P ersonnel File. Personnel records will be maintained for each employee. Information contained in the personnel record will include: employment application supporting materials, performance appraisals, records of payroll activity, license training records, letters of commendation and recognition, and records of disciplinary action. By appointment, employees may inspect their personnel records. Employees will be given the opportunity to provide a written response to any written evaluation, disciplinary actions or other materials included in the personnel file and such comments shall be included in the employee’s personnel file. Documentation regarding conditions at date of hire (rate of pay, unit, shift, hours of work), reason for termination, change in employment status, pay or shift and leaves of absence shall be in writing with a given to the employee upon request.

Documentation placed into the employee’s file regarding conditions at date of hire (rate of pay, unit, shift, hours of work), reason for termination (quit, discharge or retirement), change in employment status, pay or shift and leaves of absence shall be in writing. Upon request, an employee will be given a copy of any material in the employee’s personnel file.
P ersonnel File. The Superintendent shall maintain, for official school district purposes, one personnel file for each employee. This file shall be kept under conditions that ensure its integrity and safekeeping. Documents associated with disciplinary action, may be placed in an employee’s personnel file only after the employee has signed the document acknowledging receipt thereof or has refused to sign as acknowledged by a witness, and has been given the opportunity to read and discuss the contents with the issuing supervisor. P ersonnel File. Each employee will have the right, upon request, to review the content of his/her personnel file (except for pre-employment references) in the presence of the Superintendent or his/her designee. At the employee’s request, a representative of the employee’s choosing may accompany the employee in this review. The employee may be required to sign a log indicating he/she has reviewed the file at the time of said review. Any employee will have the right to attach dissenting material to any item in his/her personnel file. The employee will receive a copy of any item placed in his/her personnel file that was not originated by the employee.
P ersonnel File. An employee shall be permitted to inspect his own personnel file at a reasonable time and upon making such arrangement with the Fire Chief. If he wishes to file a written statement with respect to any information contained therein, he may do so and said statement shall remain a part of the employee’s personnel file so long as the information he is responding to remains a part of that file. An employee will be given an opportunity to read and initial any notes or memoranda, including written reprimands and suspension notices, before placement in the employee’s personnel filed. An employee’s initials on a document shall mean only that he has seen the document and not necessarily that he agrees with its contents. If the employee refuses to sign or initial a document, the supervisor shall attach a notation to the document that the employee has seen the document but has refused to sign or initial it.
P ersonnel File. 34 4.2.1 The personnel file of each unit member shall be maintained at the District's central 35 administration office under the supervision of the Assistant Superintendent and Director of Human 36 Resources who are the custodians of the District. No adverse action of any kind shall be taken 37 against a unit member based upon materials which are not in the personnel file. Personnel files may 38 be removed from the Human Resource Office under the following circumstances: 1) when requested 39 by the Board or the Superintendent and 2) when subpoenaed by a court of competent jurisdiction.
P ersonnel File. A. Only one (1) official file shall be maintained. No complaints or other documents which will be used to evaluate or discipline an employee shall be placed in the file unless the employee has had an opportunity to read such material. After investigation, if a complaint is found to be credible, an employee will be afforded due process as defined in Section 4.11 for the ensuing discipline. The employee shall acknowledge that he/she has read any complaints or other documents used to evaluate or discipline by affixing his/her signature on the copy to be filed. However, any documents related to evaluation or discipline which have not been reduced to writing within forty-five
P ersonnel File. To review upon reasonable request, in the District Office, the contents of the TEACHER’S own personnel file. It is understood that only one personnel file is to be maintained for each TEACHER. Any material shall be placed in said file no later than thirty
P ersonnel File. There shall be only one (1) official personnel file for each educator. The educator shall have an opportunity to respond to any material placed in his/her file, provided such is submitted within twenty-five (25) educator employment days of the date a copy of such materials is furnished to the educator. No evaluation material shall be placed in such file unless the educator has had the opportunity to see it. Any material which may be used to form the basis of disciplinary action against an educator shall be placed in the educator’s personnel file no later than the end of the school term during which such comments were generated. All educators shall have the right to review their own personnel file and may be accompanied at such review by a representative, provided such review shall occur during normal business hours and shall not in any manner inconvenience the operation of the Administration. A representative of the Administration may be present at the time of such review. Nothing shall be permanently or temporarily removed from such personnel file without the consent of the Administration and the educator.
P ersonnel File. Each teacher shall have the right, upon request, to review in the presence of management and/or make one (1) copy the contents of his/her personnel file. At the teacher's request, a representative of the WEA may accompany the teacher in this review. Additionally, the teacher may request that a copy of his/her file will be made available within 24 hours of said request. A teacher will receive notification when a document is placed in his/her personnel file. Said notification will be given within two (2) working days of document placement. A teacher will be allowed to place in his/her personnel file a written response to any document included in said personnel file.
