Assessment Phase. The initial evaluation may last 2-4 sessions. During this assessment phase, I will be getting to know you. I will ask questions to gain an understanding of your worldview, strengths, concerns, needs, relationship dynamics, etc. During this relationship building process, I will be gathering a lot of information to aid in the therapeutic approach best suited for your needs and goals. If it is determined that I am not the best fit for your therapeutic needs, I will provide referrals for more appropriate treatment.

During the assessment phase I am getting to know and understand you, your worldview, strengths, concerns, needs, family and relationship dynamics, etc. During this phase I am gathering a lot of information. During this phase it may not feel like we are moving forward quickly, but it is imperative for me to gather this information to assist you the best I can. During this time, we both decide if I am the best person to provide therapeutic services for your specific needs. If you or I determine that I am not the best person to address your needs and treatment plan, then referrals will be made for a more appropriate treatment provider.
Assessment Phase. After successful validation, the SM is assessed by the FAMHP and in principle the Ethics Committee that was responsible for the assessment of the initial dossier. The assessment regarding the aspects covered by Part I of the CTA is performed in parallel by the FAMHP and the EC with the exception of the modifications related to the quality part of the dossier which are only assessed by the FAMHP. The aspects covered by Part II are assessed by the EC. It is to be noted that the EC will have access to the submission dossier for substantial modification on Part I (excepted quality documentation) even if “A.3 Notification for an opinion to the Ethics Committee” was not ticket in the EU application form. The EC will decide on case by case basis if an EC evaluation (and thus an EC approval) is needed. During the assessment procedure of Part I of the dossier, if the SM dossier is not directly granted a positive assessment, the sponsor will receive a list of questions and/or requests for additional information from the national contact point. SM contents covered by the Part II of the CTA pursuant to the CTR are assessed in parallel by the Ethics Committee. Questions and/or requests for additional information regarding these aspects are sent to the sponsor by the national contact point at the same time with the list of questions related to Part I of the SM dossier. If the substantial modification is related to an update of the Inform Consent Form (ICF), the ICF is reviewed by the EC in only one language. The correct translation into all other languages remains the responsibility of the sponsor. Comments/remarks on the ICF could be provided by the EC into one of the language versions of the PDF document. In this case, the commented PDF will be added as an annex to the RFI letter and these comments/remarks have to be taken into account by the sponsor when providing the answers to the questions. However, the sponsor remains responsible for the translation of the updated ICF into the other ICF’s languages. In the case of a RFI letter, the sponsor is called upon to remedy the deficiencies noted or to supply the requested information within 12 days at the most in order to comply with the deadlines specified in the CTR. As before, the answer here should also be as a single response sent by E-mail (Eudralink/CESP if possible) to the national contact point (XXXxxxxx@xxxxx-xxxx.xx). Assessment Phase. A team of local military and civilian partners, the Assessment Team, will review the systems’ maps, identify procedures for more effective coordination between the military and civilian system’s responses, identify additional resources, technical assistance or trainings needed to support the implementation of the coordinated plan, and create an initial work plan. Assessment Phase. After receiving a report, at times but not always in the form of a CSR, the POC will meet with the reporter to gather the available facts, and will conduct a careful evaluation both of these facts, and of the credibility of the person reporting them. Some reporters choose to limi the details, or the names of potential witnesses, at the time of the CSR submission. Although this i acceptable, the CSR must sufficiently describe the allegations such that relevant citations from “Rights and Responsibilities” are identifiable. The POC will consult with the Title IX Investigator/Compliance Officer or Title IX Coordinator, who, consulting with the Title IX Coordinator, will determine if a reasonable relationship might exist between the CSR’s narrative o alleged policy violations allegations and one or more prohibited behaviors described in “Rights and Responsibilities.” A determination will be made if the alleged policy violations should be investigated further, through an Informal Investigation or through the SEP. If an SEP is warranted, a Case Manager will be assigned by the Chief Student Affairs Officer o designee. The Case Manager will review the SEP in full with the complainant, including which sexual misconduct and interpersonal violence provisions(s) may have been violated. The Case Manager will also review the decision about the complainant’s choice of Advisor. If the complainan areas, including but not limited to medical and counseling services, academic support services, living arrangements, classroom assignments, directory information accessibility, and travel considerations. Further, the complainant will be reminded that the SEP does not substitute for the filing of a complaint with law enforcement. The complainant may initiate both processes. Information about initiating a criminal complaint will be provided. Criminal investigations need not be initiated nor concluded prior to the initiation of an SEP.
