Required District Contribution for Premiums. 1. The required annual contribution for the benefits set forth in Appendix C, sections 1., 2., and 3., above shall not exceed Four Hundred Dollars ($400.00) per month.
2. Should the plan(s) cost more than the District’s required contribution, each covered retiree shall pay the difference in advance to the District Office. Failure to prepay the difference shall result in permanent cancellation of eligibility.
Required District Contribution for Premiums. Effective July 1, 2018, the District’s Health & Welfare (H&W) cap shall be nine hundred four dollars ($904.00) per month on behalf of each full-time unit member or ten thousand eight hundred forty-eight dollars ($10,848.00) per year.

In the event the monthly cost of a bargaining unit employee's choice of benefit plans exceeds the District's monthly cap, the District is authorized to make whatever payroll deduction that may be necessary in order to maintain benefit coverage for such employee.
2. IRS Section 125 Flexible Benefits Plan Effective 2002-03, or sooner if arrangements can be completed, employee health and welfare benefits will be administered through the implementation of a fully qualified, Internal Revenue Service Section 125 Flexible Benefits Plan. Employees will have the option of purchasing health and welfare plans (e.g. medical, dental, vision) with pre-tax District fringe contributions and directing any remaining contribution into 1) other insurance plans such as life insurance, 2) 403(b) tax sheltered annuities, or 3) deferred compensation plans. The employee will have the further option of taking any or all of the District’s fringe benefit contribution as cash, on which federal and state taxes will be calculated as applicable.
Required District Contribution for Premiums. The District’s maximum benefit contribution, if actually required, shall be $13,588.56. This represents the full cost of premiums for dental, vision and Blue Cross Plan 2 as it existed on June 30, 2007. The purpose of the contribution is to allow a full-time employee to enroll in one of the medical plans and to also enroll in the dental plan and to also enroll in the vision plan.

For 2017/18 - The Required District annual contribution for a full-time employee for the benefits set forth in Sections 1., 2., and 3. above shall not exceed Eleven Thousand Dollars ($11,000) (Nine Hundred Sixteen Dollars and sixteen Cents ($916.66) per month).
b. For 2018/19 and beyond - The Required District annual contribution for a full-time employee for the benefits set forth in Sections 1., 2., and 3. above shall not exceed Twelve Thousand Dollars ($12,000) (One Thousand Dollars ($1000) per month).
c. A person employed on June 14, 2000, who was receiving benefits on that date, shall not be pro-rated as set forth in Section 4. above.
d. Should the plan(s) cost more than the District’s required contribution, each covered employee shall sign a payroll deduction form and pay the difference by payroll deduction.
Required District Contribution for Premiums. Current cap is $10,800.00 per year, which is prorated for late hires and early terminations. Should the plan(s) cost more than the District’s required contribution, each covered employee shall pay the difference by payroll deduction.

For the 2017-18 school year, the required District annual contribution for 16 the benefits set forth in 1, 2, 3 and 4 above shall not exceed Seven 17 Hundred Seventy-Nine Dollars and Thirty-Five Cents ($779.35) per month 18 per eligible employee. Nine Thousand Three Hundred and Fifty-Two 19 Dollars and Twenty Cents ($9,352.20) per fiscal year.
21 b. 1. Should the monthly cost for an enrolled employee exceed the District’s 22 required monthly contribution obligation set forth in 6.a. above, the 23 District is authorized to make whatever payroll deductions that may be 24 necessary.
Required District Contribution for Premiums a. The required annual contribution for the benefits set forth in 1., 2., and 3. above shall not exceed ($549.40614.40) per month (Seven Thousand Five Three Hundred NinetySeventy-two Dollars and eighty cents [$7,372.80] annualized).
b. Should the plan(s) cost more than the District's required contribution, each covered unit member shall, by way of their signed request for coverage form, agrees to pay the difference by payroll deduction.
1. A retired District employee who qualifies for retirement benefits at the time of retirement, may enroll in a medical, dental or vision plan available to active unit members subject to all of the following:
a. The employee is currently working and has been in working status for the district for 20 years without a break in service and the employee currently qualifies for Health benefits.
b. The employee is currently enrolled in the district benefit package or has recently enrolled for district benefits during the prior open enrollment window.
c. Arrangements are made with the Business Office to pay the total premium in advance;
d. The insurance carrier permits retired employees to continue to be enrolled.
2. Should a retired employee fail to make the required advance payment listed in c or d, he/she shall be disenrolled and may not resume participation.
