Hospital, Medical and Surgical Insurance. The Company will provide full semi-private hospital coverage for each Employee, spouse, and own unmarried children under 21 years of age. If the Employee is covered under the spouse’s insurance plan, the Company will not provide double coverage.

Hospital, Medical and Surgical Insurance. The Township agrees to pay for hospitalization and medical insurance coverage for all of its employees in the bargaining unit, their spouses and their unmarried dependent children under age 19, and their unmarried dependent children under age 23 who are full- time students in a course of study or training recognized by the medical insurance carrier. Said coverage is currently furnished by Guardian Insurance Company. The above named insurance may be changed to another medical insurance carrier selected by the Township under a plan providing benefits as substantially equivalent as possible (or better) to those pertaining under the Guardian Insurance Company plan enjoyed. In any event the employees' hospital, medical and surgical insurance will be covered by the Township continuously. Employees shall cooperate in completing such forms and furnishing such information as is requested by the applicable health care insurance provider. If requested by the Township, an employee shall submit to an annual physical examination to be conducted at the expense of the Township. The examining physician shall be designated by the Township and the results of said examination shall be made available to the Township and the employee. The Township agrees to pay for prescription drug coverage for all of the employees and their eligible dependents, except such coverage shall not apply to probationary, temporary, part-time or seasonal employees or their dependents. Such coverage shall pro- vide for covered prescription drugs in accordance with Time Insurance Company, or other applicable prescription drug insurance plan policies. Employees shall cooperate in completing such forms and furnishing such information as is requested by such prescrip- tion insurance provider. The employees under this contract agree to said Township having health insurance with a $500.00 deductible program. This is being done so that the Township may purchase the health insurance at a cheaper rate. The Township agrees that the deductible for the employee and dependents will remain at $100.00 and that after showing that the $100.00 deductible has been met (bills paid) by the employee for himself or dependent, to the employer representative, the employee shall be reimbursed for all subsequent bills until the $500.00 limit is reached, meaning the Township through its savings in premiums will pay additional bills (reimburse the employee) until $500.00 is reached.
Hospital, Medical and Surgical Insurance. The Employer agrees, for the life of this Agreement, to maintain group hospital, medical, and surgical insurance benefits of Blue Cross/Blue Shield MVF-2 with ML, Master Medical option IV, two dollar prescription drug and Emergency Room Care / (XXX-XX) riders for the employee and his/her dependents, the cost of the monthly premium to be borne by the Employer. The Employer further agrees, for the life of this Agreement, to pay bills incurred by employees and their dependents for dental and optical care and related services up to a maximum of four hundred ($400.00) during the fiscal year which shall apply to the employee and shall cover all such services rendered to the employee and their dependents. To the extent that there remains any unpaid portion of this benefit at the end of the fiscal year, such unpaid portion shall not be paid to the employee at the conclusion of the Employer's fiscal year or by July 1.

  • Hospital and Medical Insurance The University shall make available health insurance to the employees covered by this agreement to the same extent and in the same manner as is available to other University employees, such as Faculty and the Executive, Administrative and Professional Staff employees. It is the University's goal to have the same health insurance plans offered uniformly to all University groups and employees.

  • MEDICAL AND HOSPITAL INSURANCE 14.1 Current practices will prevail for the duration of this Agreement, except that any changes in medical or hospital insurance plans, including the premium payable by employees, applicable to the majority of those employed in the Public Service for whom the Treasury Board is the employer, will during the life of this Agreement be applicable to the employees under this Agreement.

  • Medical Insurance Upon termination of employment, the Executive shall be entitled to all COBRA continuation benefits available under the Company's group health plans to similarly situated employees. To the extent permitted under Code Section 409A, during the applicable Payout Period, the Company shall provide such COBRA continuation benefits to the Executive at the active employee rates similarly situated employees must pay for such benefits.   • Basic Medical Insurance All regular Employees may choose to be covered by the medical plan for which the British Columbia Medical Plan is the licensed carrier. Benefits and premiums shall be in accordance with the existing policy of the plan. The Employer will pay one hundred percent (100%) of the regular premium.
