Health and Hospitalization. The Town shall provide bargaining unit members with the choice to enroll in either the Lumenos plan or the AB15IPDED-RX 10/20/45 plan provided by HealthTrust for single, two person, or family coverages. Employees will provide to the Town, through equal weekly payroll deductions, an insurance premium cost sharing of the following amounts of the total monthly premium for that employee’s level of coverage: 12.5% for the Lumenos plan and 12.5% for the AB15IPDED plan.
Health and Hospitalization. The School District will contribute to the premium cost of the group health and hospitalization insurance as follows:
Health and Hospitalization a) The Board will provide the opportunity to participate in health and hospitalization insurance for all eligible teachers and eligible dependents. The method of establishing the premium payments, stabilization funds, and related financial decisions are for the review and decision of the Board with recommendations by the Insurance Committee. The District contribution to the total cost of the plan will be no less than 60%. For example, in 2018 the District contributed approximately 6 million when the total plan cost approximately 10 million. The current members that are now “pooling” will be grandfathered under the past system. Health and Hospitalization. 20.1 (1) Effective January 1, 2000, the County shall pick up the premium payment for the Building Trades Plan in the following ratio: Tier County Employee Employee 95% 5% Employee + 1 94% 6% Employee + Family 92% 8% Increases and decreases in the Building Trades Plan will be shared between the County and employee in the same ratio as above.

The parties have mutually agreed that the health and hospitalization insurance program shall be Plans I/II, III and IV (High Deductible Health Plan) provided by the Hoosier School Benefit Trust with the provisions in effect as of the date of this agreement. Enrollment in the corporation health and hospitalization plan will be closed except for qualifying events an annually established open enrollment period and as required by the Patient Protection and Affordable Care Act. If an employee’s spouse is eligible for health insurance at his/her place of employment, the spouse will be required to enroll in his/her employer’s health insurance plan if the employer pays at least sixty percent (60%) of the single premium. The spouse’s plan will then become primary for all his/her health insurance claims. It is understood that the MSDDT PPO will be secondary for payment of claims. If the employee elects to continue secondary coverage for his/her spouse or dependents, the employee will be responsible for paying the additional premium required to provide this coverage.
Health and Hospitalization. The School District shall provide the Director with a family health insurance policy under the School District's plan, with a defined contribution not to exceed $19,500. In the event that the Director decides to take a high deductible family health insurance plan, the district will contribute up to $4000 into an HSA or VEBA plan, however, the amount of contribution towards the health insurance plan will be reduced by the contribution amount of the HAS or VEBA contribution. At no time will the cost of the plan and the contribution to the HSA/XXXX together, exceed $19,500. * In the event this Contract will cause or does cause penalties, fees, or fines to be assessed against the School District, the parties agree to reopen negotiations that result in a revised Contract between the parties that eliminates or reduces penalties, fees, or fines to be assessed against the School District. The amount of any reduction in the School District’s contribution toward the employee’s healthcare benefits as a result of addressing the “highly compensated employee” component of the ACA will be placed into another School District provided benefit(s) (i.e., a retirement HRA, salary, etc.) as agreed upon between the parties.
Health and Hospitalization. 1. Subject to the applicable waiting period, as defined by law, under the Employer's health insurance plan, the Employer shall pay all but Forty-Six and No/100 Dollars ($46.00) of such employee's single rate monthly health insurance premium under the Employer's health insurance plan. Such employee shall be responsible for paying the remaining Forty-Six and No/100 Dollars ($46.00) of the single rate monthly health insurance premium. Nurses desiring family coverage will also pay the difference between the individual contract premium and the family premium. The Employer shall contract with a health insurance carrier to provide group health insurance benefits for the nurses as well as other employees of the Employer. The health insurance carrier shall be chosen at the discretion of the Employer. The parties hereto recognize that upon entry into such a contract between the Employer and the health insurance carrier, the health plan provided shall have certain contractual rules contained therein. In the event that the said contractual rules shall conflict with any provision of this Agreement, the contractual rules shall prevail.
