Reports and Evaluation. ▪ In the three months following the signing of this Contract, the Committee will develop a plan to evaluate the Contract’s components. ▪ The evaluation plan will outline the main questions involved in the evaluation, assign responsibility for the evaluation activities, define proposed indicators and methodologies, and determine cost sharing and schedules. ▪ In the three months after the end of each fiscal year, the Steering Committee will submit a progress report to the Ministerial Committee for the Montréal region and to Montréal’s City Council, consisting of: a) The Annual Report, which shall include the following: - A statement of the annual objectives for each component of the City Contract; - Quantitative and qualitative results, where possible; - An analysis of the results in terms of the logic and transversal cohesion of the actions. - Financial statement; - Adjustments to be made as applicable. b) The operating report of indicators for strategic or operational results. ▪ For the fifth fiscal year, a final evaluation report must be submitted to the Ministerial Committee for the Montréal region and Montréal’s City Council.

Reports and Evaluation. 1. Develop and submit an annual Success Story with two (2) photographs and two (2) photograph release forms to DSHS. A Success Story Draft must be reviewed and approved by DSHS prior to the final version submission date. 2. Develop and submit a Final Evaluation Report for each fiscal year, following the template provided by DSHS. A Draft Evaluation Report must be reviewed and approved by DSHS prior to the final version submission date. 3. Participate in monthly conference calls or face-to-face meetings (e.g., monthly project status reports) with DSHS to be conducted on or before the 15th of each month of the Contract term, unless otherwise agreed to in writing by DSHS. During the meetings, Grantee will discuss the following: 1) implementation status; 2) barriers and methods to address those barriers; 3) opportunities to enhance the activities; 4) lessons learned; and 5) next steps. Other meetings or calls may be added, as appropriate, with Grantee and DSHS. 4. Participate in conference calls with Quitline provider as needed to provide updates and relay technical concerns as appropriate. 5. Submit quarterly Progress Reports to DSHS via PMATS. Progress report due dates are as follows: 1. Clinic site list for all Healthcare System Integration records considered undergoing negotiation, in process, and integrated. Clinic site list should include name and address for clinics with access to the healthcare system’s eTP and/or Ask Advise Refer options only; and 2. Roster for all training records. Xxxxxx should clearly identify participant name, provider type, and certification number (if applicable). Where Grantee is not the host, the Grantee should request the roster during the planning with host. If no roster is obtained, a justification must be entered in the training record.

At least 10 vessel captains or owners have signed agreements to fish with circle hooks only and said vessels are verified to be doing so. 2. At least 10 fishing trips with approved observers on board have been completed. At least 60 days prior to the end of the Effective Period of the MOU the First Party shall submit a report and documentation for Initiative Two that gives evidence the following minimum requirements for the MOU have been fulfilled: 1. At least 10 vessel captains or owners have signed agreements to keep voluntary fishery data collection records on each fishing trip. 2. At least 5 fishing trips with approved observers on board have been completed. At least 60 days prior to the end of the Effective Period of the MOU all parties shall evaluate, through summary documentation and direct meetings, options for moving forward with formal Fisheries Improvement Project (FIP) process, including a MSC pre-assessment or rapid assessment utilizing MSC criteria.

Reports and Evaluation. The Grantee is required to provide the Foundation with online financial and narrative reports summarizing project results. The Grantee will receive an email request for any report approximately one month before it is due. The Grantee also agrees to provide any other information reasonably requested by the Foundation. If the Grantee obtains any audited financial statements covering any part of the period of this Agreement, copies of such statements shall be provided to the Foundation. The Foundation may, at its own expense, monitor and conduct an evaluation of the operations under the Grant which may include visits by representatives of the Foundation to observe the Grantee’s program procedures and operations and to discuss the program with the Grantee’s personnel.
