalization, dental, life insurance, and other insurance plans as may be adopted by Employer’s Board for similarly-situated executive officers of Employer and their dependents to the extent the provisions, rules, and regulations of such plans make Executive and his dependents eligible for participation therein.
Health and Other Insurance Benefits. Employee shall be entitled to participate in such health, hospitalization, dental, life insurance, and other insurance plans as may be adopted by Employer’s Board of Directors for similarly situated executive officers of the Employer and their dependents to the extent the provisions, rules, and regulations of such plans make Employee and his dependents eligible for participation therein. In addition, Employer will provide Employee with a minimum of $300,000 in group term life insurance coverage during the Term.
Health and Other Insurance Benefits. A. Health Insurance - The City shall continue to make available group health, dental, and vision benefits to all HBFA employees.
Health and Other Insurance Benefits. Effective as soon as open enrollment can be completed following the ratification of this Agreement, and continuing through the end of the last payroll period in 2014, the following insurance and other benefits will be offered:

Health and Other Insurance Benefits. Subject to the last sentence of this paragraph (b), the Company will continue to provide to Executive, on the same terms and conditions (including cost) provided to its active employees generally, coverage under its major medical, dental, vision care, and group term life insurance benefits plans, during the twelve (12) month period following the Termination Date. The Company shall adopt such amendments to its employee benefit plans and insurance policies as are necessary to effectuate the provisions of this Agreement. However, notwithstanding the above, the benefits and coverage provided for in this paragraph 4(b) shall terminate in the event the Executive or his spouse becomes eligible for comparable employer provided coverage from any other employer(whether or not the Executive and his spouse accepts or elects any such alternate coverage).
Health and Other Insurance Benefits a) The District shall provide health insurance for the duration of this agreement.
b) Effective January 1, 2016, and for the duration of this Agreement, the District will pay an amount equal to 100% of the monthly premium for the Kaiser Bay Area plan for the employee’s selected coverage level (employee, employee +1, employee +2), which will be the employer’s share, every month towards medical insurance for each employee covered by this Agreement. All premiums that exceed the employer’s share every month will be paid by the individual employee and will be deducted from the employee’s paycheck each pay period.
c) The District shall continue its vision plan for the duration of this Agreement, at no cost to the employee.
d) The District shall continue to provide family dental coverage for the duration of this Agreement. The District shall pay 90% of the monthly premium. The remaining 10% of the monthly premium shall be paid by each employee and shall be deducted from the employee’s paycheck each pay period. Should the District change plans during the life of this agreement, a copy of the Summary Plan Description for the new Plan shall be made available to employees at the time of the change.
e) If satisfactory evidence of medical coverage is produced by the employee, the employee may elect not to be covered by the District's medical insurance plan. In each case, the District shall pay $500 per month to the employee. Eligibility for this benefit shall continue in accordance with the current contract.
f) The District shall continue a flexible benefit plan pursuant to IRS Code section 125. Such plan shall provide for the cash option as outlined in (e) above as well as an option to pay health insurance premiums, medical expenses not covered by insurance and dependent child care.
g) The District shall provide Long-Term Disability insurance at no cost to employees for the duration of this Agreement.
