Semi-Annual Progress Reports. The semi-annual progress reports are due on every 6 month interval determined from the date the application is created by Public Service. Progress reports are no longer required once the garden has obtained commercial operation.

Semi-Annual Progress Reports. Between the Effective Date and the Expiration Date, the Grantee shall provide the Grantor with a progress report two times per calendar year (submitted within 30 days of June 30 and December 31). The Grantee will use the forms provided by the Grantor, which will include information regarding the expenditure of the Grant moneys, compliance with the terms of this Agreement, progress toward completion of the Project as documented in the Exhibits, a narrative on the activities completed as part of the Project, match that has been contributed towards the Project to-date, and public benefits achieved.
Semi-Annual Progress Reports. Semi-Annual Progress Reports shall summarize the results of the contract work for the six month period covered. The report is due by the 15th of the month following the end of each semiannual performance period. The Contractor shall submit two (2) paper copies AND two copies on digital magnetic media such as computer files on 3.5 inch, high density computer diskettes or other digital medium approved by the Project Officer, in formats readable using IBM-type personal computer; for example, Microsoft Word™ 2000 version 9.0 for Windows and Microsoft Excel™ 2000 version 9.0 for Windows. The Project Officer may approve alternate forms of transmittal of reports (such as via email as attached files). The report shall include the following: (A) Face page to include contract number, contract title, performance period covered, Contractor’s name and address, telephone, and telefax numbers, e-mail address, and the submission date. (B) An executive summary, to include: (1) An overview of the status of the project, including personnel, adherence to timelines and milestones, and research and development activity; (2) A brief overview of the work that was completed for the reporting period and/or justification for failure to complete intended work or performance of work beyond that initially planned; (3) A brief overview of the activities that occurred during the current reporting period and any problems (technical or financial) that occurred during the current reporting period; and (4) The fulfillment of goals and of the specific aims set forth in the proposal. (C) A full description of: (1) The work performed during the reporting period; (2) The relation between the accomplishments and the goals and objectives of the contract; and (3) A full discussion of the results and their relevance; explanations of any differences between planned and actual progress, and, if necessary, what corrective steps are planned or have been implemented. Portions of this Exhibit were omitted and have been filed separately with the Secretary of the Commission pursuant to the Company’s application requesting confidential treatment under Rule 406 of the Securities Act. (D) Copies of manuscripts (published or unpublished) derived from research performed under the contract and copies of all abstracts, manuscripts, preprints and publications that resulted from work conducted or any protocol or method developed specifically under this contract during the performance period. Semi-Annual Progress Reports. The Borrower shall prepare, under terms of reference satisfactory to the Association, semi-annual progress reports, on or about January 31 and July 31 of each year of Project implementation, and furnish to the Association copies of such reports for its review and comments. The Borrower shall undertake any corrective actions deemed by the Association to be necessary to remedy any shortcomings identified in the implementation of the Project.

In addition, the report can provide any further qualitative results information the recipient would like to include to demonstrate the results achieved vis-à-vis the project objectives during that particular reporting period.
Semi-Annual Progress Reports. Semi-annual progress reports documenting performance will be due for periods October 1st through March 31st, and April 1st through September 30th, no later than the last day of the month following the end of the progress periods. Performance Reports must adequately address progress in achieving agreed-upon project/program outputs and environmental outcomes. The recipient will work with EPA, Region 6, to define the scope of work performed. Progress reports can be submitted under GRTS.

Semi-Annual Progress Reports. Progress reports must be submitted semi-annually for each budget period during the cooperative agreement. The Provider must submit this information electronically. At a minimum, progress reports must provide the information below: (1) A comparison of accomplishments with the goals and objectives for the reporting period; (2) An annotated version of the approved budget that compares actual costs with budgeted costs by line item, and explains differences. The explanation should include, as appropriate, an analysis of cost overruns and high-cost units and a description of service requests not anticipated in the Provider’s original budget; (3) A description of the services provided to include: (i) Number of requests received by topic area and program type (AmeriCorps, Learn & Serve, Senior Corps); (ii) Activity conducted to address each request (e.g., training, on-site technical assistance, phone consultation and other electronic communication, and materials development and shipment) and mode of delivery (e.g., staff member, consultant, peer and/or other Provider); (iii) Number of participants at each training and technical assistance event; (iv) Client feedback on the services rendered (including the aggregate evaluation of each training event); and (v) Problems encountered in delivering services with recommendations for correcting them. (vi) List of upcoming activities and events with dates and locations; (vii) Recommended training and technical assistance focus areas as suggested by analyses of service activities and trends; (viii) Discussion of developments that hindered, or may hinder, compliance with the cooperative agreement; (ix) List of materials submitted to the National Service Resource Center and National Service-Learning Clearinghouse; (x) List of practices and supporting documentation or materials submitted to EpiCenter.
Semi-Annual Progress Reports. The Municipality shall submit semi- annual written reports to the Department of its efforts to comply with the obligations set forth in Paragraphs 3 through 15 and 17 and 18 above until those obligations are completed. Said report shall be sent to the address in Paragraph 27 and submitted to the Department no later than the 31st day of January and July of each year. The Credit for Past Work Form is attached hereto as Appendix F and must be signed, with the following certification: I certify under the penalty of law that I believe the information provided in this document is true, accurate, and complete. I certify under penalty of law that I am familiar with the information submitted in this document and all attached documents and, based on my inquiry of those individuals immediately responsible for obtaining the information, believe that the submitted information is true, accurate, and complete. I am aware that there are significant civil and criminal penalties, including the possibility of fine or imprisonment or both, for submitting false, inaccurate or incomplete information. Within sixteen (16) months of the submittal, the Department will issue a decision accepting or rejecting the Municipality’s claim for past work. In the event that a dispute should arise as to the Department’s decision in this matter, that shall be subject to the Dispute Resolution provision of this Consent Order and Agreement.
