Insurance Benefits While on Leave. At least ten (10) days prior to a change by the Board of the employment status or insurance coverage of an employee who is on sick leave and whose sick leave has expired, the Board shall notify the employee of his/her options and responsibilities. At that time an employee is officially notified that he/she has been granted a leave of absence, the Board shall notify the employee of his/her options and responsibilities with regard to maintaining insurance and with regard to his/her return to duty. Such notification as provided above shall be sent by certified mail, return receipt requested, to the last address provided the Board by the employee as shown on the employee’s paycheck.

Insurance Benefits While on Leave. While on unpaid leave of absence, the College will continue to pay its share of all group insurance benefits consistent with the procedure set forth in this Agreement such that said group coverage is continued through the end of the calendar month in which the employee last actually worked, or was on paid leave status, whichever is later. Beginning with the first day of the calendar month immediately following the month in which the employee last actually worked or was on paid leave status, whichever is later, the employee shall be responsible for paying for insurance benefits consistent with the provisions of COBRA.

If the nurse has contacted the Human Resources 30 Department within the required time, he/she may continue to receive coverage 31 under Hospital group health insurance coverage by paying 100 percent of the 32 premium (Hospital portion plus the nurse portion). If after 24 weeks the nurse 33 has exhausted all accrued paid leave time, he/she will be offered continued 1 health coverage through COBRA. 3 A nurse on leave of absence may choose to cancel his/her group health 4 insurance coverage during the leave. In that event, their group health insurance 5 coverage will be reinstated upon the nurse’s return to work by completing a new 6 enrollment form in the Human Resources Department.

If cancelled, their group health insurance coverage will be reinstated upon the nurse’s return to work by completing a new enrollment form in the Human Resources Department.
