Scheduled Use. The nurse must have sufficient accrued PTO to request time 12 off. Scheduled PTO may be rescinded at any point if sufficient PTO cannot 13 be accrued to cover the requested absence. A nurse may request PTO at least six (6) weeks but not more than 16 twelve (12) months prior to the first day of the PTO requested. 18 Requests for PTO shall be considered on a first-come-first-served basis. If 19 two (2) or more requests are submitted on the same date for the same time 20 off and granting both of them would result in a critical staffing shortage, the 21 senior nurse will be given the time off. The staff nurse shall be notified of 22 approval within two (2) weeks of the submission of a request. 24 PTO which occurs during the pay periods(s) containing Spring Break, 25 Thanksgiving, Christmas, and New Year’s will be arranged according to 26 departmental staffing practice. Requests for these time periods will be 27 granted by the manager on a rotational basis. Nurses will be notified no later 28 than ten (10) weeks prior to the holiday. 30 PTO request from the beginning of Memorial Day weekend through the end 31 of Labor Day weekend will be limited to FTE hours times two (FTE x2), so all 32 nurses may enjoy a part of the summer months off. Requests for additional 33 hours may be granted by the manager on a rotational basis. 1 PTO requests above established unit quotas, or in the immediate six (6) 2 weeks following the date the request off is submitted, may be accomplished 3 by “shift swaps” that do not result in overtime. 5 Nurses and department managers will work together in the scheduling of 6 time off to ensure adequate staffing to meet the needs of patient care. The 7 nurse will be required to arrange coverage for those scheduled shifts for 8 which the nurse has requested PTO in the immediate six (6) weeks following 9 the date the request off is submitted. Assistance in arranging such coverage 10 may be requested from the manager or staffing office. 11 A nurse may ask to rescind scheduled PTO prior to the date when the 12 schedule covering such time off is posted. After schedule is posted, such a 13 request for rescission may be granted if the department manager consents.
