Qualifications of Recipient. (a) Any permanent unit member who is, or whose family member is suffering from a catastrophic illness is eligible to apply for use of sick leave days in the catastrophic illness leave bank.
(b) To be eligible for use of sick leave bank days, the unit member must have exhausted all accrued paid leave credits, including all days of partial pay sick leave, vacation and other forms of paid leave.
(c) A unit member must use all paid leave credits that he or she continues to accrue on a monthly basis before receiving sick leave hours which have been donated to the catastrophic illness leave bank.
(d) The maximum number of hours to be utilized by one unit member for a single catastrophic illness shall not exceed 400 hours or 50 percent of the total available leave bank, whichever is less.
(e) Any unit member requesting use of sick leave hours in the catastrophic illness leave bank must provide the Governing Committee with written verification of the catastrophic illness. Such verification must be prepared in writing by a licensed physician of the State of California. The Governing Committee may require the unit member or family member who is incapacitated to undergo an examination by a physician selected by the District, at the District's expense, to verify the injury or illness, the degree of disability, and the anticipated length of disability.
Qualifications of Recipient. 15.12.6.1 Any permanent (full or part-time) unit member suffering from a catastrophic illness or injury or whose spouse, dependent child under the age of 18, or dependent full-time student up to the age of 25 is suffering from a catastrophic illness or injury is eligible to apply for Catastrophic Leave.
15.12.6.2 The unit member must have exhausted all accrued paid leave credit, including all full days and partial days of sick leave, vacation, and other forms of paid leave.
15.12.6.3 The unit member must use all paid leave credit that continues to accrue on a monthly basis before receiving catastrophic leave hours that have been donated to the Catastrophic Leave Bank.
15.12.6.4 The maximum number of days to be used by a unit member for a single catastrophic illness or injury shall not exceed eighty (80) days.
15.12.6.5 The maximum number of days used by a unit member for any number of granted cases of Catastrophic Leave in a single fiscal year (July 1 – June 30) shall not exceed eighty (80) days.
15.12.6.6 Any unit member requesting Catastrophic Leave must provide the District with written verification of the catastrophic illness or injury. Such verification must be prepared in writing by a licensed physician of the State of California. The District may require the incapacitated unit member or eligible family member to undergo an examination by a physician selected by the District, at the District’s expense, to verify the injury or illness, the degree of disability, and the anticipated length of disability.
15.12.6.7 Group health plan coverage and premium payment shall be maintained for eligible unit members while on Catastrophic Leave.
Qualifications of Recipient. (a) Any tenured certificated unit member whose spouse, child, or legal dependent has suffered a catastrophic illness as defined and determined by Section 8.7.3 is eligible to apply for use of sick leave days in the care giver catastrophic illness leave bank, provided that unit member has donated to the sick leave bank, in accordance with the requirements of Section 8.7.5.
(b) The maximum number of days to be utilized by one unit member for a single catastrophic illness to a spouse, child, or legal dependent shall not exceed 30 workdays.
(c) A unit member may appeal in writing to the Care Giver Catastrophic Illness Committee for an additional 30 leave bank days. A unit member shall be limited to no more than two (2) requests for use of the care giver catastrophic illness bank in a single school year. The decision of the committee concern a written request will be final. The unit member must have exhausted all sick leave to which their is eligible to use.
(d) Any unused days awarded to a qualifying employee from the care giver catastrophic illness bank shall be returned to the care giver catastrophic illness bank.
(e) Any unit member requesting use of sick leave days in the care giver catastrophic illness bank must provide the District with written verification of the catastrophic illness suffered by their spouse, child, or legal dependent. Such verification must be prepared in writing by a licensed physician of the State of California. The District may require the spouse, child, or legal dependent stricken by the catastrophic illness to undergo an examination by a physician selected by the District, at the District's expense, to verify the injury or illness, the degree of disability, and the anticipated length of disability.
(f) The "Care Giver Catastrophic Illness Committee" will examine all request and documentation for request to use the care giver catastrophic illness bank and shall then determine the unit member’s eligibility according to this section. The decision of the committee shall be final and is not subject to the grievance procedure.
Qualifications of Recipient. Any regular employee suffering from a catastrophic illness is eligible to apply for use of leave days in the catastrophic illness leave bank. Only employees who meet the qualifications listed below will be eligible to utilize the Catastrophic Illness Leave Bank.
Qualifications of Recipient. (a) Any permanent unit member suffering from a catastrophic illness or subject to Family Care Leave section 7.12 is eligible to apply for use of sick leave days in the catastrophic illness leave bank, provided that the unit member has donated to the sick leave bank, in accordance with the requirements of Section 7.11.4
(b) To be eligible for use of sick leave bank days due to personal injury or illness, the unit member must have exhausted all accrued full paid leave credits, including sick leave, vacation and other forms of paid leave.
(c) To be eligible for use of sick leave bank days for Family Care Leave, in accordance with section 7.12, the unit member must have exhausted all vacation, and 50% of their current available year sick leave entitlement. (Prior year sick leave accrual cannot be used for Family Care Leave.)
(d) A unit member must use all paid leave credits that he or she continues to accrue on a monthly basis before receiving sick leave hours which have been donated to the catastrophic illness leave bank.
(e) The maximum number of hours to be utilized by one unit member for a single personal catastrophic illness, whether one
(1) or recurring, shall not exceed 60 work days or 50% of the total available leave bank, whichever is less.
(f) The maximum number of paid hours to be utilized within a twenty four (24) month period from the catastrophic illness leave bank by one unit member for paid Family Care Leave, (see section 7.12 for additional requirements) shall not exceed 30 days or 50% of the total available leave bank whichever is less.
(g) Any unit member requesting use of sick leave hours in the catastrophic illness leave bank must provide the District with written verification of the catastrophic illness. Such verification must be prepared in writing by a licensed physician of the State of—California. The District may require the unit member who is incapacitated to undergo an examination by a physician selected by the District, at the Districts expense, to verify the injury or illness, the degree of disability and the anticipated length of disability.
(h) Provided the unit member meets all the criteria of this section, use of sick leave bank provisions shall be granted.
Qualifications of Recipient. “Catastrophic illness/injury” means an illness or injury that is expected to incapacitate the employee for an extended period of time, or that incapacitates a member of the employee’s family which requires the employee to take time off from work for an extended period of time and taking extended time off creates a financial hardship for the employee because he/she has exhausted all of his/her sick leave and other paid time off (Ed Code 44043.5 (a)).
Qualifications of Recipient. Any permanent unit member who has donated at least one day of sick leave to the Catastrophic Leave Bank during the most current drive cycle and who meets the definition of catastrophic illness or injury is eligible to apply for use of sick leave days in the Ccatastrophic illness Lleave Bbank. A unit member who was not a member of the Catastrophic Leave Bank during the previous cycle must wait 30 days after their donation to the Catastrophic Leave Bank to be eligible to access catastrophic leave. This waiting period is waived does not apply during the 2021-22 school year. Donations must be submitted using the “Catastrophic Illness Sick Leave Bank Donation Form.” This form must be submitted during an advertised Catastrophic Leave Drive.
Qualifications of Recipient. Any permanent unit member who has donated at least one day of sick leave to the Catastrophic Leave Bank during the most current drive cycle and who meets the definition of catastrophic illness or injury is eligible to apply for use of sick leave days in the Catastrophic Leave Bank. A unit member who was not a member of the Catastrophic Leave Bank during the previous cycle must wait 30 days after their donation to the Catastrophic Leave Bank to be eligible to access catastrophic leave. This waiting period is waived during the 2021-22 school year.

(a) Any permanent unit member or an immediate family member 24 suffering from a catastrophic illness is eligible to apply for use of 25 sick leave days in the catastrophic illness leave bank. 27 (b) To be eligible for use of sick leave bank days, the unit member 28 must have exhausted all accrued paid leave credits, including all 29 days of partial pay sick leave, vacation and other forms of paid 30 leave. 32 (c) A unit member must use all paid leave credits that he or she 33 continues to accrue on a monthly basis before receiving sick leave 34 hours which have been donated to the catastrophic illness leave 35 bank. 37 (d) The maximum number of hours to be utilized by one unit member 38 for a single catastrophic illness shall not exceed 400 hours or 50% 39 of the total available leave bank, whichever is less. 40 41 (f) Any unit member requesting use of sick leave hours in the catastrophic illness 42 leave bank must provide the Governing Committee with written verification of 43 the catastrophic illness. Such verification must be prepared in writing by a 44 licensed physician of the State of California. The Governing Committee may 45 require the unit member who is incapacitated to undergo an examination by a 47 the injury or illness, the degree of disability, and the anticipated length of 48 disability.
Qualifications of Recipient. (a) Any permanent unit member suffering from a catastrophic illness is 20 eligible to apply for use of sick leave days in the catastrophic illness 21 leave bank.
(b) To be eligible for use of sick leave bank days, the unit member 24 must have exhausted all accrued paid leave credits, including all 25 days of partial pay sick leave, vacation and other forms of paid 26 leave.
(c) A unit member must use all paid leave credits that he or she 29 continues to accrue on a monthly basis before receiving sick leave 30 hours which have been donated to the catastrophic illness leave 31 bank. 33 (d) The maximum number of hours to be utilized by one unit member 34 for a single catastrophic illness shall not exceed 400 hours or 50% 35 of the total available leave bank, whichever is less.
