School Age Children. My Child attends the following school Name of School and Address School Ph. # His / her immunization record is on file at the school and all required immunizations and/or tuberculosis test are current. Vision and Hearing screening records are also on file. My child has permission to: ride a bus and/or walk to and from school, be released to the care of his/her sibling(s) under 18 years old. Name of Sibling(s): I have provided the childcare operation with a copy of my child’s most current immunization record. ADMISSION REQUIREMENT: If your child does not attend pre-kindergarten or school away from the child-care operation, one of the following must be represented when your child is admitted to the child-care operation or within one week of admission. Please check only one option: 1. HEALTH-CARE PROFESSIONAL’S STATEMENT: I have examined the above named child within the past year and find that he / she is able to take part in the day care program. Health Care Professional’s Signature Date 2. A signed and dated copy of a health care professional’s statement is attached.
School Age Children. An employee shall be granted leave without pay for the care and nurturing of the employee's pre- school age children in accordance with the following conditions: an employee shall notify the Council in writing four (4) weeks in advance of the commencement date of such leave; leave granted under this sub-clause shall be for a minimum period of six (6) months: the total leave granted under this sub- clause shall not exceed (5) years during an employee's total period of employment in the Public Service; such leave shall be deducted for the calculation of "continuous employment" for the purposes of calculating severance pay and vacation leave; time spent on such leave shall not be counted for pay increment purposes. Leave Without Pay for Family-Related Needs Leave without pay will be granted for family related needs, in the following manner: Subject to operational requirements, leave without pay for a period of up to three (3) months will be granted to an employee for family-related needs. School Age Children. School-aged children (kindergarten and older) may be allowed to participate in activities outside the direct supervision of the provider. Such activities occur on the premises of the day care home. The provider will visually check such children every 15 minutes*. * If the property contains a water hazard, which includes, but is not limited to pools, ponds, creeks, etc., fire/safety representatives from OCFS will determine if the children cannot be outside without the presence of the provider. Parents must acknowledge existence of this hazard in writing.
School Age Children. Unique Individuals has strict policies on after school pick up. The safety of my child and the other children being picked up is top priority. Because of this, I agree to notifyUnique Individuals by 2:00pm each day if my child will not be picked up at their elementary school. If I fail to notifyUnique Individuals, I agree to pay a $15 no call penalty. On days that the children are not in school you must notify us by 9:00am or a $25 no call penalty will apply. School Age Children. An employee shall be granted leave without pay for the personal care and nurturing of the employee's age children in accordance with the following conditions: an employee shall notify the Employer in writing as far in advance as possible but not less than four (4) weeks in advance of the commencement date of such leave, unless because of an urgent or unforeseeable circumstance such notice cannot be given; leave shall be granted under this clause once per child for a minimum period of six (6) weeks and for a maximum period of two (2) years, with a lifetime maximum of five (5) years. where the employee returns from a leave of up to one (1) year, the Employer will return the employee to the same position at the same classification and level which the employee held prior to the leave provided the position exists. In any event, the employee shall be reinstated to a comparable position with the same wages and benefits; where the employee returns from a leave of more than one (1) year and the employee's previous position has not been filled on a permanent basis or eliminated, the Employer will return the employee to the position. Where the position has been filled permanently or no longer exists, the Employer will reassign the employee to a vacant position, when available, for which the employee is qualified. An employee who accepts a lower position shall be provided the opportunity to return to their previous position when a permanent vacancy occurs. An employee who declines a reassignment shall be deemed to have abandoned his or her position. Where no reassignment is made after two (2) years from the expiry date of the employee's leave, the employment of the employee shall be terminated and the employee shall be paid severance pay.
School Age Children. School aged children can endure slightly longer periods of separation than infants and toddlers because their need is for the caregiver to be available rather than in close physical proximity. Face to face interactions can be supplemented by appropriate telephone and email contact.

My child will / will not attend CKLA/LSLA on non-school days. Returned Check: A $30.00 fee will be charged for each returned check.   • Children/Grandchildren An employee may purchase life insurance in the amount of ten thousand dollars ($10,000) as a package for all eligible children/grandchildren (as defined in Section 2A2 and 2A3 of this Article). If you have an unmarried child of any age who is financially dependent upon you and medically determined to have a physical or mental impairment, which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than twelve (12) months, that child is an eligible disabled dependent under this agreement. Please contact our Customer Service Department, to obtain the necessary form to verify the child’s disabled status. Periodically you may be asked to submit additional documents to confirm the child’s disabled status.

  • Spouse The spouse of an eligible employee (if legally married under Minnesota law). For the purposes of health insurance coverage, if that spouse works full-time for an organization employing more than one hundred (100) people and elects to receive either credits or cash (1) in place of health insurance or health coverage or (2) in addition to a health plan with a seven hundred and fifty dollar ($750) or greater deductible through his/her employing organization, he/she is not eligible to be a covered dependent for the purposes of this Article. If both spouses work for the State or another organization participating in the State's Group Insurance Program, neither spouse may be covered as a dependent by the other, unless one spouse is not eligible for a full Employer Contribution as defined in Section 3A. Effective January 1, 2015 if both spouses work for the State or another organization participating in the State’s Group Insurance Program, a spouse may be covered as a dependent by the other.
