E ligibility. Participation in medical, vision, dental and any other insurance benefits shall be subject to specific plan eligibility requirements. Beginning the first date of employment, employees regularly scheduled to work twenty (20) or more hours per week (0.5 FTE) shall be eligible for enrollment in the Employer's medical, vision and dental benefit plans. For a job change in which the employee becomes newly benefits-eligible (0.5 FTE or higher), eligibility for the Employer’s medical, dental, and vision benefits shall begin with the date of the employee’s job change.

E ligibility. In order to register for the Sim, you must be an adult according to the laws of the jurisdiction (i.e. state or country, as the case may be) in which you reside. If you are not an adult, if you wish to use the Sim you must have a parent or guardian register for the Sim on your behalf. By registering for the Sim, you are making a representation to iRacing that you are an adult in the jurisdiction in which you reside and that you agree to this Agreement on behalf of yourself and, at your discretion, for one (1) minor child not under the age of 13 for whom you are a parent or guardian and whom you have authorized to use the account you create to use the Sim. In the event that you register a minor child not under the age of 13 to use the Sim, you hereby agree to this Agreement on behalf of yourself and such minor, and you understand and agree that you will be responsible for all uses of the Sim by the minor you register, whether or not such uses were authorized by you. In any event, the Sim is not designed for minors under the age of 13, so minors under the age of 13 are not permitted to use the Sim. Notwithstanding the foregoing or anything to the contrary in this Agreement or the Privacy Policy, you hereby acknowledge and consent that, if the information of any user of the Sim who is a minor (not under the age of 13) (a “Minor User”) is submitted or collected in connection with such Minor User’s use of the Sim (including through a requested account name change or otherwise), and including without limitation such Minor User’s name, photograph, audio/visual recording, and other information described in the Privacy Policy, all such information may be used and processed by xXxxxxx.xxx and the Promoters (as defined below) in accordance with the terms of this Agreement and the Privacy Policy. Without limitation of the foregoing, you hereby expressly grant consent to the Promoters to: (a) process and disclose such Minor User’s and/or your information and Likeness (as defined below) in accordance with the Privacy Policy; (b) transfer such Minor User’s and/or your information and Likenesses throughout the world, including to the United States or other countries that do not ensure adequate protection for personal information (as determined by the European Commission); and (c) disclose such Minor User’s and/or your information and Likenesses to comply with lawful requests by public authorities, including to meet national security or law enforcement requirements.

Per diem RN’s must first meet their minimum shift within the schedule before becoming eligible for an incentive shift. a. Per diem RN’s must meet the CBA definition of a covered or represented per diem registered nurse in order to qualify 5. Incentive pay only applies to those shifts designated by each unit/department manager. Designating a shift(s) to be incentive eligible and identifying the number of incentive shifts are rights reserved to management. The designation of a given shift on a specific date as eligible for incentive pay applies only to that shift and is not a permanent designation. 6. Employees must have completed their probationary period or orientation to the department/unit before becoming eligible for the incentive. 7. An employee calling out sick for an incentive shift will provide notice to their unit/department manager following standard unit/department practice. Failure to do so may result in ineligibility for future incentive shifts.
E ligibility. Provider agrees to immediately notify Plan in the event Provider or any Provider Affiliate is or becomes excluded from participation in any federal or state health care program under Section 1128 or 1128A of the Social Security Act. Provider also will not employ or contract for the provision of health care services, utilization review, medical social work or administrative services, with or without compensation, with any individual or entity that has been excluded from participation in any federal or state health care program under Section 1128 or 1128A of the Social Security Act. E ligibility. I&M determines eligibility of Customers at its sole discretion. I&M may request verification of eligibility requirements at any time during the Program period. Customer agrees to allow the installation of all LCS’ provided by I&M under this Program; provided, however, that if the Customer does not install all LCS’, then it shall return any uninstalled LCS’ to I&M.
E ligibility. The retirement incentive program is available to SDEA Unit members who meet the following criteria: a. Are age 55 or more and have at least 15 years of service to the District as of December 31, 2020 and, b. Notify the District of their intent to participate in the retirement incentive within the defined timeline outlined in this agreement; and, c. Complete the necessary CalSTRS or CalPERS paperwork, including resignation form for purposes of retirement. d. Are eligible to be covered under a District-sponsored group medical benefit plan as an employee immediately prior to the effective date of retirement with CalSTRS or CalPERS. Any employee who is currently benefits eligible, but waiving coverage is eligible for the incentive program.

E ligibility. 1. The Employer reserves the right to limit the number of employees to no more than four percent (4%) of the total number of staff in a given executive division. 2. No more than one Educational Leave may be granted for the same work unit. 3. The employee must be a full-time staff person who has completed two (2) or more continuous years of service with the Franklin County Department of Job and Family Services. 4. The employee shall be in good standing with the Agency and have no discipline for one (1) year prior to the request for educational leave. 5. The school of application must be an accredited institution. 6. There must be a correlation between the content of the courses (or degree program) in either the employee’s current job or other jobs within the agency.
E ligibility. Supervisors who have completed at least 10 years of uninterrupted service as a supervisor to the School District shall be eligible for the benefits described in this section. If, at the time of the Supervisor’s resignation, the School District’s total contributions to the Supervisor’s 403(b) account is not $30,000, the School District shall contribute the difference between $30,000 and the actual amount of matching contributions made to the Supervisor’s 403(b) account in three equal payments as follows: (1) on the first monthly payroll after resignation; (2) on the first January payroll after resignation; and (3) on the January payroll one year from the previous payment. If a member dies and is eligible for severance pay pursuant to this section at the time of death, the severance payment shall be made to his/her beneficiary or estate to the extent the member would have received such payments under the terms of this agreement.
