Fee Structure. In consideration of Consultant providing services, Municipality shall pay Consultant for Services performed in accordance with Exhibit A – List of Services and Fee Schedule.
Fee Structure. The City agrees to pay the AARC for services to be rendered hereunder the following, where applicable, for animals brought to the AARC by an official of the City or a resident of the City (except items d. and e. when citizen is specifically requesting these services for his own pet) and registered on an Animal Identification Form and not identified as a returned or reclaimed animal in the animal disposition. The City reserves the right to change or eliminate any fee waiver for City residents at any time during this Agreement upon written notice from the Joplin Director of Health and Welfare to the AARC. All returned or reclaimed animal fees are to be paid by animal owner.
(a) Animal Intake and Tracking Fee of Eight and 67/100 ($8.67) dollars per animal.
(b) Animal Sheltering Fee of Eight and 13/100 ($8.13) dollars per day, not to exceed seven (7) days, (excluding wild animals).
(c) Quarantine Fee of Fifteen and 62/100 ($15.62) dollars per day, not to exceed ten (10) days.
(d) Euthanasia Fee of Twelve and 32/100 ($12.32) dollars per animal.
(e) Cremation Fee of Fifteen and 79/100 ($15.79) Dollars or .55/pound for large wild animals, for each animal registered on an Identification Record Form and brought to the AARC by an Animal Control Officer of the City that requires cremation.
(f) Small Animal Cremation Fee of Five and 22/100 ($5.22) dollars per small wild animal such as birds, squirrels, rabbits, rodents, or other small wild animals under 3 lbs. in weight.
(g) AARC out-of-pocket veterinary charges due to its acceptance of a sick or injured animal, as determined in paragraph 10 herein.
(h) The AARC agrees to provide to the City, by the 10th day of each month, an invoice for all charges incurred, pursuant to this Agreement, during the previous month, in a format approved by the City. The City agrees to process all invoices by the end of the month presented provided such invoices are received by 10th of each month.
(i) The AARC agrees that the fee for services charged by AARC to City shall be the same fee charged for like services to other governmental entities that have a contract with AARC; although for convenience, flat fees may be charged to other governmental entities. The calculation utilized by AARC to arrive at a flat fee for such government entities shall be the same calculations utilized by AARC to arrive at the fee charged to City for such services.
Fee Structure.
(a) The Contracting Officer reserves the unilateral discretion to allocate between 15 percent (%) and 25% of the Available Fee for the Base Period and Option Period(s) (if exercised), to each fiscal year as described in this Clause; and as adjusted in the Section B Clause entitled, Changes to Contract Cost and Contract Fee. The Contractor will have the opportunity to earn 100% of the Available Fee, as adjusted, within a Contract period.
(b) The Available Fee shown in Table B.4-1, Contract Cost and Contract Fee, can be earned through objective fee components and/or subjective fee components. The performance measures for these components and the Available Fee for the period allocated to the fiscal year are provided in the Section J Attachment entitled, Performance Evaluation and Measurement Plan (PEMP). The PEMP may contain annual and multi-year performance measures.
(1) Available Fee for the period allocated to annual performance measures may only be earned in that fiscal year. Allocated Available Fee for the fiscal year not earned in the fiscal year for an annual performance measure is unavailable and not payable to the Contractor. The Contractor forfeits any rights to unearned fee. The Contracting Officer reserves the unilateral discretion to determine how any unearned fee will be utilized.
(2) Available Fee for the period allocated to fiscal years for multi-year performance measures may be earned incrementally or upon final fee determination. Allocated Available Fee not earned for a multi-year performance measure is unavailable and not payable to the Contractor. The Contractor forfeits any rights to unearned fee. The Contracting Officer reserves the unilateral discretion to determine how any unearned fee will be utilized.
(3) Provisional Fee is defined as Available Fee that is paid contingently during an annual performance period. Provisional Fee may become earned fee upon the final fee determination.
(4) Incremental Fee is defined as Available Fee that the Contractor may earn by achieving a specific, fee-bearing, performance measure event.
(5) Individual performance measures may require the Contractor to exceed approved baseline performance to earn 100% of the fee allocated to that performance measure.

Fee Structure. Customer agrees to pay fees for the Initial Services as set forth in the Services and Charges Schedule. If Customer elects to receive Services that are not specifically set forth in the Services and Charges Schedule, Customer agrees to pay fees as mutually agreed upon for such Services.

6.2. The Agent shall be reimbursed by the Client for any agreed out-of-pocket expenses incurred.
Fee Structure. Any employee currently employed or hired into the bargaining unit covered by this Memorandum who is not a member of the Teamsters shall, beginning with the third full pay period after individual written notice has been given in accordance with Article 21 (Fair Share) and until the expiration of this Article, pay to the Teamsters a Fair Share Agency Shop Service Fee for services rendered by the Teamsters. The Service Fee shall be equal to the monthly dues of regular membership, less non- chargeable costs as defined in Article 21 (Fair Share), and/or by statutory or case law, but in no event to exceed ninety-five percent (95%) of the regular membership dues. The Teamsters shall have the exclusive right to decide and determine whether any such action or proceeding referred to in the above paragraph shall or shall not be compromised, resisted, defended, tried, or appealed, but shall consult with the County prior to making any such decision or determination.
Fee Structure. The billing for this matter will involve two types of items--(1) "Fees" the amount due for the attorney's services and the services of other professionals within the firm, and (2) "Costs," the expenses for the services of outside consultants or vendors, and any out-of-pocket expenses of the firm, including such items as messenger or process service fees, investigators, filing fees, recording fees, copying charges, expert witness fees, court reporter fees for depositions, travel expense, and parking. This list is not intended to be an exhaustive list of all costs which could be incurred in your case.

CTO will provide written notification to PO of any change in the fee structure sixty (60) days in advance of such change taking effect. CTO Stream is web-based management system that allows for the electronic submission of applications and documents for research ethics review and oversight by an REB of Record of a Clinical Study using the CTO Streamlined Research Ethics Review System (“SRERS”). CTO Stream requires a computer/tablet/mobile device, web browser and secure internet access. CTO Stream supports access through Internet Explorer, Firefox, Google Chrome and Safari. Users are encouraged to use the latest version of their web browser and browser support is subject to ongoing support by the browser manufacturer. For more information on supported browser versions, please access CTO Stream policies and procedures available at xxx.xxxxxxxxx.xx. Separate portals are maintained with different URLs for Clinical Study applicants and the Research Ethics Boards to access CTO Stream. All users are required to enter a username and password to access the system. The system uses strong passwords of 8 characters or more using capital and lowercase letters and numbers. User accounts are tied to levels of access according to user roles to ensure a controlled and secure environment. In order to obtain a username and password, individuals must electronically agree to the confidentiality clauses as stipulated by CTO.
Fee Structure. The intent of this fee structure is to implement the provisions of the JWOD Act and 41 C.F.R. § 51-3.5 by defining a more transparent process for the delivery of CNA services to the qualified NPAs. The flexibility in the fee structure is intended for the CNA to generate revenue within their approved business plan, to incentivize the expansion of employment opportunities and to assist in start-ups. All fees collected under this Cooperative Agreement are considered Program Fee.
a) The parties agree to an Award Fee Simulation (Pilot test) (Attachment 3) for the first two (2) years of this Agreement. Based on results of the Simulation, at least 90 days before the start of the third year, an Award Fee structure will be negotiated for the final third year of this Agreement.
