Policy Procedures. The Employer reserves the right to establish reasonable departmental rules, regulations, policies and procedures not inconsistent with the provision of this Agreement. Such rules, regulations, policies and procedures shall be available for inspection and review by employees, if such rules, regulations, policies and procedures concern working conditions. If the GELC believes that such rules, regulations, policies and procedures are unreasonable, a grievance may be filed within ten (10) days after the establishment of such rules, regulations, policies and procedures and thereafter considered in accordance with the Grievance Procedure. This will acknowledge that the parties have agreed to the Drug and Alcohol Policy in the Sheriff’s Office as corrected.
Policy Procedures. 1. In consideration of the benefits of participating in the S.L.F., each applicant for membership in the Bank and for benefits from the Bank shall, as a condition to such application, agree, in writing, as follows: “I specifically acknowledge and agree that the granting of days from the S.L.F. shall be at the sole discretion of the S.L.F. Board. All decisions of the S.L.F. Board will be final and binding but not subject to grievance. I further agree to abide by such decision and to indemnify and hold harmless the Greeneview Local School District, the Greeneview Classified Employees Association, the S.L.F. Board, and all of their agents for any loss they may sustain as a result of any claim or legal proceedings I may bring against any of them with respect to a decision made by any of them concerning this application.”
2. Application for the S.L.F. days must be made to the Superintendent.
3. The S.L.F. Board shall meet and render a decision within ten (10) days of receipt of request.
4. Unused requested days shall be returned to the S.L.F.
5. The S.L.F. will begin with two (2) days from each contributing employee. When the fund is depleted below fifty (50) hours, each member will be assessed one (1) additional day. The Treasurer shall be responsible for notifying employees of each assessment period.
6. Extension of additional days may be applied for in the same manner as original application.
7. When an employee donates hours/days to the Fund, he/she agrees to the above-stated rules for administration of the Bank and agrees to abide by the stated rules.
8. All decisions of the S.L.F. Board shall be final and binding, but are not subject to the grievance/arbitration provisions of this AGREEMENT.
9. These guidelines will be reviewed annually by the Sick Leave Fund Board if requested by either party.

4. Application to the SLB can only be made one (1) time every five (5) years unless the application is being made for a recurring condition.
5. Unused requested days shall be returned to the SLB.
6. The member borrowing days from the SLB shall pay the days back at a rate of five (5) days per year. If the member resigns or retires, any remaining days due to be paid back will be deducted from any accrued sick leave at the time of the retirement/resignation. If there are no days remaining at the time of the retirement/resignation, the payback shall be waived.
7. Prior to April 1, the SLB will notify the District Treasurer of the persons who have been approved for usage of the SLB and the number of days those persons were granted. Using this information, the District will determine the number of days those persons have used and paid back. This will be reported to the SLB no later than June 1, who will notify the District no later than July 1 of any changes.
8. The SLB will begin with one (1) day from each contributing member. When the fund is depleted below one hundred (100) days, each member will be assessed one (1) additional day. The Association shall be responsible for notifying members of each assessment period.
9. When a member donates days to the Bank, he/she agrees to the above-stated rules for administration of the Bank and agrees to abide by the stated rules.

I further agree to abide by such decision and to indemnify and hold harmless Hillsboro City Schools, the S.L.P. Committee, and all of their agents for any loss they may sustain as a result of any claim or legal proceedings I may bring against any of them with respect to a decision made by and of them concerning this application.”
Policy Procedures. A. E.L.F. leave will not be granted until:
1. All paperwork is received. Proper paperwork needs to be turned in prior to any surgery, procedure, or therapy. In the event of an unforeseen situation, information may be submitted at the earliest possible time and days may be awarded retroactively. This will be determined at the discretion of the E.L.F. Board.
2. The E.L.F. Board has examined all documentation from the employee’s healthcare provider and determined that the employee’s condition merits E.L.F. leave.
3. The E.L.F Board has met and rendered a final decision. The E.L.F. Board shall meet and render a decision within 10 days of receipt of request.
B. All decisions of the E.L.F. Board are final, binding, and not subject to grievance.
C. Application for emergency sick leave can only be made under the following conditions:
1. All accumulated sick and personal leave must be used first.
2. Extended leave may be granted for catastrophic personal accidental injury, non- elective surgery, or serious illness.
3. Verification by a physician shall be mandatory prior to any leave being granted.
4. The application for E.L.F. days is submitted to the Director of Human Resources.
D. Additional days may be granted above the maximum by the E.L.F. Board. A letter must be submitted to the Human Resources Office requesting additional leave accompanied with a physician verification justifying the need for additional leave. The E.L.F. Board will meet and determine if the request is granted. The decision of the E.L.F. Board is final.
E. In consideration of the benefits of participating in the E.L.F. each applicant for membership in the Fund shall, as a condition to such application, agree in writing as follows: “I specifically acknowledge and agree that granting of days from the E.L.F. shall be at the sole discretion of the E.L.F. Board. All decisions of the E.L.F. Board will be final, binding and not subject to grievance. I further agree to abide by such decision and indemnify and hold harmless the Middletown City School District, Middletown Teachers Association, Middletown Classified Employees Association, the E.L.F. Board, and all of their agents for any loss they may sustain as a result of any claim or legal proceedings I may bring against any of them with respect to a decision made by any of them concerning this application.”

I further agree to abide by such decision and to indemnify and hold harmless the Mad River Local School District, the Ohio Association of Public School Employees Local #342, the Sick Leave Bank Board, and all other agents for any loss they may sustain as a result of any claim or legal proceeding I may bring against any of them with respect to a decision made by any of them concerning this application.”
