Health Insurance Continuation. You and your beneficiaries, if applicable, will receive under separate cover notice of your rights under the Consolidated Omnibus Reconciliation Act (“COBRA”) upon your separation from the Company. If you elect to continue health care coverage under COBRA, you are responsible for paying the premiums.
Health Insurance Continuation. Employees “laid off” by the City shall continue to receive the above-described health care coverage benefits for the period of six (6) months following the day of “layoff” for involuntary termination of employment; except where the employee was “fired for cause,” in which case the coverage benefits shall be terminated and cease as soon as possible under the City’s policy.
Health Insurance Continuation. Employees who retire and do not meet the severance eligibility criteria in Section 1. and are enrolled in the district group health and hospitalization plan prior to retirement may elect to continue participation in the plan, at the employee’s expense, until the employee reaches the age of Medicare eligibility if they are receiving a disability benefit or an annuity from a MN public pension plan as outlined in Minnesota Statute § 471.61.
Health Insurance Continuation. Lake Superior State University offers the opportunity for retired staff members and their spouses to continue health, dental and vision insurance benefits as they may be made available by the University. Availability of health insurance benefits to retirees and their spouses is subject to the concurrence of the benefit carriers. An official University retiree is defined as one who has completed ten (10) years of continuous full-time or equivalent part-time service at Lake Superior State University and has met the age and service requirements for retirement as defined under the provisions of the Michigan Public School Employees Retirement System MIP program. Retirees may participate in the available benefit program only if they are not being provided with a health benefit insurance paid, in whole or in part, by another entity. Application for continued health insurance must be made at the University’s Human Resources Office no less than 30 days prior to the effective date of retirement. The retiree is fully responsible for the payment of all premiums in order to maintain coverage. The University will issue guidelines for receipt of payment. Failure to meet those guidelines will mean discontinuance of coverage. If the retiree precedes a spouse in death, the spouse may continue participation in the health insurance program the University may offer for up to the period of time COBRA law allows (currently 36 months). The University reserves the right to:
a) change at any time the benefit plan or plans offered to retirees;
b) change benefit plan carriers at its discretion, without consultation with the retirees, singularly or as a group, or with any other group acting on behalf of the retirees;
c) set, change or adjust subscription rates and payment schedules, at its discretion; or
d) withdraw or alter eligibility of retirees as participants in any University benefit plan with 180 days’ notice.
Health Insurance Continuation. Retired employees who have been employed with the County for a minimum of ten (10) consecutive years 2, disabled employees who have been employed with the County for a minimum of ten (10) consecutive years 3, or employees on an authorized leave of absence, shall be entitled to continue coverage under the group insurance plan at one-hundred and four percent (104%) of the group rate. The employee shall pay the cost of the single or family plan premium to the Employer, who shall forward the premium to the insurance carrier.
Health Insurance Continuation. At your option, you may continue to be covered under the Company’s group health insurance plan up to nine (9) months after your Separation Date, subject to the terms and conditions provided for in the Consolidated Omnibus Budget Reconciliation Act of 1985, as amended (“COBRA”) provided you have timely and properly elected COBRA coverage in accordance with the Company’s COBRA election procedures, notice of which shall be sent to you under separate cover. Subject to the Company’s receipt of this Agreement timely signed by you, the expiration of the seven (7) day revocation period set forth below, and your compliance with the terms of this Agreement, the Company will pay the entire COBRA premiums for your health insurance continuation coverage until the earlier of (i) nine (9) months after your Separation Date, (ii) the date you find alternative full-time employment, or (iii) until such earlier time as you enroll in alternate health insurance coverage, provided you have timely and properly elected COBRA coverage in accordance with the Company’s COBRA election procedures, notice of which shall be sent to you under separate cover. You agree to notify the Company within three (3) business days if and when you become eligible for alternate medical coverage or if you become employed full-time during this nine (9) month period the Company is paying your COBRA premiums.
Health Insurance Continuation. Provided Employee completes the required documentation to continue Employee’s health insurance coverage under COBRA Theravance will pay the cost of continuing Employee’s health insurance coverage under COBRA for up to 9 months at the level of coverage applicable to the employee on the Termination Date. COBRA coverage payment by Theravance will cease if Employee obtains other health insurance coverage during the period Employee is eligible for Theravance paid coverage under the terms of this Agreement. Employee agrees to notify Theravance in the event Employee obtains other health insurance coverage. Employee will be responsible for making payments to continue health insurance under COBRA after the period for which Theravance is paying the cost under the terms of this Agreement. If Employee does not accept this Agreement, he still will be eligible to continue his health insurance under COBRA, but he will be responsible for paying for the coverage himself.
Health Insurance Continuation. If you presently have insurance through the Company plan, your group health insurance will cease on the last day of the month in which your employment ended. At that time, you will be eligible to continue your group health insurance benefits at your own expense, except as described below, subject to the terms and conditions of the benefit plan, federal COBRA law, and, as applicable, state insurance laws, and you will be responsible for COBRA enrollment. You will receive additional information regarding your right to elect continued coverage under COBRA in a separate communication. The Company does not take responsibility of enrolling or making COBRA payments on behalf of the employee, the process of COBRA election and payment is the responsibility of the Employee.
Health Insurance Continuation. Executive shall have the right to receive Health Insurance Continuation. For purposes of this Agreement, the term “Health Insurance Continuation” means that the Company shall continue to provide the Health Insurance Benefits. Company’s responsibility to provide Health Insurance Continuation shall at all times be contingent upon:
