The supervisor is to submit at that time the evaluation which determines the non-probationary status. 2. The written evaluation, to be placed in the personnel file, will be discussed at a joint conference between the supervisor and employee to be held within 3 days. 3. Any areas in which "Needs Attention" or "Unsatisfactory" are indicated must be followed by written comments explaining the deficiency and recommendations for helping the individual become effective. 4. A copy must be given to the staff member upon completion of each evaluation conference. 5. All employees will be evaluated a minimum of once annually.
