Program Application. 3.1. The Member may submit one or more applications for Subsidy on the terms described below and pursuant to the terms and conditions of the Program Guide.
3.2. The Member may request disbursements of Subsidy on behalf of applicants for the Subsidy (“Applicants”) by completing (with Applicants) and submitting a request in the form provided by the Bank (“Application”) in accordance with the Bank’s procedures and policies for the completion of the Application in effect at the time of submission. If an Application is awarded a Subsidy, the Member agrees to pass on the full amount of the Subsidy to the Applicant.
3.3. The Member is responsible for the content, accuracy and completeness of each Application, and for obtaining all authorizations necessary to provide each Application (and all associated documents and information) to the Bank.
3.4. The Bank shall have no obligation to provide a Subsidy under this Agreement unless sufficient funds are available under the Program to fully disburse the amount requested and the Bank is satisfied as to compliance with the terms of the Program Guide at the time of funding.
3.5. The receipt or approval of this Agreement by the Bank shall in no way constitute a firm or binding commitment on the part of the Bank to provide the Subsidy. In consideration for the issuance of the Subsidy, the Member acknowledges that this Agreement is subject to the terms specified in the Program Guide and this Agreement.
Program Application. The Property Owner hereby represents and warrants to the Authority that the information set forth in the Program Application submitted to the Authority in connection with its request for financing is true and correct as of the date hereof, and that the representations set forth in the Program Application with respect to the Property and the Property Owner are true and correct as of the date hereof as if made on the date hereof.
Program Application. 1. A Vendor application will be required for participation in the WIC Program from:
a. A new owner of Vendor’s business who wishes to participate as a WIC Vendor;
b. A Vendor who is disqualified after Vendor’s period of disqualification has expired; or
c. A current WIC Vendor in good standing wishing to continue as a WIC Vendor after expiration of its current WIC vendor agreement.

Program Application. 1. An employee who meets eligibility requirements must complete a memorandum requesting application to the Employee Leave Donation Program. Along with the memorandum, the employee must submit a physician’s statement verifying the terminal nature of the injury or illness and the need for the employee’s presence for care of the spouse or child. The employee must submit a physician’s statement biweekly after initial acceptance to the program.
2. If the employee is unable to complete the application due to incapacitation, the employee’s relative or agent may submit an application for him. A physician’s statement must accompany any application to the program.
3. Upon receipt of an application for assistance through the Employee Leave Donation Program, the supervisor forwards the paperwork to the Personnel Director through the chain-of-command.
4. The Sheriff ensures that all eligibility requirements are met. The Sheriff notifies the employee or the employees’ agent of his decision when he approves or disapproves the application.
5. Fulltime non-probationary employees who want to contribute leave must fill out an Employee Leave Request form, checking the type of leave they wish to donate and the number of hours they wish to donate. Employees must donate a minimum of eight (8) hours. Compensatory time donations are not allowable. Employees must write, “Employee Leave Donation” on the top of the form along with the employee’s name that they want to receive the leave donation. Employees submit this form to the Sheriff through the chain-of-command. Contributions are irrevocable.
6. The Sheriff’s administrative assistant deducts the leave credit from the contributing employees’ leave balance and adds it to the balance of the employee receiving the donation.
Program Application. The Voluntary Separation Incentive Option (VSIO) will be activated by the Corporation to assist individuals affected by specific business change. When it is deemed appropriate by the Corporation to activate this program the City Manager will identify the specific Employee Group eligible to make an application under the VSIO. The relevant union leaders will be consulted and correspondence will be sent to each eligible employee with copies to the relevant union. Only the identified employees will be eligible to make application not withstanding the following exception. Should an eligible employee have access to bumping options under a collective agreement and should such an employee exercise that option then the individual who has been bumped will become eligible to make an application under the VSIO.
Program Application. Partner’s acceptance into the Program is subject to SAGE’s acceptance of Partner’s application, which can be accepted or denied in SAGE’s sole discretion. In the event that Partner is not accepted to the Program, SAGE will notify Partner of the same, Partner shall not be liable for any fees otherwise due hereunder, and this Agreement shall terminate.
