Use of Bank Leave Days. Use of sick leave bank days will be on a daily basis; e.g., each eligible employee will draw each day until the total leave bank days have been exhausted. An employee is one who has timely volunteered for participation in the sick leave bank and who has exhausted all applicable leaves without being eligible for long term disability, worker’s compensation, and/or Social Security disability.
Use of Bank Leave Days. Use of Sick Leave Bank days will be on a daily use basis; e.g., each eligible employee will draw each day until the total Bank Leave days have been exhausted. An eligible employee is one who has timely volunteered for participation in the Sick Leave Bank and is absent at least 10 days as a result of the same serious health condition and has exhausted his/her personal Sick Leave without being eligible for long term disability, workers’ compensation, and/or social security disability. Use of the Sick Leave Bank shall be limited to those absences where the treating physician verifies the serious health condition. The Sick Leave Bank may not be used for standard pregnancy related absences, but may be used for complications during an employee’s pregnancy and/or delivery. An employee who qualifies for Sick Leave Bank and returns to work may be eligible for additional Bank days later in the year. Additional Bank days may be authorized by the district for an absence related to the original qualifying serious health condition. A statement from the treating physician will be required for additional Bank days. Bank grants will not automatically be carried over from one contract year to another. All Bank grants will end June 30 or the last regular duty day of the contract year, whichever is sooner. Additional Rules as determined pursuant to Article VI, Section G: 1. The Sick Leave Bank may only be used for the contributor’s own serious health condition; it may not be used for illness of other members of the contributor’s family. 2. Only earned or available sick leave may be contributed to the Sick Leave Bank. 3. No employee shall be required, for purposes of maintaining membership status in the Sick Leave Bank, to contribute more sick leave days than other members. 4. No payment for a sick leave bank grant will extend beyond the first of the month following the month in which the Medical Board of the Iowa Public Employee Retirement Systems (IPERS) or the Social Security Administration (SSA) approves disability retirement. 5. Each application for a grant from the Bank must include a new treating physician statement on the appropriate WEA Sick Leave Bank Request form.
Use of Bank Leave Days. 36 A maximum of 30 sick leave bank days per year may be used for a single, extended illness 37 after an employee’s sick leave and PTO leave (when provided by the contract) has been 38 depleted. This limitation may be extended with the approval of a Sick Leave Bank Board 39 consisting of 2 representatives for the School District and 2 representatives for the Association. 40 Use of sick leave bank days will be based on a per person and per day basis, e.g. everyone 41 eligible will draw each day until the total sick bank leave days have been exhausted. 3 TEACHER SALARIES 5 TEACHER SALARY SCHEDULE: The salary schedule for each teacher is set forth in Exhibit E, which 6 is attached hereto and made a part hereof.

  • Retirement Date If the Executive remains in the continuous employ of the Bank, the Executive shall retire from active employment with the Bank on the Executive’s sixty-fifth (65th) birthday, unless by action of the Board of Directors this period of active employment shall be shortened or extended.

  • Christmas or New Year's Day Off The Employer agrees to make every reasonable effort to ensure that employees required to work shift shall have at least Christmas Day or the following New Year's Day off.

  • Abuse of Leave If it is found that a leave is not actually being used for the purpose for which it was granted, the Employer may cancel the leave and direct the employee to report for work by giving written notice to the employee.

  • Personal Leave Days Section 1. All employees after completion of six (6) months of service shall be entitled to receive personal leave days in the following manner: (A) All full time employees shall be entitled to twenty-four (24) hours of personal leave with pay each fiscal year; (B) Part-time, seasonal, and job share employees shall be granted such leave in a prorated amount of twenty-four (24) hours based on the same percentage or fraction of month they are hired to work, or as subsequently formally modified, provided it is anticipated that they will work 1,040 hours during the fiscal year; Section 2. Should any employee fail to work 1,040 hours for the fiscal year, the value of personal leave time used may be recovered from the employee. Section 3. Personal leave shall not be cumulative from year to year nor is any unused leave compensable in any other manner. Section 4. Such leave may be used by an employee for any purpose he/she desires and may be taken at times mutually agreeable to the university and the employee.

  • Application for Leave Any request for a leave of absence shall be submitted in writing by the employee to the Employer or designee. The request shall state the reason the leave of absence is being requested and the length of time off the employee desires. Authorization for a leave of absence shall be furnished to the employee by the Employer or designee, and it shall be in writing.

  • Recall from Vacation Leave When during any period of vacation leave an employee is recalled to duty, he shall be reimbursed for reasonable expenses, as normally defined by the Employer, that he incurs:
