Notification and Verification. Faculty members who are absent due to illness or injury may be required to keep their Xxxx/Director or Human Resources informed as to their status. Procedures for reporting shall be as specified by the Xxxx/Director or Human Resources. Verification of the necessity for absence from work and use of sick leave from an attending physician or health care practitioner may be required.
Notification and Verification. Employees who intend to be absent from work due to illness or injury must notify their supervisor prior to the start of the workday. If the absence continues beyond one day, the employee is expected to keep the supervisor informed of his/her condition and anticipated return date. At the employer’s discretion, the employee may be required to provide medical verification. The City reserves the right to have an employee examined by the City’s medical professional, at City cost, to determine fitness for duty. The City’s right to have employees examined by the City’s medical professional does not in any way limit the employee’s rights to be treated by a medical professional of their choice. Nothing in this article will limit the City’s right to deem leave taken as leave provided for under the State or Federal Family Medical Leave Acts.
Notification and Verification. Lender may, at its sole discretion from time to time, (i) verify invoices and (ii) notify Account Debtors of Lender’s security interest in the Receivables.

Employees are encouraged to provide the Hospital with as much advance notice as possible in the case of illness or injury when they are unable to report for duty as scheduled. Employees shall notify the Hospital at least two (2) hours in advance of the employee’s scheduled shift if the employee is unable to report for duty as scheduled. 1 2. When a unit member becomes aware that an absence will extend beyond ten (10) consecutive 2 workdays, the unit member shall notify his/her administrator or supervisor.

On the day preceding the unit member's return from an illness or 4 injury absence, the unit member shall notify his/her immediate supervisor or the Human Resources 5 Office by the end of his/her substitute's regular workday. However, substitutes shall be released 6 routinely on Friday unless the Human Resources Office or the immediate supervisor has been 8 and loss of an additional day of sick leave.

At the conclusion of an extended absence exceeding ten (10) consecutive workdays and before 10 resuming work, the unit member must submit to the Human Resources Office a written statement 11 from a medical doctor, dentist, State Licensed Marriage, Family, and Child Counselor, chiropractor, 12 recognized religious practitioner, or optometrist which indicates that the unit member is able to 13 assume the full responsibilities and duties of his/her assigned position. Although it is the unit 14 member's responsibility to provide a written statement prior to return from an extended absence, the 15 Human Resources Office or appropriate supervisor shall attempt to notify the unit member of the 16 requirement on or before the tenth day of absence.
Notification and Verification. 11.12.3.1 Unit members will give their immediate supervisors the earliest notification possible of intent to use Personal Necessity Leave using the current electronic system.
11.12.3.2 Unit members will receive electronic verification by the current electronic system within three (3) work days of notification of intent to use personal necessity requests in accordance with 11.12.2.1.2 through 11.12.2.1.4 prior to taking this leave.
11.12.3.3 In the event of emergency, unit members will make the notification by those reasonable means at their disposal.
11.12.3.4 Unit members will complete a form stating the reason(s) requesting Personal Necessity Leave, and will sign a statement certifying that the reason(s) is/are the actual reason(s) for being absent.
11.12.3.5 Under extenuating circumstances, the unit member’s immediate supervisor or the Superintendent, may approve a verbal request for Personal Necessity Leave without requiring the unit member to state a reason(s) in writing for the leave. The verbal request may be made through the DNTA President.
11.12.3.6 In the event a unit member cannot get from their home to their work site due to impossible travel conditions the unit member will contact their supervisor or the District to arrange for an alternate work site. If no alternative is possible such absences will be deducted from their Personal Necessity Leave.
11.12.3.7 Unit members unable to return from a trip due to impossible travel conditions, unless the trip was assigned as school business, will have the absence deducted from their Personal Necessity Leave.

Investment Manager may at any time, upon the occurrence and during the continuance of any Event of Default, without notifying Borrowers of its intention to do so, notify Account Debtors of Borrowers, parties to the Contractual Obligations of Borrowers, obligors in respect of Instruments of Borrowers and obligors in respect of Chattel Paper of Borrowers, that the Accounts and the right, title and interest of Borrowers in and under such Contractual Obligations, Instruments and Chattel Paper have been assigned to Investment Manager and that payments shall, subject to the terms of the Subordination Agreement, be made directly to Investment Manager. Upon the occurrence and during the continuance of an Event of Default and at the request by Investment Manager, Borrowers shall so notify such Account Debtors, parties to such Contractual Obligations, obligors in respect of such Instruments and obligors in respect of such Chattel Paper. Upon the occurrence and during the continuance of any Event of Default, Investment Manager may, in its name or in the name of others, communicate with such Account Debtors, parties to such Contractual Obligations, obligors in respect of such Instruments and obligors in respect of such Chattel Paper to verify with such parties, to Investment Manager’s satisfaction, the existence, amount and terms of any such Accounts, Contractual Obligations, Instruments or Chattel Paper.
