Medical Physicals. During the Term, the Bank shall continue to provide Executive with an annual medical physical with a doctor of his choice (less any such expenses payable by health insurance), a free checking account at the Bank similar to the one presently provided Executive, and a free safety deposit box at the Bank’s branch office in Bluefield, West Virginia, identical to or similar in quality and size to that presently provided Executive.
Medical Physicals. A. All Fire Rescue employees who are state certified Firefighters shall be required to complete an annual medical examination, administered by the Fire Department Physician. All Fire Rescue employees who are not certified Firefighters will complete an annual physical examination unless they advise Fire Rescue no later than January 1st of their intention not to obtain a physical for the next fiscal year. B. All medical examinations shall be administered using NFPA 1582 Standards as a guideline except that employees shall participate annually. C. Employees may choose to use their own physician in lieu of the Fire Department Physician for the Medical Examination. However, employees who choose this option shall be responsible for the following: 1. Notify the Wellness Coordinator of the name and the location of their physician; 2. Bear the cost of the medical examination; 3. Ensure that medical examinations are conducted using the same guidelines, criteria, and standards as the Fire Department Physician. 4. Forward records of the medical examination to the Fire Department Physician for review and placement in the employee’s medical file. D. All combat personnel shall be responsible for scheduling and completing the medical examination and Physical Fitness Evaluation (PFE) on their own time. Any personnel assigned to a forty (40) hour work week shall be released from duty with pay in order to complete the medical physical, physical fitness evaluation, and fit testing. All employees shall have their medical physical, Physical Fitness Evaluation (PFE) and fit testing completed no earlier than one month prior to the month preceding their birth date, and no later than the last day of the month of their birth date. Failure to complete the medical examination, Physical Fitness Evaluation (PFE), and fit testing during this time frame may result in the employee being placed on unpaid administrative leave, unless prior authorization has been granted by the Fire Rescue Administrator, until such time as the process is complete. If there is a disagreement between the Fire Department Physician and the employee's physician, a third physician agreeable to both the FD Physician and the employee shall be consulted for a final determination. The cost of a third party physician shall be split between the employee and Fire Rescue. F. All employees shall receive a medical examination and a PFE prior to returning to their assignment after any medical absences greater than six
Medical Physicals. Each employee shall have an annual physical exam. This exam shall be administered by the towns contracted physician that is normally used for initial employment hire. The town agrees to pay all co- pays and any additional fees incurred by the employee in connection with the annual physical and any associated tests as a result of this annual physical exam. The medical exam shall be completed within a time period of sixty (60) days prior to or sixty (60) days following the employee’s birth month. For example, if an employee’s birthday falls on March 1, they would need to schedule an exam between January 1 and May 31 of that year. The medical exam shall be as if the employee was reporting to work and shall be paid accordingly. Verification of completion of the exam will be submitted on a form provided by the Chief. This shall constitute the only medical record kept on file pertaining to the annual exam. The Town shall obtain written consent from the employee to obtain any medical records associated with the annual physical. All medical records shall be confidential and kept in compliance with 29 CFR 1910. If at any time it is determined that the annual medical exam must comply with NFPA 1582 or any part thereof, the Town and the Union agree to negotiate this issue at this time.
Medical Physicals. MFPD will provide annual non-punitive medical physicals for each member during the month of the member’s birthday. The confidential physicals will be conducted by ERHC (Emer- gency Responders Health Care, Boise, ID) according to the essential job tasks as outlined in NFPA (National Fire Protection Association) 1582. The Fire Chief Shall receive a letter from ERHC indicating member participation, including NFPA 1582 approval or non-approval. It is mandatory that a member reports any serious health condition/s to MFPD, that could interfere with their duties.

  • Medical There shall be an open enrollment period for medical coverage in each year of this Agreement. An employee may elect no medical coverage during any open enrollment period. An employee who has elected no medical coverage may elect medical coverage during an open enrollment period. No pre-existing condition limitations will apply.

  • Medical Examination Where the Employer requires an employee to submit to a medical examination or medical interview, it shall be at the Employer's expense and on the Employer's time.

  • Medical Examinations An employee may be required by the Employer, at the request of and at the expense of the Employer, to take a medical examination by a physician of the employee's choice. Employees may be required to take skin tests, x-ray examination, vaccination, inoculation and other immunization (with the exception of a rubella vaccination when the employee is of the opinion that a pregnancy is possible), unless the employee's physician has advised in writing that such a procedure may have an adverse affect on the employee's health.

  • Wellness i. To support the statewide goal for a healthy and productive workforce, employees are encouraged to participate in a Well-Being Assessment survey. Employees will be granted work time and may use a state computer to complete the survey. ii. The Coalition of Unions agrees to partner with the Employer to educate their members on the wellness program and encourage participation. Eligible, enrolled subscribers who register for the Smart Health Program and complete the Well-Being Assessment will be eligible to receive a twenty-five dollar ($25) gift certificate. In addition, eligible, enrolled subscribers shall have the option to earn an annual one hundred twenty-five dollars ($125.00) or more wellness incentive in the form of reduction in deductible or deposit into the Health Savings Account upon successful completion of required Smart Health Program activities. During the term of this Agreement, the Steering Committee created by Executive Order 13-06 shall make recommendations to the PEBB regarding changes to the wellness incentive or the elements of the Smart Health Program.

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias.
