Medical Services Plan and Extended Health Benefits. (i) The District shall contribute eighty percent (80%) of the monthly cost of the Medical Services Plan of British Columbia and of the Extended Health Benefits Plan under the trusteeship of the Capital Area Benefit Advisory Group for regular employees, provided that the employee agrees to contribute the remaining twenty percent (20%) of both plans by payroll deduction.
(ii) The Extended Health Benefit coverage shall include:
(a) vision care providing for full reimbursement towards the cost of the purchase of one (1) pair of eyeglasses or laser eye surgery every two (2) years for each regular employee and his/her dependents to a maximum cost of four hundred dollars ($400.00) per pair or surgery;
(b) charges for routine eye examinations every two (2) calendar years when performed by a Physician or legally authorised optical provider shall be increased to one hundred dollars ($100.00).
(c) hearing aids to a maximum of three thousand dollars ($3,000.00) every five (5) years.;
(d) hair pieces and wigs for chemotherapy and alopecia patients payable to a maximum of five hundred dollars ($500.00) per twenty- four (24) months.
(e) an unlimited lifetime maximum;
(f) Bluenet; and
Medical Services Plan and Extended Health Benefits. Regular employees who are eligible for coverage under the terms of the Medical Services Plan of British Columbia and the Extended Health Benefits Plan in effect through the Capital Area Benefit Trust, may participate in both plans from the first day of the month following completion of ninety (90) days employment. Eligible employees shall contribute as follows:
(i) Medical Services Plan: Eligible employees shall contribute twenty-five percent (25%) of the premium for the plan and the employer shall contribute the remaining seventy-five percent (75%). Effective January 1, 2001, the employer shall contribute one hundred percent (100%) of the premium;
(ii) Extended Health Benefits: Eligible employees shall contribute twenty-five percent (25%) of the premium for the plan and the employer shall contribute the remaining seventy-five percent (75%). Effective January 1, 2001, the employer shall contribute one hundred percent (100%) of the premium. The plan shall include vision care providing for eighty percent (80%) reimbursement towards the cost of the purchase of one (1) pair of eyeglasses every two (2) years for each regular employee and their dependents to a maximum cost of two hundred dollars ($200) per pair (including hearing aids to a maximum of five hundred dollars ($500.00) every five years).
Medical Services Plan and Extended Health Benefits.
(i) The Employer shall contribute eighty percent (80%) of the monthly cost of a regular employee's participation in the B.C. Medical Services Plan and the Extended Health Benefits Plan under the trusteeship of the Capital Area Benefit Trust, provided that the employee agrees to contribute the remaining twenty percent (20%) on a check-off basis.
(ii) The Extended Health Benefit coverage shall include:
(a) Vision care providing for full reimbursement towards the cost of the purchase of one (1) pair of eyeglasses or laser eye surgery every two

Medical Services Plan and the Extended Health Benefits Plan under the trusteeship of the Capital Area Benefit Trust, following completion of their probation period, provided that the employee agrees to contribute the remaining twenty percent (20%) on a check-off basis.
(ii) The Extended Health Benefit coverage shall include vision care providing for full reimbursement towards the cost of the purchase of one (1) pair of eyeglasses every two (2) years for each regular employee and his dependents to a maximum cost of four hundred dollars ($400.00) per pair; hearing aids to a maximum of two thousand dollars ($2000.00) every five (5) years; an unlimited lifetime maximum; bluenet and no deductible. Effective April 1, 2006 Extended Health Benefit coverage shall also include eye examinations for each regular employee and dependents to a maximum of seventy-five dollars ($75.00) every two (2) years. The parties agree that the Employer shall utilize the employee portion of the UIC rebate to improve the Extended Health Benefit coverage.
(iii) A newly hired regular employee may enrol in the Medical Services Plan on the first day of the month following their commencement of employment by paying one hundred percent (100%) of the cost of the premium.

(b) hearing aids to a maximum of two thousand dollars ($2000.00) every five (5) years. Effective January 1, 2012 the maximum shall be increased to three thousand dollars ($3,000.00) every five (5) years;
(c) an unlimited lifetime maximum;
Medical Services Plan and Extended Health Benefits. (i) After the completion of six (6) months' service, all employees covered by this Agreement shall be eligible for the Medical Services Plan and the Extended Health Benefits Plan. The Extended Health Benefits Plan includes coverage for the services of a Clinical Psychologist to a maximum of seven hundred dollars ($700.00) per person annually. The Corporation will pay 100% of the monthly premium of the two plans for all eligible members of the Fire Department.
(ii) The Vision Care Option portion of the Extended Health Benefits Plan shall provide every employee with a maximum benefit of four hundred dollars ($400.00) claimable in any twenty-four month period, which may also be used for laser eye surgery, subject otherwise to the provisions of the Plan. Premiums for the Vision Care Option shall be borne one hundred percent (100%) by the Corporation.
