Contractor Roles and Responsibilities. The Contractor, though not a member of the PMO, shall work with the PMO to ensure comprehensive governance of the Contract. Regarding the PMO: 3.2.1 The Contractor shall have overall responsibility for managing and successfully performing and completing the Services and providing the Solution to Eligible Users as specified in this Contract, subject to the overall direction and supervision of the PMO. 3.2.2 The Contractor shall provide early notice to the PMO if the State's failure to perform its responsibilities is likely to delay the timely achievement of any tasks or delay Delivery of any Deliverable. Material delays shall be addressed according to section 24, Exhibit B, General Contract Conditions, Form PUR1000. 3.2.3 The Contractor shall have overall responsibility to coordinate the Delivery of Deliverables with the PMO.

Contractor Roles and Responsibilities. 1. Provide experienced Consultant(s) with skills and knowledge appropriate to carry out the activation services using the Division’s established schedules and tools and through the creation and execution of process improvements. 2. Support the Deputy Director, Activation Management with such advice and assistance as may be reasonably requested from the assigned. 3. Provide assistance to the Deputy Director, Activation Management, or a selected representative, in planning, monitoring, and controlling the activation activities and tasks to be carried out by the Consultant(s). 4. Monitor the performance of the Consultant(s) and adjust staffing levels appropriately to ensure completion of tasks listed in Section C, Scope of Services, within the stated Period of Performance. 5. Comply with all applicable CDCR, CCHCS and State policies and procedures including, but not limited to, the CDCR Project Management Office guidelines. 6. Provide alternative way to get in touch with Activation PM such as mobile phone during work hours when not reachable at CDCR provided workspace. 7. Submit weekly timesheets to track actual time worked. The Deputy Director or designee will endorse these timesheets at the end of every work week.

Shall be obligated to keep open and regular channels of communication in order to ensure the successful execution of the services provided within this contract. The contractor is responsible for effectively communicating any potential problems or issues to the CDCR PM, or selected representative, immediately within becoming aware of said problems. 7. Provide assistance to the CDCR PM, or a selected representative, in planning, monitoring, and controlling the activities and tasks to be carried out by the Consultant(s). 8. Monitor the performance of the Consultant(s) and adjust staffing levels appropriately to ensure completion of tasks listed in Section C, Contractor Tasks/Requirements, within the stated Period of Performance. 9. Comply with all applicable CDCR and EIS policies and procedures including, but not limited to, the EIS Project Management Office guidelines. 10. Comply with all laws, orders, rules, applicable regulations, and industry standards that are associated with the performance of their duties and obligations under this Agreement. 11. All Contractor correspondence and communication shall be directed through the CDCR PM, or selected representative, as the single point of contact. The Contractor shall not solicit any products or services to any institution staff. 12. Be responsible for its personnel, fiscal, and general administrative services to support this Agreement. 13. Once the Agreement has been awarded, the Contractor shall be provided bank account information to transmit the Lock Box funds to the CDCR Bank Account.
Contractor Roles and Responsibilities. Contractor’s Project Manager will be the primary point of contact for the State. The Contractor’s Project Manager and designated team members shall be responsible for providing progress updates against the in- progress requests during the weekly meeting as well as in the weekly progress report to HCD. Any change orders must be approved in writing by the Contract Manager. Contractor commits to a next business day turn- around time for questions and requests for information. Change requests returned for revision will be reviewed and either disregarded or resubmitted to HCD for approval within two business days of receipt. The Contractor shall facilitate Department update conference calls and on-site meetings with HCD on a periodic basis, with an agreed upon frequency. The Contractor will prepare a project work plan with a clear weekly progress report component and conduct weekly status meetings. Contractor shall prepare agendas; action item lists and disseminate the meeting minutes as appropriate. Meeting agenda should be circulated twenty-four hours prior to the meeting. This is mandatory for any meeting that includes HCD Executive Leadership. At a minimum, the following agenda shall be covered in the weekly status meetings:  Status of high-priority items (to be determined in conjunction with the HCD Contract Manager)  Discussion of open questions or issues  Discussion of potential risksDecision log, change management log, risk registry  Change Control log (tracking all change requests) Contractor’s Project Manager will document deliverables for Contractor and HCD review and approval, at a minimum, weekly and will include:  Decision Logs  Change Control logs (tracking all change requests)  Project implementation plan and schedule, the plan needs to include a high- level summary with a detailed plan, schedule and proposed changes to the timeline. The Contractor shall complete a Deliverables Expectation Document (DED for HCD review and approval. The DED form is provided as Attachment 2 to this Statement of Work (SOW). For each deliverable, the parties will agree ahead of time on deliverables specification and acceptance criteria, which the Contract shall document on a DED.
